[A report of two cases of coronary bypass surgery after renal transplantation].

Coronary artery bypass surgery was performed in two patients who had had renal transplantation. They had been taking immunosuppressive therapy. There were some problems in the management of these patients including prevention of postoperative infection, renal damage due to cyclosporin, and rejection of the transplanted kidney. Immunosuppressive therapy was continued under the meticulous monitoring of blood cyclosporin levels, blood T-cell subsets, markers of infection such as antiviral titers. The anticipated problems were not encountered in both cases. This is the first report in Japan which describes successful coronary artery bypass surgery for kidney transplant recipients.

Endoscopic and clinicopathological features of primary gastric lymphoma.

Thirty-three patients with primary gastric lymphoma over a period of 32 years were studied. Endoscopic findings indicated that there were many cases of multiple lymphoma in the stomach, or with extensive tumor-infiltration, and that it was necessary to carefully inspect the entire stomach to determine the scope of resection and improve diagnostic accuracy. It is also particularly important not to damage the tissue during biopsy and to take deep large specimens from the ulcer margin, surrounding wall, and area of erosion. As compared clinicopathologically with gastric carcinoma, primary gastric lymphoma is found more frequently in younger females, with invasion of the entire stomach and extensive lymph node infiltration. By performing adequate resection of lymph nodes, as is done for gastric carcinoma, and administering appropriate combination chemotherapy, postoperative longterm results were similar to, or better than, those obtained with gastric carcinoma.

[Age estimation from tooth attritions of lower incisors--discussion on the "Amano's method"].

As a method of age estimation from teeth we focused our attention on attritions of lower incisors. Using the Amano's method, we took records of tooth attritions in the planted state of 83 subjects whose ages ranged from 14 to 76 years. A positive correlationship was found to exist between the tooth attrition index and the actual age, allowing to establish the following formula which graphically shows a straight line: Y = 8.505X + 26.073 (r = 0.607), where X is the mean of attrition indexes of lower incisors and Y, the estimated age. In our method of age estimation through attrition indexes, we took advantage of probability and confidence interval. In addition, our method was contrasted with Amano's method, a widely used one in Japan.

[Effects of vanadate on glucose metabolism of the streptozotocin-diabetic rat lens].

The effects of oral administration of vanadate, an oxidized form of vanadium, on glucose metabolism of the lens were studied in 5-week-old male Sprague-Dawley rats rendered diabetic with streptozotocin (50 mg/kg, intraperitoneal injection). The rats were given 0.2g/l NaVO3-5 g/l NaCl solution in drinking water ad libitum for 2 weeks and biochemical parameters in their lenses were determined. Blood glucose levels significantly decreased in the vanadate-administered diabetic rats (DM-V group), compared with the diabetic rats given no vanadate (DM group). In the DM-V group, significant decreases were observed in lens contents of glucose and sorbitol without significant decrease in fructose content, compared with the DM group. In the former group, aldose reductase activity was not decreased, but sorbitol dehydrogenase (SDH) activity was significantly increased, compared with the latter group. These results suggest that oral administration of vanadate may decrease sorbitol content in the diabetic rat lens by an acceleration of the sorbitol pathway to fructose accompanied by increase in SDH activity.

[A case of anterior mediastinal mature teratoma containing air because of penetration to the lung].

A case of anterior mediastinal mature teratoma containing air because of penetrating to the lung was reported. The patient was a 28-year-old woman. The chest X-ray film on admission showed a homogeneous mass in the anterior mediastinum. This shadow changed to cystic one after ten days later. The tumor was removed together with adhering part of the left lung and pericardium through median sternotomy. Histologically, the wall of cysts contained sebaseous glands, bronchial glands and pancreatic tissue containing both exocrine and endocrine elements.

[Immunological effect of recombinant interferon-gamma in renal cell carcinoma].

The immunological effect of interferon-gamma (IFN-gamma) was investigated in 18 cases of renal cell carcinoma before and after the operation. In 6 patients, IFN-gamma was administered preoperatively for 21 days (administration group), while 12 patients underwent nephrectomy alone without preoperative treatment (control group). The peripheral immunological effects were measured at before and 12 days after the operation in the administration group and at 11 days in the control group. In the administration group, a marked increase was noted in the test of antibody dependent cell-mediated cytotoxicity (ADCC) activity and natural killer (NK) activity, and slight increase in CD4/8 and CD11b. In low stage cases, no appreciable effect was obtained by the administration of IFN-gamma. However, in high stage cases, IFN-gamma tended to increase the value of ADCC activity and NK activity. Immunohistochemical studies of tumor infiltrating lymphocytes in renal cell carcinoma showed a high incidence of CD8 and CD11b in the administration group. Moreover, the presence of CD8 was higher than that of CD4 in the administration group in contrast to the results of the peripheral blood analysis.

[Pathological assessments of allografts after heart transplantation using a dog model--allograft vasculopathy].

The pathological findings of allografts after heart transplantation were assessed in long-term period using a dog model. Eight orthotopic heart transplantations were performed with cross-circulation method using a dog. Two dogs survived over 1 year after heart transplantation. One dog died due to cardiac rejection, and the another dog died due to infection at 14 months and 16 months after the transplantation, respectively. The coronary artery morphologic features were assessed in the allografts. The pathological findings showed fibrous intimal thickening with infiltration of lymphocytes and macrophages. And it was diffusely distributed to each area of transplanted heart. The stenosis of coronary lumen was significantly seen in middle size of coronary arteries which was from 100 microns to 50 microns. The characteristics of these findings in comparison with arteriosclerosis change were 1) rapid progression, 2) diffuse fibrous intimal thickening, 3) preserved internal elastic lamia. These findings might be important for treating this complication.

[Efficacy of myocardial preservation using GIK solution for prolonged ischemic heart].

The efficacy of myocardial preservation using GIK solution combined with continuous cold blood perfusion was assessed in patients with prolonged ischemia. Fifteen patients receiving more than 4 hours' ischemia were divided into two groups. In Group A, seven patients were associated with severe aortic regurgitation and showed increased left ventricular volume of more than 60 mm in LVDd and 45 mm in LVDs. In Group B, eight patients had a normal left ventricular volume of less than 60 mm in LVDd and 45 mm in LVDs echocardiographically. There was no significant difference in ischemic time, or preoperative hemodynamic parameters between the two groups. The postoperative cardiac index was 2.93 +/- 0.87 in Group A and 4.3 +/- 0.8 in Group B (p < 0.01). The postoperative shortening fraction was 0.17 +/- 0.01 in Group A, 0.43 +/- 0.12 in Group B (p < 0.01). These parameters significantly deteriorated in Group A compared to Group B. Released enzymes (GOT, CK-MB) were significantly increased in Group A with prolonged ischemic time. In conclusion, postoperative cardiac function after 4 hours' ischemia using GIK solution deteriorated in the patients with a dilated heart due to aortic regurgitation.

[Usefulness of subtotal colectomy with colorectal anastomosis for chronic ulcerative colitis].

Recently in Japan, most of surgeons have been performing restorative proctocolectomy with ileoanal anastomosis for ulcerative colitis, leaving unsolved problems with regard to postoperative complications. Analysis was made on subtotal colectomy with colorectal anastomosis with the following policies. Firstly, the inflammation of the remnant intestine could be easily and correctly controlled by medication in outpatient. Secondly, carcinomas associated with ulcerative colitis can be discovered by postoperative colonoscopic examinations. Operations were carried out to 26 cases of ulcerative colitis for the past 13 years. Subtotal colectomy with colorectal anastomosis were performed to 11 cases. Three cases of colorectal carcinomas associated with ulcerative colitis were experienced. One of the resected specimen was investigated using stereomicroscopy after alcian blue staining to formalin fixed one. As a result of this research, we have convinced that colorectal anastomosis should be favorable surgical potion to the cases of chronic ulcerative colitis because postoperative anal function and quality of life are excellent in our series, in addition, carcinomas in the remnant intestine can be probably easily found out at the early stage by annual colonoscopic examinations with dye staining.

Disappearance of Ph1 chromosome with intensive chemotherapy and detection of minimal residual disease by polymerase chain reaction in a patient with blast crisis of chronic myelogenous leukemia.

We diagnosed a patient with chronic myelogenous leukemia (CML) in chronic phase (CP) on the basis of clinical findings, Ph1 chromosome detected by cytogenetic analysis, and bcr-abl fusion mRNA detected by reverse transcriptase-dependent polymerase chain reaction (RT-PCR). One month after diagnosis, the patient developed extramedullary blast crisis in the lymph nodes, and then medullary blast crisis in the bone marrow, in which different surface markers were shown. Combination chemotherapy with BH-AC, VP16, and mitoxantrone was administered; this resulted in rapid disappearance of the lymphadenopathy, restoration of normal hematopoiesis, and no Ph1 chromosome being detected by cytogenetic analysis. RT-PCR performed to detect the residual Ph1 clone revealed that although the Ph1 clone was preferentially suppressed, it was still residual. The intensive chemotherapy regimen preferentially suppressed the Ph1-positive clone and led to both clinical and cytogenetic remission in this patient with BC of CML; we suggest that RT-PCR is a sensitive and useful method for detecting minimal residual disease during the clinical course of this disease.

[Sutureless aortic valve replacement for peri-annular pseudoaneurysm and perivalvular leakage due to prosthetic valve endocarditis].

A 49-year-old man who had undergone AVR entered our hospital with congestive heart failure and continuing slight fever. By cardiac imaging, he was diagnosed prosthetic valve endocarditis with periannular pseudoaneurysm and perivalvular leakage. He underwent a new translocation method using a composite valve (a prosthetic valve directly sutured a ring prosthesis, which was separated from an intraluminal ringed graft). Post operative course was uneventful. He recovered well after the operation.

[Functional evaluation of skeletal muscle ventricles for circulatory assist].

Skeletal muscle has a potential power for cardiac assist. Two approaches have been used to harvest this power: dynamic cardiomyoplasty, which involves the application of muscle directly to the heart to support cardiac contractile function; and the construction of skeletal muscle pouches or ventricles, which are used as separate pumps working either in parallel or in series with the heart. In this study, we evaluated the function of the skeletal muscle ventricles (SMVs) for circulatory assist. In six dogs (18.5 +/- 2.3 kg, mean +/- S.E.M.), SMVs were constructed from the right latissimus dorsi muscle, and placed in the right hemithorax. After a 3-week vascular delay period, the SMVs were preconditioned electrically by 2 Hz continuous stimulation for six weeks. Nine weeks later, SMVs were connected to a mock circulation device for functional evaluation. As right-sided pumps, at a preload of 10 mmHg, SMVs generated stroke work of 0.63 +/- 0.04 x 10(6) ergs with 25 Hz and 0.80 +/- 0.06 x 10(6) ergs with 85 Hz, which exceeded that of the native right ventricle. As left sided pumps, also at a preload of 10 mmHg, SMV stroke work was 0.49 +/- 0.13 x 10(6) with 25 Hz 0.90 +/- 0.09 x 10(6) ergs, which was roughly half that of the left ventricle. These results demonstrate that SMVs have the potential to function as left or right heart assist devices.

Partial characterization of a rabbit liver Ca(2+)-calmodulin-dependent kinase with myosin light chain phosphorylating activity.

A Ca(2+)-calmodulin-dependent protein kinase that phosphorylates the regulatory light chain of hepatocyte myosin was purified from rabbit liver. The kinase catalyzed the incorporation of phosphate into the 22-k light chains of hepatocyte myosin, resulting in a 7-fold activation of the Mg(2+)-ATPase activity by F-actin. The kinase did not show any glycogen synthase kinase activity which has previously been shown to phosphorylate isolated chicken gizzard myosin light chain. ML-7, an inhibitor specific for smooth muscle myosin light chain kinase, inhibited the liver kinase with a Ki value of 13.2 microM.

[Coronary artery bypass grafting with all arterial grafts using the internal thoracic, the gastroepiploic and the inferior epigastric arteries].

CABG with all arterial grafts using both the internal thoracic artery (RITA, LITA), the right gastroepiploic artery (GEA) and the inferior epigastric artery (IEA) was performed in 26 patients from July 1989 to August 1991. There were no early and late deaths. Early postoperative coronary angiography in all patients revealed that the best choice of anastomosis was RITA to LAD, LITA to LCX or DIA, and GEA to RCA (type A). All arterial grafts CABG is safe and feasible, but the saphenus vein graft must be used to avoid the anastomosis from small GEA to small LCX.

[Quantitation of regional myocardial blood flow using continuous inhalation of C15O2 and positron emission tomography].

A C15O2 continuous inhalation and a steady-state theory using positron emission tomography (PET) have been used to quantitate regional cerebral blood flow. We extended this method for quantitative regional myocardial blood flow (RMBF) measurement. Forty-three patients, involving 4 normal volunteers, 16 patients with coronary heart disease (CHD), 20 patients with hypertrophic cardiomyopathy (HCM), and 3 patients with dilated cardiomyopathy (DCM), were studied. 6 min-scan was recorded in the steady-state condition during C15O2 continuous inhalation. A C15O inhalation scan was required to obtain the blood pool image. The subtracted image showed myocardial perfusion image. These images demonstrated homogeneous accumulation in normal volunteers. Reduced accumulation were detected in patients with myocardial infarction. Hypertrophied myocardium was demonstrated in patients with HCM. Diffuse heterogeneous accumulation was demonstrated in patients with DCM. Fourteen patients with HCM, whose left ventricular myocardial thicknesses were more than 20 mm, were selected for the RMBF measurement to minimize errors due to the partial volume effect. The arterial blood activity was measured by assigning a region of interest to the left atrial cavity on PET images. RMBF was calculated using the steady-state theory. Calculated flow values ranged from 21.2 to 152.6 ml/min/100 g. The good correlation was obtained between RMBF using 13NH3 first pass method and RMBF using gas inhalation method in six patients. These results indicate that C15O2 PET has the potential capability for the noninvasive quantitation of RMBF.

[The study of neuropsychological alterations following coronary artery bypass operation as predicted by computed tomography scan of the brain].

The objective of this clinical study is to provide information regarding the association between coexistent cerebrovascular disease and neuropsychological abnormalities after coronary artery bypass operations. Computed tomography scan of the brain was performed in 104 patients pre-operatively, and their post-operative neuropsychological functions were evaluated. The patients were categorized as follows according to the CT findings: Seventy-three patients showed normal or slight cerebral cortical atrophy which usually seen in patients over fifty of age (group A). Sixteen showed moderate or severe cortical atrophy (group B). Fifteen patients demonstrated the characteristic findings of Binswanger type; severe white matter hypodensity especially in frontal horns and dilated ventricles (group C). Overt neuropsychological dysfunction was not observed in patient in groups A and B. Six patients in group C showed a combination of dementia, bizarre behavior, disorientation and gait dyspraxia following bypass operations. The pseudobulbar signs were also found in 3 patients. These clinical abnormalities persisted for six days to three weeks, and were most often reversible. Although the underlying mechanism of these deleterious alterations is not elucidated, the ischemic nature of the characteristic white matter lesions was highly suspected. The arteriosclerotic changes of the arteriole of the cerebral cortex and hypoperfusion during cardio-pulmonary bypass were supposed to be responsible. Therefore it was concluded that special attention should be focused on neurological evaluation for bypass surgery in group C patients.

[A right internal thoracic artery graft to the left anterior descending artery].

From 1987 to 1991, 48 patients received a right internal thoracic artery (RITA) to the left anterior descending artery (LAD). No operative death occurred, but two patients (4.2%) died during hospitalization due to graft versus host disease, and congestive heart failure. Graft patency of the RITA was 100%. After a 17.2 month mean follow-up, the 4 year actuarial survival rate (including all causes of death) was 85.3%, and the 4 year cumulative cardiac event free rate (including all cardiac deaths, myocardial infarction, reoperation, and PTCA) was 87.6%. The length of the RITA graft to the LAD, 15.6 +/- 1.1 cm, was significantly longer than that of the left internal thoracic artery (LITA) to the LAD, 12.9 +/- 1.1 cm. However, the flow of the RITA-LAD graft, 42.3 +/- 21.1 ml/min, was same as the flow of the LITA to the LAD, 42.1 +/- 19.6 ml/min. These findings justify wider use of the RITA to the LAD as a second arterial graft.

[The effect of LDL-apheresis on the long-term prognosis of hypercholesterolemic patients with coronary artery bypass grafts: a multicenter study].

Hypercholesterolemia is well known as a risk factor which had been shown to affect the long-term prognosis after coronary artery bypass grafting (CABG). The data were collected from multi-institutions documenting the long-term results of CABG in 61 hypercholesterolemic patients who received LDL-apheresis. Mean post-CABG period was 50 +/- 34 (+/- SD) months. Mean period of therapy by LDL-apheresis using Liposorber system after CABG was 25 +/- 17 months with a frequency of once per 2.6 +/- 1.3 weeks. Before initiating LDL-apheresis mean serum cholesterol (TC) and LDL-cholesterol (LDL-C) were 327 mg/dl and 261 mg/dl, respectively. With combined LDL-apheresis and lipid-lowering drug therapy mean TC and LDL-C were reduced to 247 mg/dl and 177 mg/dl immediately pre-apheresis and 106 mg/dl and 60 mg/dl immediately post-apheresis, respectively. Two of 61 patients had PTCA post CABG. Percent free from cardiac event during the long-term period post CABG as calculated by the life-table method was 97% at 3 years and 94% at 4 years and thereafter. While number of the patients is still small, the aggressive cholesterol-lowering therapy by LDL-apheresis is suggested to improve the prognosis of hypercholesterolemic patients with bypass grafts.
