Empty sella syndrome in childhood.

The empty sella syndrome is common in middle-aged women, usually presenting with headache, and only occasionally associated with endocrine or visual abnormalities. It is rare in childhood. Childhood cases tend to present either with endocrine disturbances, visual symptoms, or with craniofacial syndromes. We present three cases of complete empty sella with childhood onset, each discovered unexpectedly during evaluation of endocrine or visual dysfunction.

Oral immunisation with killed Haemophilus influenzae for protection against acute bronchitis in chronic obstructive lung disease.

Fifty patients with chronic obstructive lung disease were randomly allocated to three groups, to assess whether an oral vaccine containing non-typable Haemophilus influenzae protected against acute bronchitis. The double-blind prospective study over a three-month winter period included two placebo groups and one test group. Oral immunisation with H influenzae induced a tenfold reduction in the incidence of infection (p less than 0.001). During the subsequent winter, without further immunisation, protection by the vaccine was no longer statistically significant. There was no clear correlation between clinical protection and either carriage of H influenzae or the level of antibody to H influenzae antigen in saliva.

Multiresidue screening of pesticides in foods using retention time locking, GC-AED, database search, and GC/MS identification.

Fruit and vegetable extracts were screened for over 400 pesticides by gas chromatography with atomic emission detection (GC-AED) and an experimental database. A technique called retention time locking was used to match GC-AED and GC with mass spectrometry (MS) retention times to those of the database. Samples were analyzed for sulfur, nitrogen, phosphorus, and chlorine by GC-AED. Possible pesticides were suggested by database search and identified by GC/MS. Forty-four pesticide standards were analyzed to determine the precision of retention time matching and the accuracy of the database search. Analytical retention times matched database retention times within 0.32 min. Using elemental criteria, the database search identified the correct compound for 41 of 44 pesticide standards. For blind spikes of fruit and vegetable extracts, the database suggested 22 of 26 spiked pesticides as matches. Nineteen were identified by GC/MS. The combination of retention time locking, GC-AED, database search, and GC/MS can be a powerful tool for identifying pesticides in a complex matrix.

Multiresidue analysis of pesticides in fresh fruits and vegetables using procedures developed by the Florida Department of Agriculture and Consumer Services.

Improved quality and efficiency of pesticide residue analysis were achieved by examining all aspects of the laboratory process. In an effort to eliminate methylene chloride hazardous waste, an acetonitrile extraction method, originally developed by the California Department of Agriculture, was modified and adopted. Sample size and solvent consumption were reduced with the new method. Custom glassware racks and disposable supplies reduced overall analysis time. Gravity-fed, solid-phase extraction simplified sample preparation and provided cleaner extracts for gas chromatographic analyses. Modifications to the method were made to achieve the ruggedness needed to maintain quality objectives during routine analysis. Instrumental improvements, including new selective detectors, retention time locking, and mass spectrometry screening for all samples, provided the laboratory with efficient, reliable, and confirmed analytical results.

Bone condensing in the placement of endosteal palatal implants: a case report.

Bone condensing as a technique to prepare an implant site is demonstrated on a patient who was to undergo orthodontic treatment utilizing a palatal implant as an anchor. Based on the anatomic constraints of the palate and the desire to load the implant as axially as possible, adapted-instrumentation for bone condensing is presented. Bone condensing for preparation of the implant site in soft maxillary bone avoids the risk of heat generation, and palatal implants can be placed precisely with primary stability.

Overexpression of nm23-H4 RNA in colorectal and renal tumours.

A new member of the nm23 gene family, designated nm23-H4, was cloned recently. Nm23-H4 is a mitochondrial protein. To determine whether nm23-H4 could have a function in tumour progression like nm23-H1 or nm23-H2, we analysed nm23-H4 expression in 18 renal tumours and 42 colorectal carcinomas obtained from patients who underwent curative resection. As compared to the corresponding healthy tissue, 14 renal tumours and 41 colorectal carcinomas showed increased nm23-H4 mRNA expression. While the increase was only moderate in renal cell carcinoma, a strong overexpression of nm23-H4 was noted in most colorectal carcinomas, possibly indicating a role of this gene in tumour genesis. The nm23-H4 transcript levels did not correlate with tumour stage, grade of differentiation or lymph node involvement.

NM23-H1 and NM23-H2 gene expression in human renal tumors.

The two nm23 genes, nm23-H1 and nm23-H2, are implicated in the metastatic process and tumor progression in some human tumors. Until now no data exist about nm23 expression in the different types of human renal tumors. To investigate if the nm23 genes play a central role in the progression of renal tumors, we have examined nm23-H1 and nm23-H2 gene expression using Northern-blot analysis and immunohistochemistry. We analysed clear cell type RCC, chromophilic RCC, chromophobic RCC, collecting duct type RCC and renal oncocytomas. Our results indicate that the nm23 genes do not play a central role in the prognosis of renal cell carcinoma in the analysed tumors.
