[Mucocutaneous lentiginosis, ephelides and cardiac myxoma].

Abnormalities of skin pigmentation are known to be associated sometimes with cardiovascular diseases. Such cases have been reported first in 1954 (14), then in 1962 (16) and 1966 (11), leading to the individualization of the leopard syndrome. In 1973 Rees et al. (21) described a lentiginosis, cardiac myxoma association, and this in turn resulted, in 1985, in a new syndrome (4) supported by several similar findings. We report here a new case with this association.

Spontaneous chromosomal anomalies in lymphocytes from xeroderma pigmentosum. A study of ten patients.

A cytogenetic study of the lymphocytes from 6 classic and 4 variant forms of xeroderma pigmentosum is reported. This study performed on 978 R-banded metaphases shows that there is no specific chromosomal rearrangement in this disorder. In UDS-deficient forms, the rates of deletions, chromatid gaps and chromosome gaps are significantly increased. The preferential involvement of G-bands is discussed.

[Presence of free retinol receptor (cRBP) and retinoic acid receptor (cRABP) in human skin tumors].

The concentrations of cellular retinoic acid binding protein (cRABP) and free cellular retinol binding protein (cRBP) were determined by ultracentrifugation in sucrose gradient in the cytosol of 41 human skin tumours (14 melanomas, 19 basal cell carcinomas, 8 squamous cell carcinomas). cRBP was found respectively in 36%, 42% and 37% of the studied samples. On the contrary, cRABP was more frequently found in carcinomas (89% in basal cell carcinomas and 100% in squamous cell carcinomas) than in melanomas (21%) (p less than 0.001). These results are discussed according to the different embryologic origin of carcinomas and melanomas. Furthermore, the better efficiency of synthetic retinoids in carcinomas than in melanomas should be explained by a different way of action in these 2 kinds of tumours.

[Malignant atrophic papulosis (Degos' disease): clinical review. Apropos of a case].

Malignant atrophic papulosis was first described by Degos in 1942. It is an occlusive arteriopathy involving small caliber vessels in the whole organism. Ocular involvement was reported in 35 out of the 105 observations published so far. The ophthalmological symptoms were reviewed and compared to a personal case. They consist of either involvement of the eye tissues with a predominance of the conjunctiva (but also sclera, episclera, retina, choroid and optic nerve), or of neuro-ophthalmological lesions. Up to now, both etiopathogeny and appropriate treatment of the disease remain unknown.

Infantile choriocarcinoma with cutaneous tumors. An additional case and review of the literature.

Choriocarcinoma is a malignant growth of trophoblastic cells characterized by the secretion of human chorionic gonadotropin. Primary choriocarcinoma arising in the placenta during a seemingly normal gestation is rare. Very few choriocarcinomas occurring simultaneously in mother and child have been reported so far. We describe an additional case of placental choriocarcinoma metastasizing to the newborn and showing many different cutaneous tumors. The primary tumor was found in the placenta. In the newborn, diagnosis was performed by skin biopsy only a few days after birth (by optic and electron microscopy). Immunohistochemical localization of human chorionic gonadotropin was performed by the immunoperoxidase technic with the use of monoclonal antibodies. This report describes an additional case and summarizes previously reported cases of placental choriocarcinoma metastasizing to the infant, as well as cases of skin metastases from malignant gestational trophoblastic disease.

[Estrogen and progesterone receptors in malignant melanoma, benign pigmented nevi and basocellular epithelioma].

45 malignant melanomas, 10 benign nevi and 17 basal cell carcinomas were investigated for estradiol (ER) and progesterone (PR) receptor activity by a single saturating-dose method (5 nM for ER and 10 nM for PR) using DCC. ER were found respectively in 7%, 10% and 24% of the studied samples and PR in 18%, 40% and 29% of the same samples. The low rates of ER+ and PR+ samples were discussed according to the data of the literature, more specially to the weak results obtained with anti-estrogenic therapy and to the possibility of tyrosinase to mimic the estrogen binding detected in melanomas.

[Peri-anal ulceration. Tuberculosis should not be overlooked].

Two cases of peri-anal ulceration of tuberculous origin are reported: one revealed an active pulmonary and ileo-caecal tuberculosis, the other complicated a chronic pulmonary tuberculosis of several years' duration. Cutaneous manifestations of tuberculosis are exceptional. In patients with protracted peri-anal ulceration, a biopsy should be performed that will show a typical tuberculoid granuloma. The most frequently encountered anorectal tuberculous lesions are suppurations and fistulae. The main differential diagnosis is Crohn's disease with anorectal manifestations.

An example of a pragmatic trial in the treatment of basocellular carcinoma of the skin.

A clinical trial is generally defined by a comparison of two or more treatments. This definition encloses two radically different viewpoints: problems of knowledge (explanatory approach) and problems of decision (pragmatic approach). The pragmatic approach is still not used very often; we describe a clinical trial using this approach in order to discuss its advantages and drawbacks; the trial compares radiotherapy and surgery in basal cell carcinoma of the skin. These two techniques are likely to give similar carcinological results, so that the therapeutic decision will have to take into account other criteria such as the aesthetic factor, the fonction and the cost not only in terms of money but also in terms of the psychological burden of the treatment, as side effects, etc. The choice of the weights to be attributed to these various criteria raises a problem, especially since the results of the trial may be different when varying the weights. This prompts a discussion of the possibility of extrapolating the therapeutic choice after the trial.
