[Cardiocirculatory arrest due to hyperkalaemia following i.v. Imbretil for muscular relaxation (author's transl)].

The muscle relaxant Imbretil was investigated in 10 patients for initiating anaesthesia and for a prolonged artificial ventilation. In 50% of the patients a rather quick and drastic rise of the serum potassium-level was found. A cardiac arrest occurred in one patient due to a hyperkalaemia but fortunately was reversible. Consequently we do not use Imbretil in our Hospital anymore. Today better and less dangerous drugs are available instead--Imbretil must be placed as an obsolete drug.

[Plasma Mg-levels during treatment of hypopotassaemia with Potassium-magnesium aspartate (author's transl)].

The importance of magnesium in the human organism is firmly established. About 250 Mg-dependent enzymes are so far known (2). Under intensive care conditions hypomagnesaemia is a fairly frequent occurrence, but is liable, as experience has shown, to be over-corrected. Serum Mg-levels in excess of 4.0 mg/percent already affect nerve conduction and induce bradycardia (4, 6, 7,). The inhibited liberation of acetylcholine (3) induces vasodilatation with hypotension, ileus and adynamia (3, 6, 7). Magnesium-containing solutions are, therefore, also contra-indicated in myasthenia gravis. Magnesium levels of 6--12 mg/percent are liable to lead to somnolence and even comma (4, 6). Platelet function and, consequently blood coagulation, are also affected by the Mg-levels in the blood (5, 8, 9). Should potassium substitution of more than 100 mEq/litre become necessary it must be borne in mind that administration of potassium-magnesium aspartate may raise the Mg concentrations to undesirable levels, espeically if renal function is impaired.

[Long-term measurement of arterial blood pressure via the superficial temporal artery (author's transl)].

The method used for prolonged continuous measurement of arterial blood pressure in surgical intensive care unit is describe. It was employed in 150 persons aged 1--65 year for periods of up to 30 days. It involved arteriotomy of the super-ficial temporal artery followed by the insertion of a catheter. The absence of side-effects in any of the 150 patients makes this technique very suitable for prolonged continuous blood pressure measurements.

[Pancreatic juice and stress ulcers of the rat (author's transl)].

In 55 rats the role of pancreatic juice for the development of stress ulcers was stuided. After ligation of the common bile duct the stress ulcer incidence was significantly decreased. An additional pancreaticogastrostomy was followed by an even lower ulcer frequency. After pancreaticojejunostomy a higher ulcer incidence was found as after bile duct ligation alone. The role of a duodenogastric reflux for development of stress ulcers is discussed. The ulcerogenic property of bile acids and of the combination of bile and pancreatic juice, caused by lysolecithin, is demonstrated. Pancreatic juice doesn't work as an ulcerogenic substance, but possibly has a protective function by uffering the hydrochloric acid.

[Classification of mitral valve disease by clinical features and hemodynamic parameters (author's transl)].

Using the central venous oxygen saturation (SO2v) at rest and during exercise (50 Watt) a new classification of mitral valve disease is proposed, which corresponds with the recommendation of the NHA (1953). Contrary to the mostly subjective classification by clinical features, the new one results from objective hemodynamic parameters. The validity of the new proposed classification is shown using different clinical parameters.

[Surgical treatment of coronary heart disease (author's transl)].

Until 1974 146 patients underwent coronary surgery in Hamburg. The total mortality rate for the first 100 postoperative days was 16,3 per cent. In the last years the mortality was reduced to 14 per cent. This group included all different selective and emergency surgical procedures for coronary disease. Due to the different degree of the coronary disease and/or of the impairment of the left ventricular function the mortality ranged between 0 per cent and 35 per cent. Some clinical parameters influencing the mortality rate are discussed.

[Morphology and x-ray picture of the rat lung in experimental lung contusion].

Partial contusion of the lung was performed on 131 albino rats. X-ray and histological findings are comparable with lung contusion in man. Three phases are remarkable. X-ray studies of the thorax do not show impressive changes in spite of histological findings of the lung. This means: X-ray picture of the posttraumatic lung is less eventful than the impressive anatomical histological findings.
