Perirenal urinary extravasation associated with urethral valves in infants.

We present 7 cases of urethral valves and perirenal urinary extravasation, and review 37 cases reported previously in the literature. The extravasation usually is from the kidney (66 per cent) and often is associated with reflux (70 per cent). It appears to be a pressure-related phenomenon. Although the mortality associated with the condition is significant (32 per cent) renal function is usually preserved. Prompt treatment with correction of metabolic acidosis, and decompression of the tense abdomen and the urinary tract is recommended.

Surgical correction of ureteroceles.

A personal experience in the management of 73 ureterocele cases during the last 17 years and points of technical importance in the correction of this sometimes complex problem are described. Small adult-type ureteroceles are best resected, reimplanting the ureter, since simple unroofing can cause reflux. The infant-type ureterocele with a duplex collecting system requires individualization according to the status of the patient. In ectopic ureterocele cases the bladder outlet often requires repair. The spectrum of management for this group of problems is shown by representative cases.

Urologic manifestations of Wegener granulomatosis.

Wegener granulomatosis is a disease characterized by necrotizing granulomatous angiitis involving the upper and lower respiratory tracts and the kidneys. The introduction of cyclophosphamide in the treatment of Wegener granulomatosis has dramatically altered the rapidly fatal course of the disease and has altered our thinking regarding its management. Recently we have cared for 3 patients who demonstrated a dramatic spectrum of urinary tract pathology related to Wegener granulomatosis. The cases presented offer three points for consideration by the urologist: (1) the manifestations of the disease in the genitourinary tract can be varied and can present in a dramatic fashion; (2) the rapid progression of renal failure and the nearly uniform mortality associated with the disease have changed largely because of the use of cyclophosphamide; and (3) our approach toward patients with renal failure on the basis of Wegener granulomatosis can be altered, and renal transplantation is certainly feasible with the increased longevity afforded these patients by cyclophosphamide.

Carnitine metabolism in human subjects. I. Normal metabolism.

Carnitine (vitamin BT) is a compound which is involved with lipid metabolism. This article deals with the carnitine content of foods and diet, the absorption, transport, storage, and excretion of carnitine in humans. The metabolic functions and biosynthesis of carnitine are also reviewed.

Ileal loop stenosis: a late complication of urinary diversion.

Stenosis of the ileal conduit was a late complication in 12 patients (10 adults) who underwent urinary diversion by this means at our hospital. The duration of the ileal loop at diagnosis ranged from 4 to 14 years, averaging 9 years. Loop stenosis was generally without symptoms and was suggested only by routine excretory urography, while loopography confirmed the diagnosis. The etiology of the condition is not clear. A variety of possible factors is considered, including microvascular ischemia, urine-borne toxic material, infectious and allergic stimuli and an immunologic defect. The pathogenesis of the condition appears to be based upon a chronic inflammatory reaction, with progressive fibrosis in the mucosa and submucosa of the ileal segment.

Experience with the electrohydraulic disintegrator.

Experience with the electrohydraulic lithotrite in treating 14 cases of bladder calculi is reported. This revolutionary lithotrite is applicable to most bladder stones and is associated with few complications.

Control of massive prostatic bleeding with angiographic techniques.

In the 4 cases reported herein angiographic techniques with selective bilateral hypogastric embolization were used to control severe prostatic hemorrhage. The technique is offered as an alternative, non-operative procedure in the management of bleeding when other modes of therapy contain significantly higher potentials for morbidity and mortality.

Protein value of two imitation milks. Chemical and biologic assays.

Two imitation powdered milk products were compared with whole milk for both quantity and quality of protein. One product had comparable quantity of protein, but inferior quality, while the other had comparable quality but inferior quantity. Therefore, neither was equivalent to whole milk when a reconstituted serving of the product was calculated in terms of the U.S. RDA. However, a single serving of either product would constitute 10 per cent of the U.S. RDA for protein. Calcium contribution in terms of U.S. RDA would also be significant, but would not equal whole milk for these particular products. Thus ingredient composition varies widely for imitation milk products. These products were not evaluated for other nutrients, such as fats and vitamins.

Exstrophic anomalies: recent advances and long-term outlook.

Exstrophy-epispadias complex has been a difficult disease to treat. As in the minds of the public and most physicians alike, these children are crippled with life-long ailments and multiple operations. Much of the morbidity of this condition relates to failure to preserve urogenital tract function in these children. In recent years, through better understanding of the exstrophic anatomy and improved surgical techniques, complete repair of the exstrophic anomaly has become possible as soon as the child is born. Results of such closure indicate that such repair is not only feasible but mandatory for a successful outcome. Continence rate and cosmetic appearance are superior to the conventional 3-stage technique. Such repair is also socially acceptable as the parents will bring home a normal-looking baby with much fewer operations expected in the future. It is anticipated that complete one-stage exstrophy closure in the newborn period will revolutionize the outcome of all exstrophy patients.
