[Host factors that changes the distribution of immunotoxin].

Mouse monoclonal antibody A 7, which was raised against human colon cancer, was used for preparing the conjugates with neocarzinostatin, mitomycin C and adriamycin. The tissue distribution of these three conjugates were examined in athymic nude mice transplanted with human colon cancer. The distribution in tumor was different in these three conjugates. The route of administration of the conjugate affected its distribution in tumor tissues. In the case of tumor transplanted in the back of mice, intravenous administration seemed to superior to intraperitoneal one. In clinical trials of immunoconjugate composed of A 7 and polypeptide anticancer drug neocarzinostatin (A 7-NCS), human anti-mouse antibody (HAMA) was observed in most cases without serious immune response such as anaphylactic shock. Human antibody against neocarzinostatin could not be detected in any case receiving A 7-NCS.

[Immunoresponses and efficacy after arterial infusion of immunoconjugate A7-NCS in patients with colon cancer].

We prepared an immunoconjugate, A7-NCS, of a mouse-derived anti-human colon cancer monoclonal antibody A7 and the macromolecular protein anticancer agent neocarzinostatin (NCS), and evaluated changes in human anti-mouse antibody (HAMA) by ELISA in the serum of patients intraarterially administered this agent. IgG and IgM class HAMA were detected in all patients, but no IgE class HAMA. In patients with stage V disease, the survival rate was higher in a group treated with A7-NCS at an NCS dose of 4,000 units or more than in that treated at an NCS dose of less than 4,000 units. In these patients, the survival rate was higher in a group treated with A7-NCS at an NCS dose of 4,000 units or more than in one not treated with it. These results suggest the usefulness of A7-NCS administration at high dose for prolonging survival of patients with advanced colon cancer.

Suprasellar ectopic pituitary adenoma: case report and review of the literature.

The occurrence of a totally suprasellar ectopic pituitary adenoma in a 71-year-old man is described. The tumor was attached to the pituitary stalk, extending upward toward the third ventricle. No intrasellar lesion was observed. Histological examination revealed a pituitary adenoma with large numbers of eosinophilic cells with moderate nuclear polymorphism and rare mitosis. Immunohistochemical staining revealed that the tumor cells were strongly positive for anti-adrenocorticotropic hormone antibody. A review of five previously reported intracranial ectopic pituitary adenomas revealed that two were silent corticotropic tumors and two occurred with Cushing's syndrome.

[Clinical investigation of flumazenil in reversing the respiratory effects of diazepam].

Eight patients (ASA I-II) scheduled for a minor surgery under lumbar epidural or spinal anesthesia were studied. The patients were sedated with approximately 0.2 mg.kg-1 of bolus injection followed by 0.2 mg.kg-1.min-1 of continuous infusion of diazepam. In order to evaluate the reversing effects of 0.2 mg of flumazenil on the respiratory effects of diazepam, the following parameters were examined at three points, namely 1) control period, 2) during infusion of diazepam, and 3) ten minutes following injection of flumazenil; tidal volume, respiratory frequency, minute volume, inspiratory time/total time of breath (Tl/Tt) and %RC with a respiratory inductive plethysmograph, ventilatory response to carbon dioxide using a modified Read's rebreathing method. We found that with flumazenil, tidal volume and the slope of the ventilatory response curve to carbon dioxide increased and arterial carbon dioxide tension decreased. However, minute volume, respiratory frequency, Tl/Tt and %RC were unchanged with flumazenil. Flumazenil did not change other clinical features and did not produce any abnormal data in laboratory examination. We conclude that flumazenil reverses the respiratory effects of diazepam at least for ten minutes following its injection without producing any complications and side effects.

[A case of food-dependent-exercise induced anaphylaxis possibly induced by shellfish (Sulculus Supertexta and Turbo Cornutus)].

A 17 years old girl experienced an anaphylactic reaction of urticaria, dyspnea, syncope and hypotension while riding a bicycle 55 minutes after eating shellfish Lapas shellfish which was a-like Sulculus Supertexta (SS). She recovered within several hours after the emergency treatment. Another attack occurred 3 months later while she was running with a dog 30 minutes after eating shellfish (Turbo Cornutus; TC). RAST scores were 4 for Lapas and 2 for TC. RAST inhibition test by ELISA showed a high crose-reaction between keyhole limpet hemocyanin (KLH) and Lapas, and between KLH and TC, while the cross reaction between Lapas and TC was low. Gel chromatography with sephacryl G-200 revealed that both Lapas and TC had several allergens with different molecules which were detected by ELISA. Exercise challenge produced an immediate fall of FEV1 and a significant increase in plasma histamine levels for 45 minutes.

Evaluation of severity of heart failure using ventilatory gas analysis.

To evaluate the severity of heart failure, cardiopulmonary exercise testing was performed in 99 normal subjects and in 382 patients with cardiac disease. Anaerobic threshold, peak oxygen uptake, and the ratio of increase in oxygen uptake to work-rate increment were determined by ventilatory and gas exchange measurements as indexes for assessing the severity of heart failure. Anaerobic threshold and peak oxygen uptake declined with age, and males showed higher values than females in both indexes. Anaerobic threshold, as the percentage of predicted value for age, sex, and body weight, decreased as New York Heart Association (NYHA) class increased as follows: 90.2 +/- 15.4% in class I, 76.9 +/- 13.8% in class II, and 59.7 +/- 11.9% in class III (mean +/- SD). Although the ratio of increase in oxygen uptake to work-rate increment was not influenced by age or sex, it decreased as the severity of heart disease progressed. These results suggest that indexes from cardiopulmonary exercise testing are closely related to the pathophysiology of heart failure. We conclude that these indexes can be used as objective and reliable parameters for evaluation of the severity of heart failure in terms of functional capacity.

Two types of cytochrome cd1 in the aerobic photosynthetic bacterium, Erythrobacter sp. OCh 114.

Components I and II of cytochrome cd1 which had different spectral features were purified from the aerobic photosynthetic bacterium, Erythrobacter sp. strain OCh 114. Component I showed an absorption maxima at 700 and 406 nm in the oxidized form, and at 621, 552.5, 548 and 416 nm in the reduced form. Component II showed an absorption maxima at 635 and 410 nm in the oxidized form and at 628, 552.5, 548 and 417 nm in the reduced form. The relative molecular mass, Mr, of both cytochromes was determined to be 135,000 with two identical subunits. Components I and II showed pI values of 7.6 and 6.8, respectively. The redox potential of hemes ranged from +234 mV to +242 mV, except for the heme d1 of component I (Em7 = +134 mV). Components I and II showed both cytochrome c oxidase and nitrite reductase activities. Cytochrome c oxidase activity was strongly inhibited by a low concentration of nitrite and cyanide. Erythrobacter cytochromes c-551 and c-552 were utilized as electron donors for the cytochrome c oxidase reaction. The high affinity of cytochrome c-552 to component II (Km = 1.27 microM) suggested a physiological significance for this cytochrome. Erythrobacter cytochromes cd1 are unique in their presence in cells grown under aerobic conditions as compared to other bacterial cytochromes cd1 which are formed only under denitrifying conditions.

Physicochemical properties of crystalline forms of ethynylestradiol solvates: comparison of thermal behavior with X-ray crystal structure.

Four crystalline forms of ethynylestradiol solvates were obtained from three solvents [acetonitrile (Form A), methanol (Form B), and chloroform saturated with water (Forms C and D)], and were characterized by X-ray powder patterns and thermal analyses. The crystal structures of Forms A, B, and C were further analyzed using the X-ray diffraction method, and the results are discussed in comparison with the thermal behavior of the crystalline forms. The steroid conformation of ethynylestradiol was rigid and no noticeable difference was observed among the solvated crystal structures; the molecular structure differed only in the orientation of the ethynyl group with respect to the steroid skeleton. The difference in the interaction mode between ethynylestradiol and the solvent molecules primarily discriminates the physicochemical properties.

Physicochemical properties of dexamethasone palmitate, a high fatty acid ester of an anti-inflammatory drug: polymorphism and crystal structure.

Two polymorphic crystalline forms of dexamethasone palmitate were obtained from acetone (Form A) and n-heptane (Form B), and characterized by X-ray powder patterns and IR spectra. The crystal structure of Form B was further analyzed by the X-ray diffraction method. The molecular conformation of the hydrocarbon chain was shown to be fully extended and nearly at right angles to the dexamethasone ring. A definite separation between the lipophilic and hydrophilic rows, which consist of palmityl and dexamethasone layers, respectively, was evident in the crystal structure. Utilizing the conformational and molecular packing similarities between dexamethasone palmitate and phospholipid molecules, a possible interaction mode between them is proposed and outlined in this paper.

New mutations fts-36, lts-33, and ftsW clustered in the mra region of the Escherichia coli chromosome induce thermosensitive cell growth and division.

Three new mutants of Escherichia coli showing thermosensitive cell growth and division were isolated, and the mutations were mapped to the mra region at 2 min on the E. coli chromosome map distal to leuA. Two mutations were mapped closely upstream of ftsI (also called pbpB), in a region of 600 bases; the fts-36 mutant showed thermosensitive growth and formed filamentous cells at 42 degrees C, whereas the lts-33 mutant lysed at 42 degrees C without forming filamentous cells. The mutation in the third new thermosensitive, filament-forming mutant, named ftsW, was mapped between murF and murG. By isolation of these three mutants, about 90% of the 17-kilobase region from fts-36-lts-33 to envA could be filled with genes for cell division and growth, and the genes could be aligned.

New mre genes mreC and mreD, responsible for formation of the rod shape of Escherichia coli cells.

New shape-determining genes in the mre cluster at 71 min on the Escherichia coli chromosome map, named mreC and mreD, were identified by complementation experiments using delta mre-678 mutant cells, which have a 5-kilobase-pair deletion encompassing the mre region, and by DNA sequencing. The delta mre-678 mutant cells required three genes, the previously reported mreB gene and the two new genes, to restore the normal rod shape of the cells and normal sensitivity of growth to mecillinam. The mreC gene is preceded by the mreB gene and by a 65-base-pair spacing sequence containing a palindrome sequence and a possible Shine-Dalgarno sequence. The deduced amino acid sequence of the MreC protein consists of 367 amino acid residues with a molecular weight of 39,530. The initiation codon of the mreD gene overlaps the termination codon of the mreC gene by one nucleotide residue. The deduced amino acid sequence of the MreD protein consists of 162 amino acid residues with a molecular weight of 18,755. In vitro, the coding frames of mreC and mreD produced proteins with Mrs of 40,000 and 15,000, respectively, as judged by sodium dodecyl sulfate-polyacrylamide gel electrophoresis.

Trends for coronary heart disease and stroke and their risk factors in Japan.

Disease surveillance and population surveys of risk characteristics in a northeast rural community of Japan (1965 census population, 7,030) are combined in an attempt to relate morbidity and risk factor trends for coronary heart disease and stroke during the last 2 decades. Between 1964 and 1983, the incidence of coronary heart disease (i.e., combined myocardial infarction, angina pectoris, and sudden death) did not change significantly among men and women ages 40-69, and was lower than that for stroke. The incidence of all stroke declined about 60% for both men and women, ages 40-69, with a significant decrease in cerebral hemorrhage for both sexes and in cerebral infarction for men. Between 1963-1966 and 1980-1983, significant upward shifts occurred in the means and distributions of serum total cholesterol and serum total protein in every age and sex group, primarily during the 1st decade. Age-adjusted mean cholesterol levels rose 22 mg/dl to the 1980-1983 mean of 179 mg/dl in men ages 40-69. In women ages 40-69, the mean rose 29 mg/dl to 192 mg/dl. Among nutrients, animal fat intake doubled in men ages 40-59 from 4.5% of daily calories in 1969 to 9.6% in 1980-1983. Animal protein intake also increased, from 5.8% to 7.1%. Most of this increase occurred between 1969 and 1972-1975 and may be attributable to an increased intake of meat, eggs and dairy products. From 1963-1966 to 1980-1983, mean relative weight index rose significantly for all age-sex groups except men ages 50-69. Mean systolic and diastolic blood pressure levels declined for every age-sex group, with a 15-mm Hg age-adjusted decrease in systolic, 4-mm Hg decrease in diastolic pressure among men ages 40-69, and a 11-mm Hg systolic and 4-mm Hg diastolic decrease for women. Two cohorts of men and women ages 40-69 at baseline were followed for disease incidence: an early cohort (2,257 persons) followed from 1963-1966 to 1973 and a later cohort (2,711 persons) followed from 1972-1975 to 1983. In these cohorts, significant risk prediction for cerebral hemorrhage and infarction was obtained with blood pressure level and end organ effects in the electrocardiogram and fundus photographs. Serum cholesterol was inversely associated with cerebral hemorrhage in the early cohort but not in the later.(ABSTRACT TRUNCATED AT 400 WORDS)

Proton nuclear magnetic resonance study on the aromatic amino acid-guanine nucleotide system: effect of base methylation on the stacking interaction with tyrosine and phenylalanine.

The stacking interactions of tyrosine methylester (TyrOMe)-guanosine-5'-monophosphate (GMP), TyrOMe-7-methylguanosine-5'-monophosphate (m7GMP), phenylalanine methylester (PheOMe)-GMP and PheOMe-m7GMP pairs in neutral buffer solution have been studied by proton nuclear magnetic resonance (1H-NMR). The H8 proton signal of GMP showed no noticeable temperature dependence, while the signals of other protons showed usual dependences arising from the ring stacking interaction with aromatic amino acids. The results can be interpreted in terms of the intramolecular C-H ... O hydrogen bonding and ring stacking. Complex formations in 1:1 molar ratio were deduced for all pairs from their Job plots. The association constant for each pair was obtained by analysis of the Scatchard plot. Further, the van't Hoff plot provided thermodynamic parameters of the complex structure. The analyses of these data suggested that albeit the N-quaternization of GMP strengthens the stacking interaction with aromatic amino acid, the bulky methyl group in m7GMP facilitates the dissociation from the amino acid with small environmental change. The possible conformations of GMP and m7GMP in the interaction states are discussed on the basis of the coupling constants.

Scirrhous carcinoma cell invasion into the stomach wall detected by monoclonal antibody S202: a comparison between immunoperoxidase and hematoxylin-eosin stain.

We used anti-scirrhous carcinoma monoclonal antibody (MAb) S202, which reacted strongly with scirrhous gastric cancer, to evaluate the depth of cancer cell invasion into the stomach wall. Sixty-eight scirrhous gastric cancer specimens were stained with the avidin-biotin peroxidase complex method using MAb S202. There were some discrepancies between the histological evaluation of the immunoperoxidase (IP) and hematoxylin-eosin (HE) staining regarding the depth of tumor invasion. In two of the 14 HE staining cases in which carcinoma had extended as far as the proper muscular layer, IP staining revealed that the lesions had actually penetrated this layer. As well, in two of the 18 HE staining cases in which carcinoma cells had not penetrated as far as the serosa, IP staining revealed that they had, in fact, reached the serosal surface. The invasive carcinoma cells were more easily discerned using IP staining with MAb S202 rather than using histological staining with HE. Thus, MAb S202 may be useful for precise identification of carcinoma cells.
