[Multiple sclerosis: clinical analysis and development of 214 cases].

The authors report on a series of 214 cases of defined multiple sclerosis according to Poser et al. criteria. These cases were retrospectively selected by medical reports analysed from Santa Casa Medical School (São Paulo) and the private practice, from 1980 to 1993. The data were analysed as for sex, race, onset age of symptoms, onset symptoms, and evolutive symptoms. The results were compared to those found in other Brazilian series and in foreign series. This comparative analysis allows to verify similarities and differences among the several series, and the authors emphasize the necessity of multicentric studies in Brazil to assert with more details the multiple sclerosis profile in our country.

Continuous intrapartum fetal monitoring: a prospective randomized comparison of 2 types of fetal scalp electrode.

This trial was conducted to compare 2 commonly used fetal scalp electrodes with regard to ease of use, frequency and extent of neonatal injury and quality of cardiotocographic record. A randomized design was employed to study a group of 106 patients divided between a Surgicraft Copeland clip fetal scalp electrode (52 patients) and a Meditrace spiral single helix scalp electrode (54 patients). Patients were eligible for trial entry if they required an intrapartum fetal scalp electrode, at term with a singleton cephalic pregnancy. Ease of application was rated by the operator using a linear analogue score. Unidentified traces were reviewed independently for quality by 2 obstetricians and neonates were examined on day-2 postpartum for injury. The Meditrace spiral fetal scalp electrode was significantly easier to apply (unpaired t-test p < 0.02). It also obtained higher ratings for trace quality (unpaired t-test p < 0.02). There were no serious neonatal injuries and no difference was found between the 2 electrodes in this regard.

[Lead poisoning: Zinc protoporphyrin in blood of children from Santo Amaro da Purificação, Bahia, Brazil].

A cross-sectional epidemiological study has investigated lead poisoning among all the 101 children aged 1 to 5 years, living at less then 500 m from a lead smelter which has been operating since 1960 in Santo Amaro da Purificação, State of Bahia, Brazil. Lead poisoning was evaluated by determining the concentrations of zinc protoporphyrin (ZPP) in whole blood. Results were compared with those obtained from 98 children of the same age range, in a nursery from Salvador, capital of the State of Bahia. In Santo Amaro, ZPP geometric mean was 65.5 mg/dl (geometric S.D. = 1.7), much higher, therefore, than the normal threshold of 30 microg/dl recommended by the CDC-USA. A child was excluded from statistical analyses because of very high ZPP level: 789 mg/dl. In Salvador, ZPP geometric mean was 31.0 mg/dl (geometric SD = 1.6). "Extremely elevated" ZPP levels (above 156 mg/dl) were observed in 8% of the children from Santo Amaro and in none (0%) of the children from Salvador, this statistical difference being highly significant. Anaemia was present among 35.0% of the children from Santo Amaro and among 25.5% of those from Salvador. One year-old children showed the highest levels of ZPP and anaemia. No marked statistical association was found between ZPP levels and anaemia. Lead contamination of this children population remained in a permanently high level during, at least, the period from 1980 to 1992. In December, 1993 the lead smelter has definitively ceased its industrial activities in Santo Amaro da Purificação.
