Simultaneous correction of blepharoptosis and exotropia in aberrant regeneration of the oculomotor nerve by strabismus surgery: a new, simplified ptosis correction for selected cases.

Selected patients with blepharoptosis and exotropia with aberrant regeneration of the oculomotor nerve can achieve acceptable cosmesis by simple strabismus surgery. Patients with good lid position on attempted contralateral gaze are candidates for this new approach. With aberrant regeneration of the oculomotor nerve, the affected levator frequently becomes pathologically yoked to the contralateral lateral rectus muscle. In these cases, the ecotropia is corrected by a recess-resect procedure primarily or exclusively on the contralateral fixing eye such that postoperatively. The fixing eye is "abducted" by the patient to the primary position, and the ptotic lid elevates simultaneously.

Mutagenicity and chemical reactivity of epoxidic intermediates of the isoprene metabolism and other structurally related compounds.

The mutagenic activities of the epoxidic intermediates of the isoprene biotransformation were investigated using Salmonella typhimurium and compared with those of other structurally related epoxides. The compound 2-methyl-1,2,3,4-diepoxybutane, chemically analogous to the well known carcinogenic 1,2,3,4-diepoxybutane, was found to be as mutagenic as the latter. Moreover, the mutagenic activities of oxiranes were correlated to their alkylating powers towards nicotinamide and to their half-lives for spontaneous hydrolysis. The relationship between alkylating power and mutagenicity was found to hold for the stable epoxides that react mainly by an SN2 substitution mechanism.

Octylonium bromide, an antagonist of platelet-activating factor.

Octylonium bromide, a spasmolytic agent in clinical use for the treatment of increased tone and/or motility of the gastrointestinal tract, was examined for its ability to antagonize some biological actions of platelet-activating factor. It was found capable of inhibiting, in a concentration- or dose-dependent fashion, the platelet-activating factor-induced platelet aggregation of rabbit platelet-rich plasma, bronchoconstriction in guinea-pigs, hypotension in rats and lethality in mice, its potency being between 1/15 and 1/50 that of WEB 2086. On the other hand, octylonium bromide was not capable of inhibiting the platelet-activating factor-induced contraction of guinea-pig lung parenchymal strips, an effect which is inhibited by WEB 2086 only at high concentrations. It is worth mentioning that octylonium bromide antagonized endotoxin-induced gastrointestinal damage, known to be due to platelet-activating factor release, at doses comparable to those which exert a spasmolytic effect in man. The role of platelet-activating factor in human gastrointestinal disorders has not yet been clearly established and, therefore, the clinical implications of this property of octylonium bromide are at the moment unknown.
