Phagocyte functions in familial Mediterranean fever.

Monocytes derived from peripheral blood of patients with familial Mediterranean fever (F.M.F.) demonstrated lower phagocytic capacity for Shigella flexneri and depressed bactericidal activity against S. albus when compared to monocytes from healthy individuals. Treatment of patients with colchicine did not alter these functions. On the other hand, chemokinesis of PMN of F.M.F. patients was enhanced especially during attacks. Colchicine treatment decreased significantly the PMN chemotactic migration.

Thrombocytosis in rheumatoid arthritis. Recurrent arterial thromboembolism and death.

A patient with rheumatoid arthritis complicated by excessive thrombocytosis and recurrent thromboembolic events is presented. The platelet count correlated well with disease activity and thrombosis occurred when thrombocytosis was marked. The patient died from massive thrombosis of the aorta despite treatment with anticoagulants, corticosteroids, and azathioprine.

Post partum renal failure due to progressive systemic sclerosis treated with chronic hemodialysis.

An unusual case of progressive systemic sclerosis with rapidly progressing renal failure after a successful delivery is described. The presenting syndrome was malignant hypertension. The blood pressure was refractory to the various therapeutic measures which were instituted, including hemodialysis. The patient, therefore, underwent bilateral nephrectomy, following which the blood pressure returned to normal, and she survived on hemodialysis for 17 months. To the best of our knowledge, the patient presented herein is the only reported case of this kind of fetal and maternal salvage.

The dual association between lymphoma and autoimmunity.

Autoimmune rheumatic diseases and lymphocytic malignancies are related and this association is bidirectional. Lymphomas occur more frequently in the course of autoimmune disease and autoimmune rheumatic manifestations occur in the course of lymphocytic malignancies. An increased incidence of malignant lymphocytic diseases is present in patients with rheumatoid arthritis, systemic lupus erythematosus, Sjögren's syndrome, and autoimmune thyroid disease. Descriptions of lymphocytic malignancies among other autoimmune rheumatic disease have been published. In some patients, the malignant disease is diagnosed months or years before the appearance of the rheumatic disease.

Follow-up of Hadassah Nursing School's first seven graduate classes.

This article presents a study that evaluated the work performance of the first seven graduate classes of Hadassah Nursing School, The Hebrew University of Jerusalem, Israel. Most of the graduates were located and their fields of employment and future professional plans determined. Strengths and weaknesses in the graduates' practical work are discussed. While interviewing the head nurses at the graduates' workplaces, the absorption strategies of accepting the graduates were questioned as were the ways in which the graduates were evaluated.

Nursing and home care in Europe.

The demographic trend of a rapidly ageing society becomes increasingly persuasive. More chronically ill patients will be discharged to home care. More people with psychotic problems and debilitating diseases will be living in communities. All require nursing care. This situation is recognized everywhere and no boundaries can change this future.

Leukocyte chemotaxis in recurrent polyserositis (familial Mediterranean fever).

Polymorphonuclear leukocyte chemotaxis was investigated in 35 patients with recurrent polyserositis during attacks and during spontaneous or colchicine-induced remissions. Chemotaxis was found to be unchanged in the attack-free period in untreated patients, increased by about 50% during attacks, and decreased by about 50% during colchicine treatment.

The effects of long-term colchicine therapy on male fertility in patients with familial Mediterranean fever.

Four out of 19 male patients suffering from familial Mediterranean fever (FMF) had fertility problems while on colchicine therapy (0.5-2.0 mg daily for as long as 11 years). Three of the patients had had children while off therapy but their wives could not conceive while they were on therapy. In one patient primary sterility remained one year after cessation of colchicine. In this and two other patients the spermiogram was normal but the sperm penetration test was pathological. The fourth patient had azoospermia. Patients should be informed about this possible risk of colchicine therapy. The need for continued follow-up and the value of the sperm penetration test in the detection of fertility problems in male patients on long-term colchicine therapy are stressed. It is concluded however, that overall the benefits outweigh the danger of long-term colchicine treatment in male patients with FMF.

The therapeutic use of dolls.

TOPIC: Using dolls as therapy in a psychogeriatric unit. PURPOSE: To acquaint nurses with this therapeutic modality. SOURCE: The Dolls Project. CONCLUSION: Dolls can be used therapeutically to awaken pleasurable affective responses in the older psychiatric client.

Comparison of some behavioral and physiological feeding parameters of Triatoma infestans Klug, 1834 and Mepraia spinolai Porter, 1934, vectors of Chagas disease in Chile.

There are two vectors of Chagas disease in Chile: Triatoma infestans and Mepraia spinolai. We studied the feeding behavior of these species, looking for differences which could possibly explain the low impact of the latter species on Chagas disease. Both species used thermal cues to locate their feeding source and consumed a similar volume of blood which was inversely related to the body weight before the meal and directly related to the time between meals. The average time between bites were 6.24 and 10.74 days. The average bite of M. spinolai lasted 9.68 min, significantly shorter than the 19.46 min for T. infestans. Furthermore, while T. infestans always defecated on the host, this behavior was observed in M. spinolai in only one case of 27 (3.7%). The delay between the bites and defecation was very long in M. spinolai and short in T. infestans. These differences may affect the reduced efficiency of transmission of Chagas infection by M. spinolai.

Small groups in Israeli education programmes.

Small groups have been used extensively in a broad range of circumstances with various populations and vastly differing goals. This Israeli study explored the contribution made through the participation of public health nurses in a small-group supervisory experience, which was an adjunct to a theoretical and clinical learning programme aimed at preparing the nurses for intervention in families with a chronically ill child. The impact of participation in supervised small groups, as perceived by the nurses, is assessed and evaluated.

Identifying the coping behaviours used by nurses in intensive care.

This study examines the use and perceived effectiveness among nurses of five coping styles: problem-focused coping, positive appraisal, help seeking, avoidance and aggression. Subjects were asked how they would cope with a conflict between themselves and a physician in the ward. The relationship between the coping behaviour and subjects' professional characteristics was explored. The subjects were 128 intensive cardiac care unit (ICCU) nurses. The results indicate that more experienced nurses use coping strategies less and may experience more burnout. The study also shows that nurses who participate in professional extra-curricular activities tend to choose more effective coping strategies.

Academia--a stimulus for change.

When the Department of Nursing at Tel Aviv University celebrated its 20th anniversary in 1988, interest was generated on this programme's impact on its graduates and on their contribution to nursing in Israel. Based on a previous study, a questionnaire was sent to 709 graduates, of which 46.8% responded, and a telephone survey was made of 20% of nonrespondents. Below, the study's revelations.
