The basle PEEP weaner. A versatile device for respiratory assistance.

A device to modify the physiological impact of the abrupt transition of IPPV and PEEP to spontaneous breathing is described. It consists of a coaxial breathing system with a spring-loaded reservoir bag and underwater pressure limit. Electric alarms are incorporated.

Epidural analgesia for the treatment of multiple ribfractures.

Epidural analgesia can be the choice of therapy for all patients with ribfractures and only minor coexisting injuries who are conscious and able to cooperate. In these cases we believe that EA is equivalent or even better than artificial ventilation with all its problems. Generally these patients are far easier to handle and one needs less nursing staff. The dignity of the patients can be maintained and the convalescent time in the intensive care unit is shorter. The criteria for the treatment and for measuring the progress of the patients with multiple ribfractures are discussed.

New perspectives in the microscopic structure of human dura mater in the dorsolumbar region.

BACKGROUND AND OBJECTIVES: The object of this study was to describe the three-dimensional structure of the dura mater by use of scanning electron microscopy. METHODS: Microscopic dissection of the dura mater from four fresh cadavers (aged 70, 75, 76, and 80 years) 8-12 hours after death were investigated in three different planes (longitudinal, tangential, and transverse). RESULTS: The external surface of the dura mater, facing the epidural space, consisted of a network of randomly oriented fine collagen fibers. The thicker elastic fibers (2 microns in diameter) were observed on the surface of the dura. In the inner part of the dura mater, there were very fine lamellae of collagen fibers, which were bundled into thicker (4-5 microns) layers. The dura mater consisted of 78-82 layers, each layer including 8-12 very fine lamellae. CONCLUSIONS: The fibers of the dura mater do not run in a longitudinal direction and are not arranged in a parallel fashion. Cytoarchitecturally the dura mater is a laminated structure built up from well-defined layers oriented concentrically around the medulla spinalis.

Atraucan: a new needle for spinal anesthesia.

BACKGROUND AND OBJECTIVES: Atraucan 26-gauge spinal needles have a tip designed to make a small linear cut (as opposed to a V-shaped cut) in the dura mater. The cut is shorter than the outside diameter of the needle and is dilated as the needle passes through the dura. The needle is used with a 20-gauge introducer. In vitro, it causes less leakage of cerebrospinal fluid than Quincke 26-gauge or Sprotte 24-gauge needles. This study was designed to test the ease of use and any damage caused to the needle tip during lumbar dural puncture. METHODS: This was a multicenter trial (six centers in five countries) involving 362 patients undergoing spinal anesthesia. A detailed questionnaire was filled in for every patient by the anesthesiologist. All the needles were returned to the factory and examined microscopically for damage. RESULTS: Lumbar dural puncture was successful in all but one patient. Spinal anesthesia was satisfactory for the planned surgery in 97%. Microscopy of the needle tips showed only a minor degree (0.01-0.19 mm) of bending in 14%, and none of the tips had a "hook." Postdural puncture headache (PDPH) occurred in nine patients (2.5%), all but one of whom (a 15-year-old male) were females under 55 years of age. CONCLUSIONS: The Atraucan needle is easy to use and has a high success rate in identifying the subarachnoid space. Lumbar dural puncture causes minimal damage to the tip. The incidence of PDPH is low, but a larger comparative study needs to be performed.
