Normal adult performance on constructional praxis training tasks.

This study used a balanced factorial design to examine the performance of normal young adults on a parquetry block assembly task that occupational therapists use to treat constructional deficits in adults. Ninety-six 18- to 34-year-old right-handed college students or college graduates (48 men and 48 women) were tested. Independent variables were subjects' sex, presence or absence of color on the model design card, and amount of detail on the model design card. Dependent variables were speed and accuracy of design construction. Results of a factorial analysis of variance with the three variables showed that the subjects' total time scores were significantly related to their sex and to the presence or absence of color and the amount of detail on the design card. Interaction effects were significant for sex by amount of detail and for color by amount of detail. The results were used to generate clinical performance criteria and graded levels of difficulty for this modality. The subjects' learning and test behaviors were examined to generate ideas for treatment and for future research.

Independent living skills groups in a level I fieldwork experience.

Since 1985, the Tufts University-Boston School of Occupational Therapy has experimented with a service model for Level I fieldwork that uses independent living skills (ILS) groups. Under the off-site supervision of the course instructor and a Level I coordinator, students taking an Advanced Group Process course have run ILS groups in community agencies using faculty- and staff-developed group protocols. Populations served have included adolescents and adults with developmental disabilities, adults with psychosocial dysfunction, well geriatric adults, and single mothers. The response of area clinicians has been very positive--the number of settings willing to accept students for this experience has more than doubled since the ILS groups have been introduced. This approach to Level I fieldwork, which does not require as much faculty time as do other service models currently in use, could be applied to other Level I experiences and provide a structure for expanding occupational therapy into nontraditional community settings.

Occupational therapy for adults with perceptual deficits.

Although perceptual retraining has for many years been an accepted part of occupational therapy for adults with perceptual problems stemming from brain injury, few outcome studies on this type of treatment have been conducted. To assure high-quality, cost-effective rehabilitation programs for this population, occupational therapists need to do more research to (a) precisely define perceptual interventions and (b) determine the efficacy of different occupational therapy approaches to perceptual problems. This article offers guidance on the design of future studies to achieve the latter goal by exploring the current research on the effectiveness of perceptual remediation and offers suggestions for future research.

Classroom as clinic: a model for teaching clinical reasoning in occupational therapy education.

One way to help students transfer their academic skills of data gathering and analysis to clinical settings is to teach them the clinical reasoning process outlined by Joan Rogers. This paper describes a format for teaching that process through the use of carefully structured in-class evaluations of physically disabled guest speakers who role-model as clients. Pre- and posttesting of the performance of 78 students during these in-class evaluation sessions showed a significant improvement over a semester in students' abilities to accurately analyze preassessment data and to formulate appropriate treatment plans. Qualitative outcomes are also discussed.

An occupational therapy program for adults with developmental disabilities.

This paper describes an occupational therapy independent living skills programs for adults with developmental disabilities living in group homes. Four clients have participated in this program for 1 year. Verbal reports from house and workshop staffs and written documentation in the clients' records were examined to see if the clients' behaviors changed over the course of their first year in the program. These reports indicate that the clients have moved toward increased independence by showing greater initiative in directing their own care. Treatment issues in group home systems are also discussed.

Occupational therapy treatment goals for adults with developmental disabilities.

This study examined whether occupational therapists were using treatment time to prepare adults with developmental disabilities for independent living opportunities. Eighty-eight occupational therapy evaluations for 54 adults with developmental disabilities in 18 different community agencies in the Boston area were reviewed to ascertain the focus of treatment goals that reflect treatment approaches. Results indicate that remedial goals significantly outnumbered adaptive ones. Within the adaptive goals, self-care and community living skills were equally represented, but some important community skill areas like advocacy, money management, personal care attendant management, sexuality, and transportation received little or no attention. Implications of these findings for therapists, clients, and third-party payers are discussed.

Sexuality counseling for adults with disabilities: a module for an occupational therapy curriculum.

Occupational therapists, as well as other rehabilitation professionals, are aware that sexuality counseling is an important aspect of rehabilitation. However, occupational therapists have limited resources within their profession for training in this area. This paper describes the development, philosophy, goals, objectives, and format of the module "Sexuality and Disability" offered to occupational therapy students at Tufts University. Evaluations from the 288 students who have taken this module to date indicate that all students considered the module a pertinent part of their academic program. Other occupational therapy schools might wish to incorporate a similar component into their curricula to expand the resources within the profession for training in sexuality counseling.

An independent living skills training program.

The growing rehabilitation and consumer movement toward independent community living for disabled adults has placed new demands on the health care delivery system. Programs must be developed for the disabled adult that provide direct training in adaptive community skills, such as banking, budgeting, consumer advocacy, personal health care, and attendant management. An Independent Living Skills Training Program that uses a psychoeducational model is described. To date, 17 multiply handicapped adults, whose average length of institutionalization was 11.9 years, have participated in the program. Of these 17, 58.8% returned to community living and 23.5% are waiting for openings in accessible housing units.
