Ostial stenosis of the left coronary artery as a sole clinical manifestation of Takayasu's arteritis: a possible cause of unexpected sudden death.

An 11-year-old girl experienced several syncopal attacks and her electrocardiogram showed ST depression with exercise. Ostial stenosis of the left coronary artery was disclosed by the coronary angiography. We suspected the coronary involvement was due to Takayasu's arteritis. On operation, aortitis in the ascending aorta involving ostium of the left coronary artery was observed. Therefore Takayasu's arteritis is considered to be one of the causes of sudden death.

Coronary artery bypass in a 15-year-old girl with pseudoxanthoma elasticum.

A 15-year-old girl with pseudoxanthoma elasticum and associated severe coronary artery disease underwent double coronary artery bypass using left internal mammary artery and saphenous vein. Preoperatively the patient had life-threatening syncope and positive treadmill test results. The postoperative course was uneventful, and the patient has been doing well for 3 years. Pseudoxanthoma elasticum is a rare disease that can cause premature coronary arterial obstructive disease in children and adolescents.

Renal tubular involvement mimicking Bartter syndrome in a patient with Kearns-Sayre syndrome.

A 10-year-old boy had short stature, external ophthalmoplegia, atypical retinal pigmentary degeneration, and sensorineural hearing loss (Kearns-Sayre syndrome). In addition to ragged-red fibers observed on modified Gomori trichrome staining, there were scattered fibers exhibiting no cytochrome c oxidase activity, indicating a focal deficiency. Cytochrome c oxidase and other respiratory chain enzyme activities were normal biochemically. The patient also had renal tubular dysfunction, including isosthenuria, decreased urine-concentrating ability, and excessive excretion of potassium and magnesium. In addition, he had hyperreninemia and hyperaldosteronism but no hypertension. The renal dysfunction was thought to have resulted from a primary defect in the thick ascending limb of the loop of Henle, mimicking Bartter syndrome. In contrast to previously described cases of cytochrome c oxidase deficiency with de Toni-Fanconi Debré syndrome, the patient had less intensive muscle abnormalities. A renal biopsy specimen showed ultrastructural changes in mitochondria that were similar to those seen in biopsy specimens of muscle. A large-scale deletion (8.8 kilobases) in mitochondrial DNA was found in biopsy specimens of muscle and kidney.

Platelet-derived growth factor stimulates formation of active p21ras.GTP complex in Swiss mouse 3T3 cells.

The ras gene product (p21) is a GTP-binding protein and is thought to play an important role in signal transduction of growth and differentiation in many types of mammalian cells. The p21.GTP complex is an active conformation, as described previously for polypeptide chain elongation factors (EF-Tu and EF-G) and heterotrimeric GTP-binding proteins (G proteins). In the study reported here, we measured the amounts of p21-bound guanine nucleotides under various conditions in the G54 cell line, a derivative of Swiss 3T3 cells that overexpresses normal c-Ha-ras. More p21.GTP complexes were present in growing cells than in quiescent cells. When quiescent cells were stimulated with fetal bovine serum to promote DNA synthesis, p21.GTP increased approximately 2-fold. Among a number of purified growth factors, platelet-derived growth factor enhanced the formation of p21.GTP, whereas the combination of bombesin and insulin, which also induces DNA synthesis, did not. These results strongly suggest that p21 is a transducer of the growth signal from the platelet-derived growth factor receptor in Swiss 3T3 cells and that the signal is transmitted through a p21.GTP complex.

Accumulation of p21ras.GTP in response to stimulation with epidermal growth factor and oncogene products with tyrosine kinase activity.

The ras gene product (p21) is a GTP-binding protein and has been thought to transduce signals regulating proliferation or differentiation of cells. Like other GTP-binding proteins, p21.GTP is an active conformation, which can transduce the signals downstream, whereas p21.GDP is an inactive one. Recently, we have shown that p21.GTP levels increased in cells treated with fetal bovine serum or platelet-derived growth factor to initiate DNA synthesis. In this paper, we report that epidermal growth factor can also increase the amounts of p21.GTP in the cells. Effects of epidermal growth factor and platelet-derived growth factor are not additive. In contrast, mutant [Val12]p21, which has transforming activity, responded neither to platelet-derived growth factor nor to epidermal growth factor. We also found that the ratio of p21.GTP to p21.GDP increased 3- to 4-fold in transformants carrying activated erbB-2/neu or v-src oncogenes. These results strongly suggest an important role of p21 in transduction of signals for both normal proliferation and malignant transformation through growth factor receptors with tyrosine kinase activity or related oncogene products.

Diurnal variation of cerebral blood flow in rat hippocampus.

We measured local cerebral blood flow over 24 hours in 10 unanesthetized, freely moving rats to determine whether blood flow in the hippocampus fluctuated as a function of time of day. We measured hydrogen clearance at 1-hour intervals using a polyurethane-coated platinum electrode with a 1-mm bare tip implanted in the dorsal hippocampus. Individual rats displayed a wide range of local cerebral blood flow values (from 30 to 100 ml/min/100 g tissue) in a day. In seven of the 10 rats, the overall mean hippocampal blood flow for the dark cycle (7 PM-5 AM) was significantly (p less than 0.001, 0.01, or 0.05) greater than that for the light cycle (6 AM-6 PM), showing an average increase of 20%. Further, the maximum mean hippocampal blood flow at 11 PM in all 10 rats was 42% greater than the minimum at noon. Our study demonstrates for the first time that local cerebral blood flow in the hippocampus shows diurnal variation.

Aortic regurgitation: ventricular response after aortic valve replacement.

This study was designed to evaluate the usefulness of the ratio of the preoperative regurgitant stroke volume to left ventricular end-diastolic volume (RSV/LVEDV) for assessing the left ventricular function preoperatively. In 26 patients with aortic regurgitation (AR), the percent decrease in LVEDV was compared with the preoperative RSV/LVEDV, ejection fraction (EF), LVEDV, left ventricular end-systolic volume (LVESV) or left ventricular end-diastolic pressure (LVEDP). There was a significant correlation between the percent decrease in LVEDV and RSV/LVEDV. Patients with RSV/LVEDV of more than 0.26 had a significantly smaller postoperative left ventricular end-diastolic volume index (LVEDVI) and left ventricular end-systolic volume index (LVESVI), and a greater postoperative EF than patients with smaller RSV/LVEDV. All but one patient with RSI/LVEDVI larger than 0.0016 LVEDVI had normal postoperative LVEDVI. Based on these findings, it is concluded that the RSV/LVEDV is an useful indicator for preoperative evaluation of left ventricular functions in patients with AR. Surgical intervention for patients with AR should be recommended before the RSI/LVEDVI drops to less than 0.0016 LVEDVI, to expect good postoperative ventricular responses.

Thiol depletion and chemosensitization on nimustine hydrochloride by methionine-depleting total parenteral nutrition.

As an adjunct to cancer chemotherapy, we had succeeded in creating the methionine depletion in vivo, using the technique of total parenteral nutrition (TPN) by infusing an amino acid solution devoid of methionine and cysteine, as sole nitrogen source (Met-deplet. TPN). In Experiment 1, we demonstrated that the marked depletion of thiol was induced both in the liver and tumor tissues by Met-deplet. TPN in Sato lung carcinoma (SLC)-bearing rats. Then in Experiment 2, we also confirmed the presence of the enhancing effect on nimustine hydrochloride (ACNU) in Met-deplet. TPN to SLC. The tumor proliferation was inhibited significantly by Met-deplet. TPN even in conjunction with a small dose of ACNU, which showed no anti-tumor effect on the rats treated with methionine-containing amino acid solution, without apparent increase of the side effects in comparison with those in the control groups. On the other hand, to determine the carcinostatic effect on tumor-bearing animals, not only the size of the tumor but also its components, especially the percentage of necrotic tumor tissue (necrotic index), were considered to be important.

Neurinoma of the oculomotor nerve--case report.

Only 10 cases of oculomotor nerve neurinoma have been reported previously. The authors report the rarity of a neurinoma originating from Schwann cells of the oculomotor nerve. The diagnosis was based on the initial sign of oculomotor nerve paresis without involvement of other cranial nerves, neuroradiological and surgical findings, and histological features of the specimen obtained at surgery.

[Solid scintillator "Ready Cap" for measurement with a liquid scintillation counter--spectra and counting efficiencies for 15 different radionuclides].

"Ready Cap", a small plastic container coated with solid scintillator has recently been introduced (Beckman Instruments, Inc.). Pulse height spectra and counting efficiencies obtained with a liquid scintillator and Ready Cap using a liquid scintillator counter were compared for 15 different radionuclides. For radionuclides emitting low-energy beta-rays or characteristic X-rays, the spectra for Ready Cap shifted toward the higher energy side compared with the spectra for the liquid scintillator. This tendency was reversed for the nuclides emitting higher-energy beta-radiations (36Cl and 32P). Generally, counting efficiencies both in Ready Cap and in liquid scintillator increased with increase in the energy of beta- or X-rays. For some nuclides, Ready Cap gave higher counting efficiencies and for others it gave lower values than in the liquid scintillator. However, the differences were not large within each nuclide. The use of Ready Cap is recommended for measurements of radionuclides when liquid scintillation cocktails have no means of waste disposal under the present Japanese radioisotope regulation.

The influence of the supernatant obtained from a culture of IL-2-activated lymphocytes on human hematopoietic progenitors.

A supernatant obtained from the culture of recombinant human interleukin 2-activated lymphocytes was added to granulocyte (G)-, macrophage (M)-, and granulocyte-macrophage (GM)-progenitor assays using a plasma clot culture method. The supernatant was found to exert a suppressive influence on the colony and cluster formations of the G progenitors. This suppressive influence of the supernatant was almost completely neutralized by the addition of a combination of anti-human tumor necrosis factor alpha (HuTNF-alpha) and anti-human interferon gamma (HuIFN-gamma) antibodies, and it has been speculated that this suppressive influence of the supernatant is probably due to TNF-alpha and IFN-gamma. The numbers of M and GM progenitors increased remarkably in the assays in which the supernatant was added when compared to assays without the supernatant; this suggests that these progenitors were resistant to TNF-alpha and IFN-gamma. Among the bone marrow mononuclear cells (BMNC) excluding erythroid-rosette-forming (ERFC) or adherent cells, the influence of the supernatant on these progenitors was not significantly different. The results of our study have shown that ERFC and the adherent cells in BMNC hardly affected the appearance of the suppressive influence.

Results of surgery in patients with gastric cancer extending to the adjacent organs.

Of 1,211 gastric cancer patients who underwent operations, 196 (16%) had cancers extending to the adjacent organs. Palliative procedures were performed in 66 (34%), and gastrectomies in 130 (66%). Operative mortality for the former was 6% (4/66), but was 0% for the latter. Among 130 gastrectomized patients 91 (70%) underwent combined resections of the invaded organs. With respect to the mean survival, significant differences were noted between gastrectomized (18 months) and non-gastrectomized patients (4 months), between the patients with (20 months) and without (12 months) combined resections, and between curative (27 months) and non-curative resections (13 months). Presence or absence, and degree of peritoneal, liver and lymph node metastases were also prognostic among gastrectomized patients. Combined resections of the involved organs showed slight, but positive, effects on survival of patients with liver metastases, but they seemed to be ineffective in the presence of peritoneal metastases. In the case of patients with apparently incurable factors such as P2-3, H2-3 or N4, gastrectomy was prognostically ineffective.

Malignant mixed müllerian tumor of the ovary. Report of a case studied by immunohistochemistry.

A case of malignant mixed müllerian tumor of the ovary in a 57-year-old woman is reported along with the results of an immunohistochemical study. The tumor, measuring 16 x 10 x 9 cm, was composed predominantly of adenocarcinoma with a smaller amount of anaplastic carcinoma as an epithelial component and chondrosarcoma, liposarcoma, fibrosarcoma and rhabdomyoblasts as mesenchymal elements. Immunohistochemistry using paraffin sections demonstrated cytokeratin (CK) and epithelial membrane antigen (EMA), generally regarded as epithelial markers, not only in the epithelial component but also in chondrosarcoma cells. Vimentin and desmin, generally regarded as mesenchymal markers, were exhibited partly in carcinoma cells as well as in mesenchymal elements. Positive staining for S-100 protein was obtained not only in chondrosarcoma and liposarcoma cells, but also partly in adenocarcinoma cells. This intricate immunohistochemical picture reflected the histologic findings. It is noteworthy that both carcinoma cells and chondrosarcoma cells demonstrated simultaneous expression of CK, EMA, vimentin, desmin and S-100 protein. This somewhat unusual antigen expression by tumor cells may indicate a change in the nature of tumor cells due to microenvironmental factors.

[The mean of Downs and northwestern analysis in adults].

An angle analysis and linear analysis are popularly used in clinical orthodontics. Downs analysis and Northwestern analysis are two methods of angle analysis, and the mean values reported by Iizuka in 1963 are still used. However in Japan each generation's constitution (height, weight) tends to increase year by year. For example, the mean height and weight of adult males (23 years old) have increased by 5.0cm and 7.6kg as compared with nearly 20 years ago. Therefore the mean of 20 years ago may not be appropriate. Analytical values at normal occlusion Japanese adults by Kayukawa, Iizuka, Ishikawa, Yamauchi, Miura, Iwazawa and, most recently by Asai in 1973. We obtained these values and determined the validity of currently used values.

A case of Class II malocclusion associated with a deeply impacted maxillary central incisor.

The patient was a 9-year-old girl with a skeletal Class II malocclusion characterized by maxillary protrusion, excessive overjet and deep overbite. The patient's maxillary left central incisor was deeply impacted. During the first stage, a lingual arch was employed after surgical exposure. During the secondary stage, all four first premolar teeth were extracted and then edgewise mechanotherapy was performed. The total treatment time was 6 years. The malocclusion was treated satisfactorily and resulted in correction of the esthetic and functional flaws after 1 year out of retention.
