[Nodular euthyroid goitre: value of ultrasonography versus scintigraphy in the initial assessment].

PURPOSE: The aim of this study was to show that in exploration of nodular goiter with euthyroidy ultrasonography was higher than scintiscanning. MATERIAL AND METHODS: It was to show that when it was necessary to specify the topography and the number of thyroid nodules, the proportion of right results was larger with ultrasonography than with scinticanning. These proportions being respectively indicated Pe and Ps, hypothesis of search was formulated H1: Pe > Ps. Null hypothesis was written H0: Pe < Ps. Twenty three patients (21 women and 2 men with an average age of 41 years) carrying thryroïd nodules with euthyroidy had respectively undergone a scintiscanning (scintiscanning with sweeping after ingestion of iode 131) and an ultrasonography (conventional ultrasonography with a probe of 7.5 MZ) of the thyroid gland between February 26, 1996 and June 19, 1997. Per-operativedatahaving beenused as references from which the results of scintiscanning and ultrasonography were appreciated nominally, discreet: right results or false results. In this way, proportions of right and false results were collected respectively for scintiscanning and for ultrasonography, which made it possible to test null hypothesis (H0: Pe < Ps). The results were distributed on a 2 x 2 cotigence table. The chi 2 test was applied with a risk error of 5%. RESULTS: For all patients the scintiscanning found on the whole a minimum number of 39 nodules contrary with ultrasonography and surgical exploration which respectively found a minimum number of 56 and 59 nodules. Comparison of right and false results had shown a superiority of ultrasonography (17 right and 6 false) on the scintiscanning (10 right and 13 false) with statistically significant difference (P < 0.05). CONCLUSION: Ultrasonography is more precise when it is necessary to specify the number and the topography of the thyroid nodules. In addition ultrasonography is alleviating and less expensive compared to scintiscanning.

[Vaginal hysterectomy for hysterocele (report of 36 cases)].

Vaginal total hysterectomy get to better qualily of lite to patients who have genital prolapse. It gives no abdominal scar. The feeding and rising are early. Among a serie of 36 total vaginal hysterectomies for hysteroceles we performed, we report few complications. One patient presented a plentiful peroperative haemorrhage : three others present a vesical wound. We report one case of releasing suture, one had setting of bladderand one case of vaginal infection. One patient died from a cause not reliable to the procedure. The mean duration of the hospital stay was 8.36 days. After 4 years following up, 2 patients have elytrocele recurrence, and one other presents a bad reducing of cystocele. We hope to pursue this serie strenghen then our results.

[Inflammatory ointment from shea butter and hydro-alcoholic extract of Khaya senegalensis barks (Cailcederat)].

In a former study, it was proved that the alcoholic solution of hydro-alcoholic extract of Khaya senegalensis barks had an anti-inflammatory activity on animals after a local application. In this work, ointments made from the same extract and three different excipients (vaseline, lanoline and shea butter (crude and refined)) have been prepared and tested by the method of the croton oil inhibited ear oedema. Results showed inhibition percentages of the ear oedema of 58.8%, 66.7% and 75.4% when the hydro-alcoholic extract was tested at respective doses of 1%, 2% and 3% in shea butter. The two other excipients, (vaceline and Lanoline) tested at the dose of 3% showed between 52% and 58% of inhibitions. The interest of this study was to demonstrate the possibility to maintain the anti-inflammatory activity of Khaya senegalensis barks by using them in a galenic form, easy to prepare and which is, in addition, more adapted than the extract to possible clinical trials.

[Sensitivity to cotrimoxazole of bacteria isolated at the Central University Hospital of Fann, Dakar].

This study concern a survey of bacterial resistance to cotrimoxazole; 510 strains of Enterobacteria (167), Vibrio cholerae(206) and Staphylococcus aureus(137) were tested by disc diffusion and agar dilution methods. An interview was conducted with 86 health personals to appreciate the influence of prescription. Staphylococcus aureus were the most susceptible bacteria (13% of resistance), and Vibrio cholerae the most resistant (95%). Related to the gender, Enterobacteria present 43 to 72% of resistance. The data of interview show a very frequent use of cotrimoxazole, related to the disponibility and the accessibility of this drug.

[Risk factors for lymphedema of the arm after mastectomy for breast cancer].

Postmastectomy lymphedema of the arm is frequently associated to different factors including axillary node involvement and local and regional treatment of breast cancer. Our aims was to identify risk factors of postmastectomy lymphedema. From a retrospective analysis of 735 breast cancers treated in our institute, we found 61 lymphedema of the arm. We then describe our study population and identify by univariate et multivariate analysis the factors significantly associated to the disease. The majority of the patients were young black African female found to have locally advanced breast cancers (88% of T3 et T4 UICC 1988), inflammatory diseases (46% of PEV 2 and 3 of Gustave ROUSSY Institute classification of inflammatory breast cancers). Ulceration is found in half of the patients, metastasis in 20%. The patients first underwent chemotherapy mainly with cyclophosphamide alone (56%). Only 59 patients (8%) had preoperative radiation. Surgery consisted mainly in modified radical mastectomy and lymph node dissection (95%). Residual disease is left in 50% of the cases. Only 35% had post-operative chemotherapy and 9% postoperative external beam radiation therapy. From that population, during the follow up, 61 patients were found to have postmastectomy lymphedema. The disease was asymptomatic in 60% of the cases and painful in 26%. 30% of all the patients spontaneously partially regressed. From univariate analysis we found 7 factors associated with lymphedema: The big size of the tumor (p = 0.005), clinically involved axillary lymph nodes (p = 0.001), metastatic disease (p = 0.0046), traditional or inadequate surgery out of the Institute (p = 0.001), lack of post-operative chemotherapy (p = 0.002), postoperative external beam radiations (p = 0.005), relapse (p = 0.002). From logistic regression analysis three independent factors were found: clinically involved axillary lymph nodes (p = 0.0267), metastasis (p = 0.0002) and local or regional relapse (p = 0.0405). In our practice we found that advanced disease, treated by traditional healers or surgery nurses who had relapsed after mastectomy and external beam radiations without chemotherapy have higher risks of lymphedema.

Serological analysis of human tuberculosis by an ELISA with mycobacterial antigen 60.

An ELISA method for detecting serum antibodies against A60, an antigen prepared from the cytoplasm of Mycobacterium bovis BCG, has been applied to 385 subjects, namely 197 controls (neonates, healthy adults, and tuberculin negative, nontuberculous patients), and 188 subjects at various stages of tuberculous infection and disease. Most IgM determinations gave negative results. While the neonates and normal adults had titers of IgG anti-A60 antibodies below the cut off value, wide variations in antibody titers were observed among the various types of subjects infected by M. tuberculosis. The results obtained with nontuberculous subjects were: 100% negative IgG in neonates and healthy adult individuals and 6.4% "false positive" cases among 124 non-tuberculous patients. The percentage of serologically positive cases of tuberculosis was: 5.9% in latent active primary forms, 42.8% in patent active primary forms, and 82.8% in active postprimary forms. Tuberculous infections had a positively rate of 14.7%, while inactive postprimary tuberculosis had a positivity rate of 50%. The results obtained with A60 can favourably be compared with other serum ELISA tests for tuberculous antibodies against purified or semipurified mycobacterial antigens. Anti-A60 ELISA IgG antibody test can be useful to monitor the kinetics of humoral immunological response during tuberculous infection, disease and chemotherapy. A positive IgG ELISA test may support the diagnosis of active tuberculous disease.
