Techniques of adjustable suture strabismus surgery.

Adjustable sutures allow eye muscle positions to be changed following strabismus surgery, when the patient is awake and able to cooperate in muscle balance testing. Bow-tie-knot and sliding-handle sutures can be adjusted within 24 hours after surgery. Scleral traction sutures, which can be used to fix the globe to periorbital tissues during postoperative healing, can be adjusted for up to 14 days after surgery. We describe these techniques as well as an adjustable loop suture for conjunctival closure.

Ammonium production in sediments inhibited with molybdate: implications for the sources of ammonium in anoxic marine sediments.

Ammonium production in the presence of specific inhibitors of sulfate reduction and methanogenesis was investigated in six marine sediments which differed in bulk properties and organic matter input. In all cases, little effect of the inhibitors on ammonium production was observed, although sulfate reduction was suppressed by molybdate. This gives evidence that the processes of fermentation and hydrolysis are of primary importance in ammonium generation at the sites studied. Although sulfate reduction rates may appear to be coupled to ammonium production rates, sulfate reduction does not necessarily contribute directly to generation of ammonium in marine environments.

A stochastic population projection system based on general age-dependent branching processes.

"Algorithms for a stochastic population process, based on assumptions underlying general age-dependent branching processes in discrete time with time inhomogeneous laws of evolution, are developed through the use of a new representation of basic random functions involving birth cohorts and random sums of random variables. New algorithms provide a capability for computing the mean age structure of the process as well as variances and covariances, measuring variation about means. Four exploratory population projections, testing the implications of the algorithms for the case of time-homogeneous laws of evolution, are presented. Formulas extending mean and variance functions for unit population projections...are also presented. These formulas show that, in population processes with non-random laws of evolution, stochastic fluctuations about the mean function are negligible when initial population size is large. Further extensions of these formulas to the case of randomized laws of evolution suggest that stochastic fluctuations about the mean function can be significant even for large initial populations."

Effects of hypoglycemic agents on vascular complications in patients with adult-onset diabetes. VII. Mortality and selected nonfatal events with insulin treatment.

The University Group Diabetes Program is a long-term prospective clinical trial designed to evaluate the effects of various hypoglycemic agents on vascular complications in patients with asymptomatic adult-onset diabetes. Mortality and blood glucose levels were determined as well as certain nonfatal events for patients assigned to diet alone or to either of two insulin treatment regimens. Lower levels of blood glucose with mean values close to normoglycemia were achieved in the treatment group in which the insulin dosage was adjusted to achieve normoglycemia compared with the levels achieved in patients treated with diet alone or with a fixed dose of insulin. In spite of differences in blood glucose levels among the treatment groups, there were only minor differences in the occurrence of fatal or nonfatal events.

Surgical management of Cushing's syndrome.

Twenty-seven operations for Cushing's syndrome were reviewed. Included were five cases of adenoma and 22 of adrenal cortical hyperplasia. Preoperative laboratory data, particularly the metyrapone test, were highly accurate in distinguishing adenoma from hyperplasia. Bilateral flank incisions are preferable to the transabdominal approach, with fewer complications, less postoperative ileus, and shorter hospital stays. The inability to inspect both glands simultaneously is of little consequence because biochemical testing data are sufficiently accurate to obviate the need for gross evaluation. The use of perioperative prophylaxis seems to be of importance for preventing thromboembolism in these hypercoagulable patients. The preoperative use of adrenal cortical blocking agents has not proved to yield significant advantages.

Lipid cell tumor of the ovary: an ultrastructural study.

An ovarian lipid cell tumor without Reinke's crystalloids in a woman with secondary amenorrhea, minimal hirsutism, and elevated 17-ketosteroid excretion was studied by light and electron microscopy. Tumor cells were found in small clumps or scattered singly within a collagenous matrix. The cytoplasm of the tumor cells contained abundant smooth endoplasmic reticula, numerous mitochondria with tubular cristae, lipid droplets, lysosomal dense bodies, and concentric membranous whorls, characteristic of steroidogenic cells. In addition, "peripheral canalicular systems" were found at the outer margins of the nests of the tumor cells. These "peripheral canalicular systems" were bordered by the cell membranes and the surrounding collagenous stroma into which microvilli projected. Since the intercellular canalicular system present between the tumor cells was continuous with the "peripheral canalicular system," both systems probably have a common function related to steroid metabolism. The intercellular and "peripheral" canalicular systems and cytoplasmic microfilaments found in this tumor suggest that this ovarian lipid cell tumor was derived from the ovarian stroma.

A parametric algorithm for computing model period and cohort human survival functions.

A parametric algorithm was developed for computing model cohort and period survival functions used in making projections of human populations. Two levels of parameters were used in developing the algorithm. The algorithm provides a method for calculating model period survival functions as a function of an expectation of life at birth; model cohort survival functions are calculated as a function of a time series of period expectations of life at birth. Expectations of life at birth ranging from about 35 to 110 years in both sexes may be accommodated by the algorithm.
