[Local anesthetics: simultaneous HPLC determination of lidocaine (Xylocaine) and bupivacaine (Marcaine) in human serum].

In order to monitor loco-regional anaesthesia in the upper limbs with a combination of two anaesthetics, the authors participated in a clinical pharmacology study. They describe a method of simultaneous assay of bupivacaine and lignocaine by direct phase HPLC. In particular, the use of heptane sulfonic acid (counter-ion) in the mobile phase allows the elution of the local anaesthetics to be controlled. The criteria of reliability studied enable this technique to be considered for routine use.

Model independent pharmacokinetic study of cis-dichlorodiammineplatinum (II).

The disposition of cis-dichlorodiammineplatinum (CDDP) was evaluated in 8 patients with cancer. They received 80 mg/m2 of CDDP as a 20 min i.v. infusion. Sixteen blood samples and sixteen urine samples were obtained over 6 hours following the completion of cis-platinum infusion and were assayed for total platinum by flameless atomic absorption spectrometry. There was a great variation among maxima urinary concentrations (123 to 1436 mumol . l-1) and an important decrease of diuresis appeared 3 hours after the infusion. Model independent pharmacokinetic parameters: distribution volume and renal elimination clearance were determined. The calculated distribution volumes suggest an extremely rapid diffusion of CDDP into the peripheral compartment. The ratio of total platinum clearance to creatinine clearance decreased 75 min after infusion, this being the consequence of the variation of protein binding or nephrotoxicity.

[Cleidocranial dysostosis. Contribution to its study apropos of 2 personal cases (an isolated and a familial form)].

Cleidocranial dysostosis is a rare embryopathy affecting the whole osseous system. Anomalies predominate on skull, chest and dentition but the whole skeleton can be affected especially pelvic girdle and spine. An autosomal transmission is often demonstrable; isolated forms are very uncommon. We present two cases fortuitously diagnosed in our ward: a familial cleidocraniopelvic major form and an incomplete isolated form. The treatment must be especially oriented towards dental malformations.

[Angiodysplasia of the ileum and cardiomyopathy].

A patient aged 73 had repeated episodes of melaena and a systolic murmur suggesting mitral insufficiency. An obstructive cardiomyopathy was diagnosed on echocardiography, while the gastro-intestinal tract investigations proved negative (gastro-duodenal endoscopy, barium meal and follow-through), until ileal vascular tumours were diagnosed by selective mesenteric arteriography. Diagnosis could only be confirmed on exploratory laparotomy, and definitive treatment was undertaken at the same time. The authors discuss the practical and theoretical consequences, including the need for thorough gastro-intestinal investigations in patients with repeated intestinal haemorrhage, particularly in the presence of a systolic murmur which is not necessarily the sign of aortic stenosis.

Nonviral intranuclear inclusions in a nerve biopsy: an ultrastructural study.

During the ultrastructural study of a nerve biopsy from a diabetic patient suffering from chronic polyradiculoneuritis, we observed that a significant number of endoneural fibroblasts had abnormal nuclei. They contained a round, centrally located inclusion consisting of numerous irregularly arranged filaments approximately 15 nm in diameter. No such lesion in peripheral nerve has previously been reported. Interpretation of these abnormalities is difficult.

Familial Creutzfeldt-Jakob disease with extensive degeneration of white matter. Ultrastructure of peripheral nerve.

A case of Creutzfeldt-Jakob disease (CJD) in a 52-year-old man is described. At post mortem, extensive involvement of white matter was seen. A few similar cases have been reported mainly by Japanese authors. Our patients belonged to a French family in which 14 cases of CJD over three generations have been recorded. One of the patient's first cousins also had extensive white matter involvement. This is an unusual panencephalopathic form of CJD. The causes of the involvement of white matter are unknown. There was no clinical evidence of neuropathy but an electron-microscopic study of biopsied superficial peroneal nerve showed it to be present. Study of peripheral nerves is suggested for all patients with CJD.

[Treatment of human African trypanosomiasis].

The melarsoprol is still the only active drug during the nervous stage of African human trypanosomiasis, its prescription still relies on empiric rules and because of its toxicity, it appears necessary to study in gaseous phase chromatography and mass spectrometry, its pharmacokinetic in the serum and the cerebrospinal fluid (CSF) of the patients at the evolutive stage of the illness. Because of the early penetration of the Trypanosoma into the central nervous system, new therapeutic investigations should only search molecules capable to pass through the blood brain barrier: this in unfortunately not always the case, the rifamycins diffusing easily into CSF, they seem to offer a reliable way of research. For these therapeutic investigations, sheep is an easy to handle experimental model and of a reasonable cost.
