Lipid peroxidation products, and vitamin and trace element status in patients with cancer before and after chemotherapy, including adriamycin. A preliminary study.

Adriamycin is a potent chemotherapeutic agent used in the treatment of human neoplastic diseases. A major side effect limiting the use of this drug is its toxic effect on the heart. Several hypotheses have been proposed to explain the cardiotoxicity of Adriamycin. However, the most plausible hypothesis seems to be the reduction of Adriamycin and free radical production, which induces lipid peroxidation and oxidative damages in the heart. We have thus undertaken this preliminary study to investigate Adriamycin-induced lipid peroxidation by the measurement of plasma thiobarbituric acid reactant materials and antioxidant systems, namely glutathione content, glutathione peroxidase activity, and vitamin and trace element status, in patients with cancer before and after chemotherapy, including Adriamycin. The concentration of thiobarbituric acid reactant materials in plasma of patients with cancer was higher than in controls and was further increased after chemotherapy. Blood glutathione and plasma glutathione peroxidase activity, as well as plasma zinc and selenium in patients with cancer, were decreased, but not further modified by chemotherapy. However, only zinc and selenium levels reached a significant level. In contrast, plasma vitamin E and beta-carotene levels were not significantly increased in patients with cancer. Finally, plasma vitamin A and copper levels were not modified either in patients with cancer or by chemotherapy.

Caring for day care: a pilot project.

Day care in Israel is not always of high quality, and the current forecast for upgrading the standards of the existing centers is bleak, due to economic and social factors that do not place high priority on quality infant care in group settings. The agency attempted, therefore, to develop a low-cost, efficient method of early intervention, wherein skilled social workers were hired, given special training, and worked alongside the caregivers of day care centers to help children and parents defined as at risk. This article describes the observations that were made and the interventions that were chosen.

[Analysis of prescriptions for psychotropic drugs in a psychiatric hospital].

A study on the prescription of psychotropic drugs was carried out at St-Egreve Psychiatric Hospital (Isère,--France) based on in-patient prescriptions (adults only). Analysis of the 200 prescriptions issued by 26 different practitioners found an average of 3.3 psychotropic drugs per patient (including 1.7 neuroleptics). A combination of neuroleptics was very frequent (63 pour cent of the schizophrenics receiving at least 2 neuroleptics). Half of the patients treated with neuroleptics received antiparkinsonian drugs. The other psychotropic medications often combined with neuroleptics, were anxiolytics (50 per cent), hypnotics (45 per cent), antidepressants (28 per cent) and normothymics (25 per cent). Among the antidepressants, the SSRI were the most commonly prescribed, even above tricyclics, reflecting their better clinical tolerance. The significant differences in prescribing practice between the services prompted the creation of a working group comprising practitioners and pharmacists to study the prescription of psychotropic drugs and compare this with the literature.
