Advances in the management of testicular cancer.

Dramatic advances have been made over the last decade in the management of testicular cancer. There are three identifiable reasons for this. The first is the widespread availability of tumour markers to assess the state of disease and its response to therapy. The second is CT scanning, which has revolutionized our ability to locate with fine anatomical detail the location of metastases. The third is the development of effective, intensive chemotherapy together with the understanding of its side-effects and optimum usage. Problems still remain in patients who fail to respond completely to existing therapy. In addition, the profound side-effects of chemotherapy mean that much effort is being invested in the development of less toxic but equally effective forms of treatment.

Leiomyomatosis peritonealis disseminata. A case report and literature review.

Leiomyomatosis peritonealis disseminata is a rare disorder characterized by the presence of multiple small nodules, consisting of benign smooth muscle, scattered over the abdominopelvic viscera and peritoneum. The case described below is the 21st reported in the medical literature to our knowledge. The condition generally occurs in menstruating, parous women in the third decade of life, and the symptoms are similar to those of uterine leiomyomata. There is a very high association with excess exogenous and endogenous female gonadal steroids, specifically estrogen and progesterone. Leiomyomatosis peritonealis disseminata is a benign condition for which conservative management is indicated, particularly if fertility is desired.

Do vitamin D supplements improve the physical capabilities of elderly hospital patients?

A randomized double-blind controlled trial of the effect of vitamin D supplementation on the abilities of elderly hospital patients to carry out basic activities of daily life is described. Those patients included in the trial had plasma 25-hydroxyvitamin D concentrations which were low or low normal as judged by the normal range in young adults. After 2 to 9 months on the trial there was no significant difference in the performance of the control and treatment groups.

Breast lumps and lymphoma.

We report two cases of young women in whom a breast lump was the first symptom of disseminated lymphoma. They were managed by wide excision of the affected segments of breast. Histological diagnosis was aided by immunoperoxidase stains for immunoglobulin chains. Further investigations demonstrated the wide spread nature of the disease, with involvement of bone marrow, bone cortex, lymph nodes and lung. Both patients received systemic chemotherapy, which achieved complete remission in one.

Advancement of the tibial tuberosity. A biomechanical study.

The effects of advancement of the tibial tuberosity by inserting bony wedges was studied on cadaveric specimens of the knee. The geometry, the contact areas and the forces acting on the patellofemoral joint were investigated, and the forces acting on the tibiofemoral compartment were calculated. A 1 cm advancement was found to be optimal in reducing the high patellofemoral joint forces occurring at 90 degrees and 110 degrees of flexion, whilst causing least reduction of the contact areas; the stresses on the joint were reduced significantly. Advancement by 2 cm and by 3 cm drastically reduced the congruity and the size of the contact area. It was also shown that 2 cm and 3 cm advancement caused an increase in forces at the patellofemoral joint and also in the tibiofemoral joint in a direction tangential to the articular surfaces.

Studies on lens vimentin.

Antibody prepared against chick lens vimentin cross-reacts with chick fibroblast vimentin and with vimentin of mammalian, reptilian, amphibian and fish lenses. This protein is localized in the epithelial and cortical fiber cells and is progressively lost from the deeper cortical cells. It is absent from the nuclear cells. Lens vimentin is readily oxidized to form high molecular components.

Cytoskeletal proteins of the aging human lens.

The cytoskeletal proteins of the human lens were studied by SDS-PAGE, 2-D electrophoresis and immunologically. Spectrin, vimentin and actin were identified in the superficial fiber cells of human lenses even to age 87 years. These proteins are lost from the deeper cortical and nuclear fiber cells, where a broad zone of acidic protein (36-42 Kd) emerges even in the transparent normal lens. Age-related changes in the water-soluble fraction include the increased prominence of proteins of 56 Kd and 36 Kd in the cortex, and of 38 Kd in the nucleus.

Splinting the rheumatoid hand.

Splinting of the rheumatoid hand is a complex topic. Understanding is assisted by the use of precise terminology and so is accurate prescription. Dynamic and static orthoses can be prescribed for any part of the upper limb to assist function or provide resistance exercise. Orthoses can be valuable in planning surgery and assisting return of function postoperatively. The importance of training in prescription, manufacture, fitting and wearing of orthoses is stressed and careful discussion with the patient is vital. Thermoplastic materials have provided a new spectrum of designs. Spring wire attachments developed at Chessington (Wynn Parry et al, 1970), Odstock (Glanville, 1962) and The London Hospital (Ellis, 1981) have introduced further possibilities. Designs should be functional and must be acceptable to the patient.

Prenatal tests for Sanfilippo disease type B in four pregnancies.

We report the prenatal diagnosis of two fetuses with Sanfilippo disease type B. In both pregnancies there were excessive amounts of heparan sulphate in amniotic fluid and the activity of N-acetyl-alpha-D-glucosaminidase was undetectable in cultured amniotic fluid cells. The predictions were confirmed by enzyme assay of cultured skin fibroblasts from the aborted fetus or the affected infant. The disorder was excluded for two other pregnancies at risk and the predictions are considered to be correct because of the normal progress of the healthy children.
