Effect of a prostaglandin F2 alpha injection 96 hours after introduction of intact bulls on estrus and calving distribution of beef cows.

Most estrus synchronization schemes facilitate the use of artificial insemination; however, combining estrus synchronization with natural service also offers several benefits to beef producers. This experiment was designed to study the effectiveness of one injection of prostaglandin F2 alpha (PGF2 alpha) 96 h after introduction of intact bulls on estrus synchronization and calving distribution of beef cows and heifers. Three hundred sixty beef cows and 45 beef heifers in seven trials were allotted by age, breed, and prior calving date to one of two treatments. Within each subclass, one group was injected with an appropriate dose of PGF2 alpha and the second group at each location received 5 ml of saline (SAL). Cows were classified as estrual or anestrual at the start of breeding based on serum progesterone levels and were observed for behavioral estrus regularly during the first 25 d of the breeding season in two of the trials (d 0 = day before bull turn-in). Pregnancy data and calving dates were recorded in each of the studies. All cows were palpated per rectum for pregnancy approximately 70 d after the introduction of bulls. During d 5 to 9 after treatment, the percentage of cows exhibiting behavioral estrus (P = .02) and the pregnancy rate (P = .02) obtained was greater for PGF2 alpha-treated cows than for SAL-treated cows. There were 6.5% more (P = .05) calves born during the overall calving period from animals treated with PGF2 alpha compared with those in the SAL group. In summary, treatment of cows and heifers with PGF2 alpha 96 h after bull turn-in was effective in synchronizing behavioral estrus of beef cows and heifers and increasing the calving rate during the subsequent calving season.

Evaluation of click-evoked oto-acoustic emissions in the newborn.

Click-evoked oto-acoustic emissions and auditory brainstem responses (ABR) have been recorded in 723 babies taken largely from a neonatal intensive care unit to evaluate the use of oto-acoustic emissions as a method of screening for hearing impairment. Twenty-nine infants failed to pass the initial ABR test and repeat ABR tests carried out up to the age of 3 months. The 'sensitivity' and 'specificity' of the oto-acoustic emission test for the ABR test results up to 3 months of age in surviving infants were 93% and 84% respectively. It is proposed that click-evoked oto-acoustic emissions be considered as the initial method to screen for hearing impairment, test failures being followed up by ABR.

Differential regulation of interleukin-1 alpha and interleukin-1 beta mRNA expression in human monocytes: evidence for protein kinase C-dependent and -independent pathways.

The expression of mRNA coding for IL-1 alpha and IL-1 beta was examined in human peripheral blood monocytes (PBM) to determine if the two genes are under the same mechanisms of transcriptional control and whether or not they can be regulated independently. In response to E. coli lipopolysaccharide (LPS), PBM express approximately 10-fold more IL-1 beta-specific mRNA than IL-1 alpha. However, treatment of these cells with phorbol myristate acetate (PMA) resulted in the expression of IL-1 beta mRNA. Likewise, treatment of PBM with phorbol dibutyrate (PdBu), phorbol diacetate (PDA), or mezerein, which, similar to PMA, were able to induce the translocation of protein kinase C (PKc) to the monocyte plasma membrane, resulted in predominantly IL-1 beta mRNA expression. The inactive tumor promoter 4 alpha-phorbol didecanoate (4 alpha-PDD) did not cause the translocation of PKc or induce the expression of either form of IL-1 mRNA. Following 18 h pretreatment with PMA to downregulate PKc activity, LPS was capable of inducing the expression of both forms of IL-1 mRNA, demonstrating that at least part of the response of PBM to LPS is PKc independent. These results suggest that the activation of PKc alone is sufficient to induce a high level expression of IL-1 beta but not IL-1 alpha mRNA. Furthermore, the possibility exists that another, as yet unknown, signal transduction mechanism is involved in inducing the expression of both IL-1 alpha and IL-1 beta mRNA in response to LPS.

The biological role of naturally-occurring cytokine inhibitors.

Four different mechanisms of cytokine inhibition might be involved in regulation of cytokine effects in vivo. Different cytokines may exhibit opposing biological effects on a specific target cell or in a particular disease. Autoantibodies to cytokines may block cytokine effects in vivo or may function as carriers to deliver cytokines to tissues. Soluble receptors of cytokines, particularly for IL-2 and TNF alpha, may be released by cell activation. Lastly, a specific receptor antagonist of IL-1, IL-1ra, is synthesized by human monocytes and macrophages, particularly under the influence of GM-CSF. IL-1ra is the first described naturally-occurring receptor antagonist of any cytokine or hormone-like molecule. It is not yet known whether IL-1ra is produced in tissues in human diseases as an endogenous anti-inflammatory factor. Whether any of these potential mechanisms to regulate cytokine effects will be of value in the treatment of human diseases remains to be determined.

Prescriptions for saying good-bye: teaching termination to family practice residents.

Physicians terminate doctor-patient relationships at various times during their careers. Preparing physicians for this process should be part of residency training in family practice. This report describes the structured role-playing exercises and physician scripts used to teach termination skills. Evaluation of these teaching methods suggests that residents value termination instruction, prefer participative approaches, and report greater sensitivity to the feelings and issues evoked by termination.

Pharmacokinetics of oral and transdermal triprolidine.

In this open, nonrandomized, three-way crossover study, six healthy male volunteers received single doses of triprolidine (TPL) hydrochloride syrup orally (2.5 mg) and wore transdermal TPL patches (5 mg and 10 mg doses) to compare the pharmacokinetic profiles and dose tolerance of the two formulations. A washout period of at least 1 week was scheduled between the three dosing periods. Blood samples were collected at defined times, and plasma concentrations were determined using a radioimmunoassay. Maximum plasma drug concentration (Cmax) decreased from 5.6 +/- 2.9 ng/mL (mean +/- SD) with oral dosing to 2.0 +/- 1.0 ng/mL and 4.2 +/- 2.0 ng/mL following 5 mg and 10 mg transdermal doses, respectively. Time to reach peak concentration (tmax) increased from 2.0 +/- 1.2 hours with oral dosing to 12.0 +/- 5.9 and 14.3 +/- 9.9 hours following 5 mg and 10 mg transdermal doses, respectively. The differences between AUC0-alpha values with the oral syrup and the 5 mg and 10 mg transdermal doses were not significant when normalized to 2.09 mg (TPL base). The bioavailabilities of the 5 mg and 10 mg transdermal doses relative to the oral 2.09 mg doses were 0.89 +/- 0.32 and 1.04 +/- 0.33, respectively. Mild erythema and pruritus were the most common adverse effects secondary to TPL transdermal application. Drowsiness observed following oral TPL, was not evident following either transdermal dose. The results of this study, therefore, indicate that TPL can be absorbed transdermally, providing consistent plasma concentrations.

Parturition in beef cows following administration of porcine relaxin at ten days prepartum.

Administration of procine relaxin (pRLX) to heifers 5 d prepartum has been reported to expedite parturition. Thirty-eight mature crossbred beef cows were randomly assigned to one of three treatment groups. Control animals (C; n = 13) received an intramuscular (i.m.) injection of 2 ml corn oil and 2 ml i.m. phosphate-buffered saline (PBS) 24 h later; relaxin treated animals (RLX; n = 13) received 2 ml i.m. corn oil and 1.0 mg i.m. pRLX 24 h later; estradiol-relaxin treated animals (E-RLX; n = 12) received 20 mg i.m. estradiol benzoate (EB) and 1.0 mg i.m. pRLX 24 h later. Treatment with pRLX occurred at 272.6+/-0.14 d of gestation. The pRLX had been purified to homogeneity from porcine ovaries collected during late pregnancy and was determined to have >/=3000 U/mg by the mouse interpubic ligament bioassay. Peripheral blood samples were collected from all cows at 0, 4, 8 and 24 h, respective to corn oil or EB administration, and assayed for plasma estradiol-17beta. At 24 h post administration of EB, plasma estradiol-17beta concentrations were 48.0+/-10.5 pg/ml for C and RLX cows and 178.5+/-14.8 pg/ml for E-RLX cows. There were no treatment effects (P>/=0.10) for elapsed time from treatment to parturition (304.2+/-22.4 h), gestation length (285.2+/-0.9 d), calving difficulty score (1.05+/-0.04), calf vigor score (1.05+/-0.04) or calf birth weight (38.0+/-0.88 kg). Additionally, there were no retained placental membranes in any cows. Administration of pRLX intramuscularly to beef cows at 10 d before expected parturition was not effective in inducing premature parturition. Furthermore, the effectiveness of pRLX in inducing parturition was not enhanced by pretreatment with estradiol benzoate.

Radial density distribution of chromatin: evidence that chromatin fibers have solid centers.

Fiber diameter, radial distribution of density, and radius of gyration were determined from scanning transmission electron microscopy (STEM) of unstained, frozen-dried chromatin fibers. Chromatin fibers isolated under physiological conditions (ionic strength, 124 mM) from Thyone briareus sperm (DNA linker length, n = 87 bp) and Necturus maculosus erythrocytes (n = 48 bp) were analyzed by objective image-processing techniques. The mean outer diameters were determined to be 38.0 nm (SD = 3.7 nm; SEM = 0.36 nm) and 31.2 nm (SD = 3.6 nm; SEM = 0.32 nm) for Thyone and Necturus, respectively. These data are inconsistent with the twisted-ribbon and solenoid models, which predict constant diameters of approximately 30 nm, independent of DNA linker length. Calculated radial density distributions of chromatin exhibited relatively uniform density with no central hole, although the 4-nm hole in tobacco mosaic virus (TMV) from the same micrographs was visualized clearly. The existence of density at the center of chromatin fibers is in strong disagreement with the hollow-solenoid and hollow-twisted-ribbon models, which predict central holes of 16 and 9 nm for chromatin of 38 and 31 nm diameter, respectively. The cross-sectional radii of gyration were calculated from the radial density distributions and found to be 13.6 nm for Thyone and 11.1 nm for Necturus, in good agreement with x-ray and neutron scattering. The STEM data do not support the solenoid or twisted-ribbon models for chromatin fiber structure. They do, however, support the double-helical crossed-linker models, which exhibit a strong dependence of fiber diameter upon DNA linker length and have linker DNA at the center.

The diameters of frozen-hydrated chromatin fibers increase with DNA linker length: evidence in support of variable diameter models for chromatin.

The diameters of chromatin fibers from Thyone briareus (sea cucumber) sperm (DNA linker length, n = 87 bp) and Necturus maculosus (mudpuppy) erythrocytes (n = 48 bp) were investigated. Soluble fibers were frozen into vitrified aqueous solutions of physiological ionic strength (124 mM), imaged by cryo-EM, and measured interactively using quantitative computer image-processing techniques. Frozen-hydrated Thyone and Necturus fibers had significantly different mean diameters of 43.5 nm (SD = 4.2 nm; SEM = 0.61 nm) and 32.0 nm (SD = 3.0 nm; SEM = 0.36 nm), respectively. Evaluation of previously published EM data shows that the diameters of chromatin from a large number of sources are proportional to linker length. In addition, the inherent variability in fiber diameter suggests a relationship between fiber structure and the heterogeneity of linker length. The cryo-EM data were in quantitative agreement with space-filling double-helical crossed-linker models of Thyone and Necturus chromatin. The data, however, do not support solenoid or twisted-ribbon models for chromatin that specify a constant 30 nm diameter. To reconcile the concept of solenoidal packing with the data, we propose a variable-diameter solid-solenoid model with a fiber diameter that increases with linker length. In principle, each of the variable diameter models for chromatin can be reconciled with local variations in linker length.

Changes in alveolar-arterial oxygen partial pressure difference during orthotopic liver transplantation.

Changes in the alveolar-arterial oxygen partial pressure difference (PAO2-PaO2) were measured in 39 patients undergoing orthotopic liver transplantation without veno-arterial or veno-venous bypass. The operation can be divided into an initial dissection phase, an anhepatic phase when the hepatic artery, portal vein and vena cava are clamped, and a post-anhepatic phase after the vascular clamps are released. There was an initial increase in (PAO2-PaO2) during the dissection phase, followed by an immediate decrease when the liver was removed. This decrease continued throughout the anhepatic period, but a further increase in (PAO2-PaO2) occurred after release of all the vascular clamps and during abdominal closure.

Click evoked otoacoustic emissions in neonatal screening.

Seven hundred and twenty-three neonates under intensive care have been tested by evoked otoacoustic emissions (EOAE) and the auditory brain stem response (ABR) to investigate the use of EOAE as a test for hearing impairment. Three hundred and thirty-one have had follow-up tests to the age of at least 2 years. The EOAE test has been found to be practical and quick to perform. The proportion of NICU infants producing a recordable EOAE is 80%, and the sensitivity and selectivity to the ABR result in the period up to 3 months post due date is 93 and 84%, respectively. These figures are high enough and the reduction in time compared to ABR is sufficient for the EOAE to be considered as the primary screen. The follow-up data show mixed results with both false positives and false negatives present. The incidence of severe hearing impairment is close to that expected from retrospective studies at 2 in 331 (1 bilateral, 1 unilateral). Firm conclusions on the sensitivity of EOAE to long-term hearing impairment await the results from larger numbers of infants and further follow up data.

Low dose methotrexate and doxorubicin in hormone-resistant prostatic cancer. Scottish Urological Oncology Group.

A group of 24 patients, all of whom had progressive disease following initial hormone therapy for advanced prostatic carcinoma, were given intravenous methotrexate (30 mg/m2) and doxorubicin (15 mg/m2) at 2-week intervals for a planned 6-treatment course. Seventeen patients completed the full course. Assessment included "quality of life" criteria, using the subjective assessment of performance status, severity of pain and analgesic consumption. Over half of the patients improved in each of these modalities and 8 (33%) improved in all 3. As toxicity was acceptable, low dose chemotherapy may have a part to play in these patients who present a difficult management problem.

The impact of health care provider market served on health care channel relationships.

A conceptual framework is provided in which manufacturer-wholesaler relationships are addressed in the context of the health care provider segment served by the wholesaler. A study of the medical supply channel demonstrates how the particular market served by the wholesaler shapes the manufacturer-wholesaler relationship along the behavioral dimensions of manifest conflict, coercion, satisfaction and openness of communications. The health care provider segment served was found to have a significant impact on these dimensions. The results of this research suggest that channel member strategies, policies and operating procedures must account for variations in input (supply) and output (market) environments.

The effect of stimulus level on click evoked oto-acoustic emissions and brainstem responses in neonates under intensive care.

Click evoked oto-acoustic emissions (EOAE) at a range of stimulus levels and the auditory brainstem response (ABR) threshold have been measured in 40 infants admitted to a neonatal intensive care unit (NICU). The stimulus levels at which the emissions were first observed had a similar mean value and distribution to those found in previous studies on normal newborns and young adult volunteers. The difference was in the proportion who had no detectable emissions, 16.7% as against 3.9% in the normal newborns and 2.6% in young adult volunteers. The distribution of thresholds for the ABR was again similar to those measured on normal newborns but 8.1% of the ears of the NICU infants had a threshold worse than 53 dBnHL compared with 0% for the normal newborns. A combined screening method starting with the click evoked oto-acoustic emission is proposed. A comparison of the mean ABR thresholds for those in whom an EOAE was recorded to those with no EOAE showed a group effect. The stimulus latency relationship of the NV peak showed too much variation between individual results to consider its use to define accurately the degree of conductive hearing loss.

Effect of nordihydroguaiaretic acid on luteal phase length and oxytocin-induced release of prostaglandin F2 alpha in heifers.

Two experiments were conducted to determine if intrauterine infusion of nordihydroguaiaretic acid, a lipoxygenase pathway inhibitor, would delay luteolysis (Experiment 1) and inhibit oxytocin-induced release of prostaglandin F2 alpha (as measured by the stable prostaglandin F2 alpha metabolite, 15-keto-13,14-dihydroprostaglandin F2 alpha) in plasma on d 16 (d 0 = estrus) of the estrous cycle (Experiment 2). Nordihydroguaiaretic acid (20 mg) or saline was infused twice daily into the uterus on d 14 to 23 (Experiment 1) or d 14 to 20 (Experiment 2) postestrus, respectively. In Experiment 1 and 2, mean concentration of progesterone was higher and luteolysis was delayed in nordihydroguaiaretic acid-infused heifers compared with saline-infused heifers. In Experiment 2, saline or oxytocin (100 IU, i.v.) was injected into each heifer on d 16 postestrus to stimulate the release of prostaglandin F2 alpha from the uterus. Mean concentration of 15-keto-13,14-dihydroprostaglandin F2 alpha increased within 1.5 h postinjection in heifers infused with saline, whereas concentration of 15-keto-13,14-dihydroprostaglandin F2 alpha in nordihydroguaiaretic acid-infused heifers did not increase within the same time period. Thus, nordihydroguaiaretic acid may inhibit both the lipoxygenase and cyclooxygenase pathways of arachidonic acid metabolism and therefore delay luteolysis.

Renal trauma. Adult and pediatric considerations.

Care of the patient with renal trauma presents a special challenge for emergency and critical care nurses. By following the standards of trauma nursing care, nurses can detect this often masked injury. Careful ongoing assessment of these patients can help to prevent serious short- and long-term complications. A working knowledge of the clinical sequelae of renal trauma facilitates the achievement of this goal.
