[Diagnosis of extracranial carotid stenoses using the Doppler sonography].

Percutaneous measurements by Doppler ultrasound techniques over the supratrochlear, supraorbital, common carotid and internal carotid arteries may detect the presence of a stenosis in the internal carotid artery between the bifurcation and the branching of the ophthalmic artery. Out of 550 patients examined by Doppler ultrasound 111 patients underwent carotid angiography and the angiographic data were compared with the noninvasive results. The diagnostic accuracy was 91% in cases with stenoses greater than 50% and occlusion; in cases with stenoses of greater than 50% it was 86%. Occlusions could always be verified. Doppler ultrasound is very valuable for follow-up examinations after reconstructive procedures of the internal carotid artery. In 70 patients we were able to document objectively cases by pre- and postoperative examinations.

Divorce therapy: an in-depth survey of therapists' views.

In-depth interviews were conducted with 21 highly experienced therapists on the criteria of a constructive divorce, the obstacles to achieving such a divorce, and the strategies and tactics of divorce therapy. The primary criterion of a constructive divorce was the successful completion of the process of psychic separation and the protection of the welfare of minor children. Therapy may focus on the decision to get divorced and/or the negotiation of the terms of a divorce settlement. Three types of therapeutic strategies were identified: reflexive intervention by which the therapist orients himself to the marital problems and attempts to gain the trust and confidence of the partners; contextual interventions by which he tries to promote a climate conducive to decision-making; and substantive interventions intended to produce resolution on terms the therapist has come to believe are inevitable or necessary. The nascent state of divorce therapy as an area of therapeutic specialization is noted. The problem of diagnostic criteria for divorce, the relationship between therapists and lawyers, the nature and consequence of therapist impartiality, and the degree to which therapists should mediate the terms of divorce are considered central issues meriting further study.

[A new design for a totally artificial heart: the ellipsoid heart (author's transl)].

Thromboembolic complications represent the main limiting factor in cardiac replacement by totally artificial hearts in calves at present. Thrombus formation within artificial hearts is caused by the appearance of stagnation areas. The ellipsoid heart eliminates stagnation areas by virtue of its production as a one-piece membrane. The heart is driven pneumatically and functions as a diaphragmatic blood pump. The stroke volume is 178 cm3 and the maximal cardiac output 15.8l/Min. Three acute experiments demonstrated a high degree of haemodynamic efficiency without compression of, or interference with surrounding structures, especially the inferior vena cava or the right atrium.

Total anomalous pulmonary venous return. Review of 125 patients treated surgically.

One hundred twenty-five patients undergoing surgical correction of total anomalous pulmonary venous return were studied. The overall mortality was 37% and was related to age at the time operation was required. Mortality was 57% during the first year of life, 29% in patients between 13 and 24 months, and 15% in those between 2 and 10 years; no deaths occurred in those over 10 years. Mortality was highest in patients with infracardiac lesions (62%), and lowest in those with cardiac defects (30%). The major cause of death was pulmonary edema, and survival was closely related to the degree of increased pulmonary vascular resistance. Surgical treatment should be delayed until at least 6 months of age, but the development of congestive heart failure may necessitate earlier operation.

Carcinoma of the male breast following thymic irradiation.

A white male teenager with carcinoma of the breast had received radiation therapy for an asymptomatic enlarged thymus on the second, fourth and seventh days of life. The dose delivered to the infant breasts, the latent period, and the unusually young age of the patient suggest that the malignancy was related to the course of radiotherapy.

Myelography in patients with medulloblastoma.

Unsuspected metastatic medulloblastoma involving the spinal cord was detected by myelography in 2 of 3 patients with newly diagnosed medulloblastoma. Myelography revealed unsuspected spinal cord metastases in an additional patient with recurrent medulloblastoma in the posterior fossa. The authors suggest that myelography be performed prior to radiotherapy in all patients with either primary medulloblastoma or recurrence in the posterior fossa.

Primary reticulum cell sarcoma of the uterus.

The authors present a case of primary uterine reticulum cell sarcoma who developed disseminated disease 6 years after undergoing a total abdominal hysterectomy. Review of the literature reveals 27 other patients with uterine lymphomas. Abnormal vaginal bleeding was the presenting complaint in 66% (14/21) of patients. Reticulum cell sarcoma was the most frequent histologic type, 68% (19/28). 28% of patients were alive and well 6-24 years after therapy.

[Left ventricular decompression by means of a paracorporeal transatrial ventriculo-aortic bypass pump: hemodynamics and long-term results (author's transl)].

Circulatory assist devices in current clinical use are analyzed. In this study the hemodynamic effects and long-term results with a paracorporeal ventriculo-aortic membrane pump with transatrial access to the left ventricular chamber are reported. The effectiveness of the pump was demonstrated by left ventricular decompression (-63%) and reduction of MVO2 (-41%), TTI (-71%), dp/dt max (-66%) and Q cor (-17%). These highly significant hemodynamic changes were, furthermore, confirmed by evaluation of the force-velocity relation (V max). In chronic experiments the bypass pump was implanted in 6 calves. The longest survival time was 18 days. 5 experiments were terminated on account of rupture of mechanical parts, one death was due to acute renal failure caused by emboli. Different bypes of ventriculo-aortic assist devices are discussed in regard to technical problems, hemodynamic efficiency and long-term results.
