Stress gastric bleeding. Prevention of experimental lesions by intravenous hyperalimentation.

Rats, weighing 125-150 gm., were restrained under a tight wire screen for one hour at 20degrees C and another hour at 4degrees C of ambient temperature. Animals receiving no infusions, or those receiving an intravenous solution of normal saline, developed 16 +/- 3 S.D. bleeding areas in their gastric mucosa. In contrast, in animals exposed to the same degree of stress who received intravenous hyperalimentation, the number of gastric stress bleeding areas counted was 4 +/- 2 S.D. These results demonstrate that the delivery of calories and protein in the form if intravenous hyperalimentation can significantly reduce the incidence of gastric stress bleeding in an experimental model (P less than .001).

Acute hematogenous osteomyelitis: an experimental model.

The injection of barium sulphate (micropaque) contaminated with Staphylococcus aureus into the nutrient artery of the canine tibia produced typical inflammatory bone changes. This alteration, similar to those observed in human hematogenous osteomyelitis, consisted of medullary destruction, spontaneous fractures, and intense periosteal new bone formation. This system seems to mimic the characteristics of some clinical bone infections and may provide a model for therapeutic trials.

The response of the local immunoglobulin system to malignant lesions of the stomach. A new diagnostic test.

The response of the local secretory immunoglobulin system to tumor antigens was studied in 71 patients suspected of harboring a gastric malignancy. Gastric aspirates were obtained by fiberoptic endoscopy and measured on single radial immunodiffusion plates (Behring Diagnostics). Patients with malignant neoplasms of the stomach were divided into Type I and Type II lesions, based on the extent and stage of the disease. Secretory IgA titers were elevated in all patients with a Type I malignant tumor confirmed by histologic examination; mean 6.1 mg/100 (SD +/- 3.7 mg/100). This leads us to believe that the host's response to malignant cells of the gastrointestinal system is an altered and elevated secretion of IgA. Evaluation of local immunoglobulin titers may be of value when used in conjunction with other diagnostic modalities in determining the nature of gastrointestinal lesions, particularly when the lesion is not metastatic or massively involving the gastric wall.

Diagnosis of chronic and postoperative osteomyelitis with gallium 67 citrate scans.

The effectiveness of gallium 67 citrate in diagnosing the exacerbations of chronic osteomyelitis or postoperative osteomyelitis secondary to open reduction of fractures was tested in thirteen patients. This isotope demonstrated an area of infection in every case in which such an area existed. The control subjects consisted of patients with noninfected wounds after open reduction. In this group, gallium was not incorporated into the post-operative hematoma. No false-negative results were observed in our series, demonstrating that this element is useful in the early diagnosis of bone infections.

The detection of acute experimental osteomyelitis with 67Ga citrate scannings.

Foci of experimental septic osteomyelitis became radioactive after the intravenous injection of 67Ga citrate. With this method, bone infection can be detected much earlier than with roentgenography. Because of its short half-life, 67Ga may be a suitable isotope for the detection of clinical, primary osteomyelitis in its early stages.

Granulomatous peritonitis due to cellulose fibers from disposable surgical fabrics: laboratory investigation and clinical implications.

Two patients with postoperative granulomatous peritonitis were found to have lesions with a distinctive type of foreign body. Laboratory investigation revealed this foreign body to be cellulose, morphologically identical with fibers derived from disposable surgical gowns and drapes, and cotton. The same type of foreign body granulomas was produced experimentally by introducing lint prepared from these gowns and drapes into the peritoneal cavities of rats. Since disposable gowns and drapes are being used with increasing frequency, cellulose lint derived from these sources should be added to the list of hazardous foreign materials which are potential wound contaminants.

Clinical infections detected by 67Ga scanning.

Gallium-67 was utilized in 34 patients suffering from clinically demonstrable infections. The isotope was able to localize infections in soft tissue, bones, and some areas of the abdomen. Its utilization in the diagnosis of pelvic inflammatory disease is complicated by the fact that the isotope is secreted into the bowel and obscures the area. This isotope is ideal for the diagnosis of acute and/or chronic inflammation of bone secondary to open reduction of fractures, and may also be of use in the diagnosis of primary osteomyelitis.
