Vascular factor in erectile failure among diabetics.

Clinical, sexual, and laboratory evaluations were made on 47 impotent diabetics. All patients were given a Doppler PAPS examination and a neurologic assessment of the penis (BCRL and/or CMG). Nerve conductivity tests of the legs were also administered to most patients. The groups were classified as to whether they were insulin or noninsulin dependent. Abnormalities of penile arterial flow were detected in 95.7 per cent of all the patients. Penile arterial occlusions without associated penile neuropathies occurred in 61.7 per cent. Penile neuropathies were found in 34 per cent, and all were associated with varying degrees of penile arterial obstruction. Diabetics who were dependent on insulin showed a high frequency of vascular pathologies and a 12 to 15 per cent higher rate of penile neuropathies than the group not dependent on insulin. Our study suggests that obstruction of penile arteries is the primary factor in erectile dysfunction of diabetics. Those impotent diabetics without any overt penile nerve damage might be good candidates for revascularization procedures.

The use of the red cell surface antigen to predict the malignant potential of transitional cell carcinoma of the ureter and renal pelvis.

This is the first report on the use of the red cell surface antigen test in predicting the malignant potential of transitional cell carcinoma of the ureter and renal pelvis. In 29 cases evaluated there was an 80 per cent correlation. The red cell surface antigen determination was shown to have practical value in the management of these tumors.

Pyelocaliceal diverticulum presenting as a disappearing renal mass.

We report a rare case of a disappearing cystic mass of the kidney. Pyelocaliceal diverticulum has to be considered in the differential diagnosis of patients with pyelonephritis and a space-occupying lesion of the kidney. Initial conservative therapy with antibiotics helps to determine the definitive diagnosis. Surgical management is recommended to prevent sequelae of pain and infection.

Bilateral testicular dislocation with unilateral rupture.

A case of bilateral luxation of the testes associated with testicular rupture is presented. Bilateral traumatic dislocation of the testes is uncommon and its association with testicular rupture has not been reported previously. The etiology, classification and treatment of testicular luxation are reviewed briefly. Reports in the literature have indicated initial non-operative management but our case demonstrates the need for early surgical exploration.

Testis cancer: the pendulum swings. Experience in 430 patients.

A review of 430 patients with testis cancer followed for at least 3 years is presented. A protocol for actinomycin D therapy before and after node dissection had been instituted in 1966. In the 58 protocol cases studied there appeared to be an increase in the survival of patients with stage A disease and a significant increase in the survival of patients with stage B disease. There was an improvement in the survival of patients with stage C disease treated with actinomycin D alone. Since 1976 all patients with stages B and C disease have been treated with multiple drug regimens, showing dramatic results. The therapy of testis tumors is multimodal, using lymphadenectomy, radiation therapy and chemotherapy, but the pendulum has swung so that chemotherapy has assumed the vital role in management.

Value of liver scintiscan in staging of testicular neoplasm.

A nineteen-year retrospective study of the usefulness of liver scanning in the staging evaluation of germinal cell testicular neoplasms was undertaken at the National Naval Medical Center. Of 94 patients, 90 (96 per cent) demonstrated accurate correlation between liver scan and histopathologic diagnosis.

Bone marrow acid phosphatase: another look.

Recent reports have indicated that bone marrow acid phosphatase is the most sensitive test in detecting bony metastases. The experience reported herein suggests that falsely positive results may be common, especially in patients with primary hematologic disorders. A plea is made that caution be given to the interpretation of this test so that some patients will not be denied appropriate therapy and the role of bone marrow acid phosphatase can be better defined by long-term followup in such patients.

The value of serial measurement of both human chorionic gonadotropin and alpha-fetoprotein for monitoring germinal cell tumors.

Quantitative serial serum measurements of human chorionic gonadotropin (hCG) and alpha-fetoprotein (AFP) levels using sensitive double-antibody radioimmunoassays were performed in nine patients with germinal cell tumors before and during treatment. The sera of eight of the nine were found to have a hCG marker and five of the nine an AFP marker. The sera of four patients were found to have both. Serial serum levels of hCG, of AFP, or both were useful for monitoring disease activity during therapy in all nine patients. In two patients tumor masses failed to diminish during chemotherapy, but the tumor markers fell appropriately. At surgery one patient had a mature teratoma, the other a mature teratoma with a microscopic focus of an embryonal cell tumor. In one patient tumor reactivation was reflected by the emergence of only one of two previously elevated tumor markers. One patient had a rise in hCG, another a rise in both markers coincident with recurrence of tumor. Serial measurements of AFP and hCG are useful for following the response to therapy of germinal tumors, and can assist in making therapeutic decisions.

Pre- and postejaculation serum acid phosphatase.

A clinical investigation was carried out to analyze the frequency of elevated serum acid phosphatase (SAP) levels after ejaculation and needle biopsy of the prostate gland. Major mechanical manipulative procedures such as needle biopsy caused marked acute elevations in SAP levels but return to normal within twenty-four hours. It is demonstrated that SAP levels remained essentially unchanged after ejaculation. It has been previously shown that procedures such as prostatic massage, catheterization, and endoscopic examination normally do not elicit abnormal SAP levels. Hence any elevation of SAP warrants further evaluation for serious urologic disease.

Vesicosalpingovaginal fistula.

The first reported case of a vesicovaginal fistula in which the fallopian tube serves as a conduit between the bladder and vagina is presented. Clinical manifestations, including recurrent urinary tract infection and cuff abscess, followed vaginal hysterectomy performed three years perviously.

Ureteroduodenal fistula.

A patient with pneumaturia was found to have a ureteroduodenal fistula, only the fourth such lesion reported. However, this patient is unique in that a fistula developed between the stump of a ureter and the duodenum in the absence of a kidney on that side. This case was namaged with a distal skin ureterostomy, thereby avoiding extremely hazardous surgery in the area of the duodenum. This successful conservative managemant offers a sharp contrast to the recommended management of reno-alimentary fistulas. The possible cause in this case based on roentgenographic analysis is discussed.
