Rheological and thermal characteristics of gel structures from various agar fractions.

The dynamic rheological properties during gelation, phase transition temperatures and the enthalpy (deltaH) for melting of junction zones in gel networks of different agar fractions were investigated. The results indicated that both gelling (Tgel) and melting (Tm) temperatures, storage moduli (G') and the deltaH values of agar gels were mainly correlated with their [eta] values, rather than with 3,6-anhydro-galactose (3,6-AG) content. Effects of 3,6-AG content on increasing G' were observed only for the gels of low concentrations (e.g. 0.8 g/dl). The concentration dependences of structural strength and structure development rate, which were presented by G' and dG'/dt respectively, were found to decrease with increasing 3,6-AG content or [eta] value. It is noteworthy that influencing extents of 3,6-AG content and [eta] on the above rheological and thermal properties vary not only with the concentration examined, but also with the stage of gelation process.

Microlymphatic bypass in obstructive lymphoedema.

A small series of patients with obstructive or secondary lymphoedema treated by microlymphatic bypass is presented. The procedures in the lower and upper limbs are described. Preoperative assessment by lymphangiography and lymphoscintigraphy are required to assess suitability for the procedure. Postoperatively, patent lymph collectors can be demonstrated by lymphoscintigraphy. Microlymphatic bypass must be carried out before the peripheral lymph collectors have been destroyed or permanently damaged by increasing back pressure and recurrent infection. Factors influencing a successful result are enumerated. Other microsurgical techniques and treatment are reviewed.

The extended deep inferior epigastric flap: a case report.

The extended deep inferior epigastric flap, described by Taylor et al. (1983), 1984), offers a versatile and reliable technique for covering defects in the lower limb. A case of shark bite is described in which extensive soft tissue loss in the thigh resulted in a denuded femoral shaft which was successfully treated using such a flap.

Micro-lymphatic bypass in the treatment of obstructive lymphoedema of the arm: case report of a new technique.

A report is presented of a case of secondary lymphoedema of the upper limb successfully treated by microlymphatic bypass with a composite graft containing three lymphocollectors from the lower limb to bypass the ablated portion of the pathway. To ensure success the surgery must be carried out before the peripheral lymphatics have been damaged or destroyed by increasing pressure and recurrent infection. Pre-operative lymphangiography is mandatory to assess suitability for the bypass procedure. Other methods of treatment are reviewed.

The brachioradialis myocutaneous flap.

The vascular anatomy and the cutaneous territorial definition of the brachioradialis myocutaneous flap has been studied by cadaver dissection and intra-arterial injection of methylene blue eye. A case is presented in which a proximal brachioradialis myocutanenous flap, based on the vascular pedicle arising from the radial recurrent artery just distal to the elbow level, was used to cover an exposed open elbow joint. The advantages and disadvantages of this flap are briefly discussed.

Degloved sole and heel.

A case of a degloving injury of the sole and heel is reported. A major problem with this rather unusual injury is resurfacing the defect, as it is a weight-bearing area. The management of the injury with a microvascular free flap is presented and discussed.
