Assessing adolescents' prosocial behavior: the Family Helping Inventory.

This study examined the structure and psychometric properties of a self-report measure of adolescents' helping behavior within the family--the Family Helping Inventory (FHI)--and of its two scales, the Sibling Helping Scale (SHS) and the Parent Helping Scale (PHS). The FHI was administered to 202 adolescents along with several existing measures of prosocial tendencies and with a measure of Machiavellianism. Factor analyses yielded four internally consistent subscales for the SHS and five for the PHS, all of which were conceptually related to inventories reflecting family support among adults. Both convergent and discriminant validity were demonstrated as well.

Induction of labor as compared with serial antenatal monitoring in post-term pregnancy. A randomized controlled trial. The Canadian Multicenter Post-term Pregnancy Trial Group.

BACKGROUND: The rates of perinatal mortality and neonatal morbidity are higher for post-term pregnancies than for term pregnancies. It is not known, however, whether the induction of labor results in better outcomes than does serial fetal monitoring while awaiting spontaneous labor. METHODS: We studied 3407 women with uncomplicated pregnancies of 41 or more weeks' duration. The women were randomly assigned to undergo induction of labor or to have serial antenatal monitoring and spontaneous labor unless there was evidence of fetal or maternal compromise, in which case labor was induced or cesarean section was performed. In the induction group, labor was induced by the intracervical application of prostaglandin E2. Serial antenatal monitoring consisted of counts of fetal kicks, nonstress tests, and assessments of amniotic-fluid volume. The outcomes we measured were the rates of perinatal mortality, neonatal morbidity, and delivery by cesarean section. RESULTS: Among the 1701 women in the induction group, 360 (21.2 percent) underwent cesarean section, as compared with 418 (24.5 percent) of the 1706 women in the monitoring group (P = 0.03). This difference resulted from a lower rate of cesarean section performed because of fetal distress among the women in the induction group (5.7 percent vs. 8.3 percent, P = 0.003). When two infants with lethal congenital anomalies were excluded, there were no perinatal deaths in the induction group and two stillbirths in the monitoring group (P not significant). The frequency of neonatal morbidity was similar in the two groups. CONCLUSIONS: In post-term pregnancy, the induction of labor results in a lower rate of cesarean section than serial antenatal monitoring; the rates of perinatal mortality and neonatal morbidity are similar with the two approaches to management.

Elevated liver enzymes and thrombocytopenia in the third trimester of pregnancy: an unusual case report and a review of the literature.

A woman presented in the third trimester of pregnancy with epigastric pain, elevated liver enzymes, and thrombocytopenia. The frozen-section liver biopsy findings were compatible with acute fatty liver of pregnancy. The light and electron microscopic findings were those of preeclampsia. All clinical and laboratory abnormalities resolved before delivery.

The generous elderly: naturalistic studies of donations across the life span.

The relation between age and altruism was examined in two experiments conducted in naturalistic settings. In both studies, individuals from 5 to over 75 years of age had opportunities to donate money to a charity concerned with the welfare of infants with birth defects. The effect of a variable designed to evoke the needs rule was also observed by using both pregnant and nonpregnant charitable solicitors. Results of both studies yielded a significant main effect of solicitor condition on donations. Study 1 yielded a linear increase with age in numbers of people donating; however, retired persons generally donated less money than did younger adults. In Study 2, which controlled for financial costs, elderly persons proved to be the most generous. These results underscore the importance of personal resources and helping opportunities in influencing altruism among older persons.

Does prolonged preterm premature rupture of the membranes predispose to abruptio placentae?

The association between prolonged preterm premature rupture of the membranes and abruptio placentae was investigated in a case-control study. The incidence of abruptio placentae among 143 women with singleton pregnancies at less than 34 weeks' gestation who had ruptured membranes for at least 24 hours was 5.6%, significantly higher than the 1.4% observed among 143 randomly selected controls without preterm rupture of the membranes (P less than .05). Among patients with prolonged preterm premature rupture of the membranes, those who experienced vaginal bleeding before the onset of labor had a significantly higher risk for abruptio placentae than women who did not bleed (24 versus 2.5%; P less than .001). Regardless of the mechanism by which this association is produced, the clinician should be aware of this potential complication, especially in patients who have recurrent bleeding episodes during the period of expectant management.
