[Microorganisms in home-care inhalation devices].

Contamination of inhalation equipment used at home, cleaning procedures, and the occurrence of potentially pathogenic micro-organisms in mouthwash water was studied in 55 patients. Fungi in the oropharyngeal space were found in 58%, Staphylococcus aureus in 29%, Enterobacteriaceae in 47% and Pseudomonas aeruginosa in 11%. Contamination with potentially pathogenic micro-organisms occurred in 64% of inhalation equipment and inhalation solutions. In a total of 151 samples from the equipment Klebsiella was present in 14%, Enterobacter in 10%, Serratia and Acinetobacter in 11% each, Pseudomonas in 5% and Staphylococcus aureus in 4.5%. In three cases the same serotype of Ps. aeruginosa was present in the oropharyngeal cavity and inhalation equipment. In most cases the cleansing of the inhalation equipment had been inadequate. Only 36% of those questioned cleaned the equipment immediately before use, only 4 of 20 stated that they washed their hands before use. Subjective assessment of the value of inhalation treatment was either "very good" or "good" in 80%.

[Microbial colonization of the walls of a copper pipeline in a central disinfection dosage unit].

Part of a copper-pipe taken from an 11 years old central disinfection dosage apparatus was investigated by Scanning Electron Microscopy. The pipe was covered by a thin paste-like coating. SEM revealed two distinct layers. In direct contact to the copper a layer of extracellular polymeric substance was seen. In direction to the luminary we found a second layer consisting of bacteria which were not embedded in extracellular polymeric substance. Some of the microorganisms of this layer showed holes. Bacteria in direct contact with disinfection solution showed a rough thickened surface indicating the existence of capsule substances.

[Induction of microbial growth on the walls of plastic tubes by drinking water].

Drinking water flowing through flexible plastic tubes (PVC) causes the development of a thin film-like coating within 24 h. At this stage microorganisms could either not or rarely be seen. After 48 h the number of microorganisms has become greater. With increasing time of experiment the microbial community becomes more dense until after 72 h it covers the whole surface. Now the microorganisms are embedded in extracellular polymeric substances. The primary film seems to be important for the development of microorganisms in equipment and apparatus.

[Scanning electron microscope demonstration of wall colonization of water-bearing plastic tubes].

A polyethylene and a silicone tube from water leading system of a dental unit, in which Pseudomonas aeruginosa could be isolated, were investigated by S.E.M. (Scanning Electron Microscopy). In the tubes thick deposits of extracellular material were found, in which microorganisms were embedded or on which they were free-lying. Pretreatment of the water systems by irrigating and by disinfection was unsuccessful. The possible protection of microorganisms by the demonstrated extracellular material against hydraulic forces and disinfection is discussed.

[Prophylaxis and treatment of respiratory tract infection in ventilated patients by endotracheal administration of aminoglycosides (author's transl)].

The effects of intratracheal administration of gentamicin on frequency of bacterial contamination of the trachea and respiratory tract infection were assessed in a controlled and prospective study in 20 patients on a respirator. Gentamicin was instilled undiluted 6-hourly in a dosage of 40 mg into the trachea. Compared with a control group (n = 20), patients with intratracheal gentamicin instillation showed earlier reduction of tracheal bacterial contamination and during further ventilation a diminished rate of colonisation of the trachea. There was no evidence of an increase of Candida albicans. Pneumonic infiltrations regressed earlier in patients with intratracheal gentamicin. Respiratory tract infections were less frequent during continued artificial ventilation than in the control group. In ventilated patients intratracheal aminoglycosides can thus be recommended for prophylaxis of respiratory tract infections.

[Microbiological studies of small hot-bath-pools and hot-whirl-pools (author's transl)].

Hot small bathing pools and hot whirl-pools have the following characteristics: small watervolume, thick squeeze of swimmers, high water temperature (37-40 degrees C) and small dimension of filters. By this, the quality of bathing-water is influenced detrimentally. To elaborate the hygienic problems, bathing-water samples were taken before, during and after the visiting-hours and were tested for facultative-pathogenic microorganisms. During this investigation E. coli was isolated in 25 degrees, Coliforms and Proteus species in 37.3%, P. aeruginosa in 36%, S. aureus in 26.3%, Enterococci in 42.3 %, Candida albicans in 3.6% and yeast totally in 8.3%.

[Identification of bacteria from use-surface disinfectant solutions and their sensibility against disinfectants (author's transl)].

The investigations are concerned with bacteria which had been isolated from use-surface disinfectant solutions originated from 19 disinfection dosage apparatus of different hospitals and from 8 taps of a centralised disinfection dosage apparatus in an other hospital. The greatest part of the isolated bacteria belonged to the genus Pseudomonas. Beyond it there were yellow-pigmented bacteria like Xanthomonas, Flavobacterium and Micrococcus. After subculture on solid peptone-containing media most of the isolated bacteria turned out to be sensitive to the disinfection solutions in a concentration range, that had been applied in the dosage apparatus. The examination of 6 disinfectants showed, that there were considerable differences in the efficiency to kill the bacteria. Even one preparation pointed out to be absolutely ineffective at applied concentrations.

[Antibacterial substances in tamponades after tympanoplasties - an "in-vitro"-study (author's transl)].

This "in-vitro"-study is concerned with the efficiency of antibacterial agents used in packings after tympanoplasties. For this purpose, gel-foam-material was impregnated either with rolitetracyclin (mostly used in German ENT-clinics), or comparatively with a combination of bacitracin and neomycin or with antiseptics like phenylmercuriborate and povidon-iodine. It is tested, how these impregnated "packings" behave in the presence of pathogenic germs (Staph. aureus, Ps. aeruginosa). Antibacterial effects and alterations of the material were judged. It is shown, that in some regards the rolitetracyclin - very successful in clinical use - is superior to the other substances tested.

[Contact allergy to a new preservative].

A 43 year old woman with no previous skin disease developed contact eczema on the face and neck after application of a lotion used for "smoothing wrinkels", available as a non-prescription item. Patch testing of the product and of one of its ingredients, Euxyl K 400, revealed strong positive reactions. Upon testing of the individual components of the preservative Euxyl K 400, the substance 1,2-dibromo-2,4-dicyanobutane showed a positive reaction even in the concentration of 0.004%. Due to the increasing use of Euxyl K 400 inclusion of the preservative in the patch test battery should be considered.
