[Measurement of the appearance of volatile fatty acids in the portal vein during digestion in conscious pigs].

A method used to measure the nutrient exchanges between the intestinal lumen and the portal blood was applied to 3 pigs receiving 800 g of a diet containing 6% of cellulose. During a postprandial period of 24 hrs., the mixture of volatile fatty acids (90 mM/h) appearing in the portal vein was composed of acetic acid (57%), propionic acid (30%), butyric acid (9%), isovaleric (2.5%) and valeric acid (1.5%). The composition of this mixture differed from that of the colic contents because of its acetic acid enrichment at the expense of butyric acid. The resulting energy supply to the animal represented about 600 to 700 kcal/24 hrs.

An improved method for electron microscopic study (SEM and TEM) of vessel prosthesis anastomoses in microsurgery.

In order to study the replacement of vessels by biological vascular prostheses in microsurgery, an improved method for scanning electron microscopy preparation which is rapid and minimizes artifacts was developed. To limit endothelial surface injury, the vessel and prosthesis unity is, at first, prefixed by injection into one end of a 2.5% isotonic solution of glutaraldehyde. After fixation by osmium tetroxide, the samples are dehydrated by a graded series of acetone solutions and critical point dried. They are then trimmed around each suture line to pieces of 1 cm including 1/3 vessel and 2/3 prosthesis for further orientation and sectioned longitudinally, with a new razor blade, for microvascular anastomoses observations. The same chemical treatment applied to samples for transmission electron microscopy studies permitted verification of the good preservation of the cellular structures.

[The rumen fermenter].

The two main pregastric compartments of ruminants (rumen and reticulum) can be compared to an anaerobic fermenter which allows them to use energetic and nitrogen substrates which are indigestible by monogastric animals. The aim of this bibliographic review is to describe the main features (characteristics) of this fermenter, focusing on the role of microbes (bacteria, protozoa and fungi), on the degradation of cell-wall carbohydrates (cellulose, hemicelluloses, pectic substances), on nitrogen digestion and finally on the utilisation of the major minerals and trace elements.

Hodgkin's disease in the elderly. A series of 30 patients aged older than 70 years.

Thirty patients aged older than 70 years formed 6.9% of all cases of previously untreated Hodgkin's disease seen in Bordeaux Cancer Center over a 20-year period. The subtype of mixed cellularity was predominant in this age group (P = 0.00035). One patient received no treatment; 20 patients received primary chemotherapy (14 polychemotherapy); all but 2 of the 18 patients in clinical Stages I or II received radical radiotherapy. Median survival for all patients is 15 months. In five cases the treatment could be held responsible for death. Eleven patients died of Hodgkin's disease within a 36-month period. Five patients are alive and the nine other patients died from other causes. The disease-free survival figures showed that one third of the patients could be considered as cured.

Metabolism and function of gangliosides in developing neurons.

Previous experiments have shown that the addition of a mixture of gangliosides to the growth medium induced morphological changes in primary neuronal cultures, producing especially a trophic effect and a sprouting of neurites (neuritogenesis). The study reported here examined the changes of some biochemical parameters that paralleled the morphological modifications of cultured neurons from chick brain hemispheres treated with gangliosides. Neurons cultured from 3 to 7 days in the presence of various concentrations of a purified mixture or of single-species of gangliosides (GM1, GD1a, GT1b) revealed that these glycolipids were easily incorporated into the cells as a function of their exogenous concentrations. Incubation of neurons with N-acetyl-D-[U-14C]mannosamine showed a final labeling of all endogenous cellular and exogenous incorporated gangliosides; however, the radioactivity recovered decreased as a function of the number of sialic acid units of the exogenously added gangliosides. The treatment of neuronal cells from 3 to 7 days in culture with a mixture of 10(-8) M and 10(-5) M gangliosides led to the following observations on some neurochemical parameters: no effect on the influx of choline and dopamine; no effect on the spontaneous choline efflux, whereas the K+-provoked one is abolished; decrease of the spontaneous and K+-stimulated release of dopamine; no effect on the spontaneous release of GABA for 10(-8) M gangliosides but an increase of both spontaneous and K+-provoked release for 10(-5) M gangliosides. The data suggest that the possible insertion of gangliosides into the neuronal membranes may imply structural modifications that may influence enzymatic activities, neurotransmitter transport, and finally, some nerve cell mechanisms.

[Histological types of 876 breast cancers according to the 1981 WHO classification].

This work presents the results of the classification by histological types of 876 cancers, operated on between November 1979 and December 1982, according to the new WHO's classification of 1981. Noninfiltrating cancers represent 6.5 per cent of the total. Among the infiltrating cancers (93.5%), the individualization of the type "infiltrating duct carcinoma with a marked intraductal component" (14.5%) is interesting for this form justifies a special treatment in consideration of its multicentric and sometimes bilateral character. Its purely quantitative criteria of definition is subject to criticism and discussion.

[Evaluation of prognostic factors in breast cancer].

The survival of the patients with breast cancer depends on many factors. The TNM system, which is the most widely used prognostic system takes into account only the size, and the local extension of the tumor, and presence or not of axillary or supraclavicular lymph nodes. The axillary nodes involvement is the single precise prognostic variable. However, other valuable and important factors are to be considered. As a matter of fact, the growth rate of the tumors, their estrogen and progesterone receptor status, and their histopathological grade may reflect with a better accuracy the tumor biology, and should be integrated among the variables to study before the choice of an adjuvant treatment.

Nonspecific interaction of the lac repressor headpiece with deoxyribonucleic acid: fluorescence and circular dichroism studies.

The nonspecific interaction of the short headpiece, the NH2-terminal domain of the lac repressor, with natural DNA and alternating polydeoxynucleotides has been studied by means of fluorescence and circular dichroism. The important quenching of the intrinsic tyrosine fluorescence of the headpiece upon complexation has been used for the determination of the binding isotherms under various environmental conditions. By comparison with theoretical binding curves, we have determined a physical site size of three base pairs. The "perturbated" site size as determined from circular dichroism measurements (about four base pairs) is slightly greater. As in the case of the entire lac repressor, the interaction is strongly ionic strength dependent. The plots log Kobsd as a function of log [NaCl] are linear for salt concentrations greater than 50 mM for the interaction with DNA and greater than 25 mM for the interaction with poly[d(G-C)]. From the slopes of the linear parts of these plots, we determine a number of three electrostatic interactions, assuming no anion release from the protein upon complexation. This value is independent of pH. On the contrary, the association constant Kobsd depends on pH. Complexation requires the protonation of one titrating group of the headpiece with a pK value of 6.7 +/- 0.3, probably the residue histidine-29. The finding is in favor of the idea that the lac repressor interacts with DNA via two headpieces as earlier work has shown that the interaction of the lac repressor with DNA requires the protonation of two groups of the protein.

Binding of lac repressor headpiece to poly[d(A-T)]. A thermal denaturation study.

The binding of the lac repressor headpiece to poly[d(A-T)] has been investigated using thermal denaturation experiments. The binding of the headpiece leads to a stabilization of the double-stranded structure. Competition experiments using circular dichroism measurements confirm that the headpiece binds stronger on the double-stranded poly[d(A-T)] than on a single-stranded nucleic acid like poly(A). Theoretical analyses of the melting curves allow the determination of the binding constant and of the site size on the poly[d(A-T)]. This latter value is found to be 4 base pairs, in good agreement with that determined by other experimental approaches, and is much smaller than the values previously found for the lac repressor.

A simple method for estimating the ventilatory response to CO2 in infants.

We report a new noninvasive method for the estimation of CO2 response in uncooperative infants. By comparing the changes in inspired minus alveolar PO2 breathing air and 4% CO2, an indirect estimate of increase in alveolar ventilation can be obtained. This report is a comparison of 3 possible indirect methods: changes in inspired minus alveolar PCO2 (delta AIPCO2), changes in inspired minus alveolar PO2 (delta IAPO2), and changes in transcutaneous PO2 (TcPO2) with the standard steady-state method for ventilatory response to CO2. Twenty-one comparisons were made, 16 on 7 preterm infants, and 5 on an older child (at 2.5 and at 4 yr of age), with central hypoventilation syndrome. We found that changes in delta IAPO2 gave the best correlation with changes in minute ventilation (r = 0.83, p less than 0.001), that changes in delta AIPCO2 were less valid (r = 0.66, p less than 0.001), and that there was no correlation with changes in TcPO2. We conclude that changes in inspired PO2 minus alveolar PO2 can be used in uncooperative infants to estimate the ventilatory response to CO2.

[Inflammatory breast cancers: correlation between anatomopathology and steroid receptor assay].

From a series of 89 clinically inflammatory breast cancers, the authors settle a subgroup of tumors, the infiltrative ductal carcinomas with a pleomorphic structure. They have better histological and hormonal prognostic criteria than the other carcinomas, especially the infiltrative ductal carcinomas with an atypical structure: lower SBR grading (p = 6.10(-5)), estrogen and progesterone receptors positive rate more frequently (p = 10(-4)). A prospective study will allow to confirm the better response to treatment and especially the better survival rate of this metastatic high risk breast cancer subgroup.

[In vitro estimation using radioactive phosphorus of the phosphorus requirements of rumen microorganisms].

Microbial requirements for P were assumed to be a function of the amount of microbial protein synthesis (microbial growth) and of the quantity of organic matter (OM) fermented in the rumen. The relationships among P incorporation into microbial matter and protein synthesis, ammonia utilization, volatile fatty acid (VFA) production and organic matter fermented (OMF) were studied in short-term incubations (3 h) using 32P-labelled phosphate. The amount of P incorporated was calculated from extracellular phosphate pool specific activity and the radioactivity incorporated into the microbial sediment during incubation (table 1). The inocula came from sheep fed a protein-free purified diet. In order to vary the intensity of fermentation, carbohydrates with a wide range of degrees of enzymatic susceptibility were used as substrates and the medium was either provided or was deficient in S and trace elements (table 4). Nitrogen was supplied as ammonium salts. Linear regression analyses showed that P incorporation was positively correlated with the criteria of protein synthesis and OM fermentation (figs. 1, 2, 3, 4). However, there was significant phosphorus incorporation when the value for nitrogen incorporation was zero (equation A: (Pi (mg) = 0.162 NH3-N + 0.376; r = 0.9). This was assumed to result either from energetic uncoupling (fermentation without concomitant bacterial growth) or from the lysis of cold microbial cells only. Equation A would reflect total P incorporation and equation A' Pi (mg) = 0.162 NH3-N (mg), net P incorporation. It was assumed that in vitro microbial requirements for P were in the range of 30-70 mg of P/liter of medium for 3-hour incubation, depending on the intensity of fermentation. From a mean value of microbial N yield of 30 g/kg of DOMR (organic matter apparently digested in the rumen), it was calculated that the total and net P requirements in vivo were 6 and 4.9 g/kg of DOMR, respectively, corresponding to 3.9 and 3.2 g/kg of DOM (digestible organic matter). From equation D, relating Pi to OMF, the P requirements were about 4.4 g/kg of DOM. It is suggested that microbial requirements for P varied from 3 to 5 g of P/kg of DOM, depending on the efficiency of microbial synthesis and the extent of carbohydrate fermentation. These results, considered as indicative, should be checked in in vivo experiments.

Effect of continuous positive airway pressure on the ventilatory response to CO2 in preterm infants.

The effect of continuous positive airway pressure (CPAP) on the ventilatory response to CO2 in newborn infants is unknown. The CO2 response to 4% CO2 in air was studied in nine preterm infants without lung disease before and during administration of CPAP (4 to 5 cm H2O) delivered by face mask. Minute ventilation, tidal volume, respiratory frequency, and end-tidal PCO2 were measured, and the slope and intercept of the CO2 response were calculated. Respiratory pattern and changes in oxygenation were also analyzed by measuring inspiratory and expiratory time, mean inspiratory flow, mean expiratory flow, effective respiratory timing, end-tidal PO2, and transcutaneous PO2. CPAP significantly decreased minute ventilation from 278.7 to 197.6 mL/min/kg (P less than .001). Tidal volume and respiratory frequency were also significantly decreased. The slope of the CO2 response during CPAP was not significantly different from the slope before CPAP (36 v 33 mL/min/kg/mm Hg, P greater than .1), but the intercept was shifted to the right (P less than .001). The decrease in respiratory frequency was primarily due to a prolongation of expiratory time (P less than .05). In addition, transcutaneous PO2 increased during administration of CPAP (P less than .001). These findings indicate that: (1) CPAP significantly decreases ventilation in preterm infants without lung disease, affecting both tidal volume and respiratory frequency; (2) CPAP does not appreciably alter the ventilatory response to CO2; (3) the changes in respiratory frequency are primarily accounted for by a prolongation of expiratory time; (4) CPAP improves oxygenation.

[Hypoplasia of the right ventricle and cyanosis due to left-right atrial shunt within the framework of Turner's syndrome. Surgical closing of the interatrial connection after occlusion testing with balloon catheterization].

The authors describe the unusual cardiac malformation in a young girl with Turner's syndrome: right ventricular hypoplasia and atrial septal defect, resulting in central cyanosis. A trial of temporary occlusion of the atrial septal defect at cardiac catheterisation before surgical closure suppressed the right-to-left shunt and the cyanosis without significantly changing the hemodynamic parameters. The authors review the types of cardiovascular abnormalities associated with Turner's syndrome and emphasize the value of a temporary occlusion test of defects in certain cyanotic cardiac malformations with atrial right-to-left shunts; it may lead to a relatively simple surgical procedure, as in the case described.

Spectroscopic study of the effects of C-alkylation of N-acetoxy-N-acetyl-2-aminofluorene on some properties of the carcinogen-modified DNA.

Several physicochemical properties of DNA reacted in vitro with the carcinogen metabolite N-acetoxy-N-2-acetylaminofluorene (N-AcO-AAF) are compared to those of DNA reacted with two C-alkylated derivatives, viz. N-acetoxy-7-ethyl-N-2-acetylaminofluorene (N-AcO-EtAAF) and N-acetoxy-7-n-butyl-N-2-acetylamino-fluorene (N-AcO-ButAAF). Ultraviolet absorption, high resolution derivative melting curves and circular dichroism techniques are employed in the present study. C-alkylation of N-AcO-AAF influences the conformational modifications of DNA induced by the covalent binding of the metabolite. The size of the alkyl group seems to determine the effect on DNA conformation (e.g., the bulkiest N-AcO-ButAAF denatures the most DNA). However, certain similarities between the three metabolites are observed, such as the preferential binding to G-C rich regions and the carcinogen-carcinogen and carcinogen-DNA base stacking interactions.
