Spontaneous coronary artery dissection is a rare event, felt to be caused by intimal disruption and formation of a false lumen in the vessel wall. Compression of the true lumen by intramural hematoma may result in obstruction to blood flow, myocardial ischemia, infarction, and ultimately, death. We report the first instance (to our knowledge) of survival in a man with spontaneous dissection of the left anterior descending coronary artery.

Managing difficult staff interactions: effectiveness of assertiveness training for SCI nursing staff.
Sixty-one members of the nursing staff of a spinal cord injury (SCI) center at a Department of Veterans Affairs Medical center were assessed before and after a 3-hour class on managing difficult staff interactions. The assessment consisted of two questionnaires that were developed to assess staff discomfort when dealing with common interpersonal situations in rehabilitation. The class involved a lecture and discussion, modeling, and behavior rehearsal. Repeated measures analysis of variance showed an overall effect of training. There was also a statistically significant relationship between staff members' educational levels and the level of discomfort related to interpersonal interactions that they experienced after attending the class. Registered nurses reported a statistically significant decrease in discomfort. The conclusion was that the class was effective in helping registered nurses reduce their discomfort in some difficult rehabilitation situations.
Bacterial toxin-induced pulmonary epithelial cytotoxicity and the protective effect of dibutyryl-cAMP.
Bacterial infection is the most common cause of the adult respiratory distress syndrome which, in turn is associated with endothelial capillary permeability and alveolar oedema. Previously, we have demonstrated the direct cytotoxicity of the bacterial toxins Pseudomonas aeruginosa exotoxin A (Exo A) and Salmonella enteritidis lipopolysaccharide (LPS) on pulmonary endothelial cells. The purpose of this study was to investigate the effect of Exo A and LPS on pulmonary epithelial cells in vitro. We also tested the protective effect of dibutyryl cyclic adenosine monophosphate (db-cAMP) on Exo A-induced cytotoxicity. In cultured rat alveolar epithelial cells (RAEC) Exo A caused cytotoxicity as measured by 51Cr release from these cells. LPS did not injure RAEC's. Pretreatment of RAEC with db-cAMP (1 mM) attenuated Exo A induced cytotoxicity. We conclude that (1) Exo A directly injures epithelial lung cells and may contribute to lung injury in cases of bacterial infection; (2) db-cAMP protects alveolar epithelial cells against Exo A-induced cytotoxicity and (3) alveolar epithelial cells in this model are resistant to LPS induced injury.

The DNA sequence and comparative analysis of human chromosome 20.
The finished sequence of human chromosome 20 comprises 59,187,298 base pairs (bp) and represents 99.4% of the euchromatic DNA. A single contig of 26 megabases (Mb) spans the entire short arm, and five contigs separated by gaps totalling 320 kb span the long arm of this metacentric chromosome. An additional 234,339 bp of sequence has been determined within the pericentromeric region of the long arm. We annotated 727 genes and 168 pseudogenes in the sequence. About 64% of these genes have a 5' and a 3' untranslated region and a complete open reading frame. Comparative analysis of the sequence of chromosome 20 to whole-genome shotgun-sequence data of two other vertebrates, the mouse Mus musculus and the puffer fish Tetraodon nigroviridis, provides an independent measure of the efficiency of gene annotation, and indicates that this analysis may account for more than 95% of all coding exons and almost all genes.
Teaching strategies used by expert nurse practitioner preceptors: a qualitative study.
Nurse practitioner (NP) programs utilize expert clinicians as preceptors for their students. Little is written regarding this important role. This study explored teaching strategies used by expert preceptors. Fifteen preceptors in a master's level NP program participated in this qualitative study. The study identified (a) orientation strategies, (b) strategies used with all students, and (c) strategies used differentially according to level of learner. Two distinct preceptor styles emerged from the data ("incremental structure" and "sink or swim"). Preceptors also identified characteristics of student readiness for increased responsibility. Results of this qualitative study can be helpful in preceptor orientation and continued education.

Wellness plays an important role and represents a major direction in today's health care delivery system. Senior Wellness Services are human assistance services for senior members of the community designed to link established hospital resources with new services that will affect the quality of life. Successful Wellness services involve the potential use of many resources within the hospital and the community. These services are designed with the older adult in mind, and offer participants lifestyle education, clinical counseling, and peer support. Wellness programs help fulfill the need for preventive health education while improving communication between the hospital and the community it serves.
