Morphometric analysis of hyperthermia-induced nuclear and nucleolar size modifications in a human melanoma cell line.

Although no prominent morphologic alterations were observed by light microscopy in a human melanoma cell line (M-14) exposed at 42 degrees C for 4 hr, the computerized image analysis demonstrated a significant enlargement of both nuclei and nucleoli. The mean nuclear and nucleolar areas were found to be 32% and 94% increased, respectively, in comparison with normal controls. The gaussian distribution of absolute frequency of the measures demonstrated that this nuclear and nucleolar enlargement was distributed among the whole cell population, taking place in clonogenic cells and in "sterilized" cells, ultimately doomed to die. This finding, not previously described in the context of the heat shock response, may be regarded as morphological evidence of an increase in the protein content of the nuclei of heat-treated cells, as determined by biochemical methods.

Outbreak of Clostridium difficile diarrhoea in an orthopaedic unit: evidence by phage-typing for cross-infection.

In a three-week period five patients had diarrhoea in an orthopaedic unit. The first case was clinically diagnosed as pseudomembranous colitis but the causative agent was not sought. Of the remaining cases, two were Clostridium difficile positive. The outbreak then apparently ceased, but during the following several days two of seven stool samples taken at random from asymptomatic patients yielded C. difficile. Phage-typing of the isolates showed that all apparently belonged to the same strain.

Lumbar chemical sympathectomy in end stage of arterial disease: early and late results.

Hemodynamic variables were used to evaluate the effectiveness of lumbar chemical sympathectomy in 20 patients with ischemic foot lesions. Early clinical improvement was obtained in 68% of the cases. The two-year cumulative limb salvage rate was 52%. Results indicate that lumbar chemical sympathectomy offers the same benefits as surgical sympathectomy and represents a useful alternative in patients with advanced ischemic disease of the lower limb, decreasing the rate and the level of amputations.

[Proposal of a detoxication program for drug dependency in pregnancy].

One of the main problems the physician has to handle in managing pregnancy complicated by the maternal intake of opiates is represented by the programming of adequate detoxication treatment that allows in particular for possible pharmacological repercussions on the foetus. On the basis of many years' experience, a therapeutic proposal is put forward as a general guideline, from which an individual approach can be obtained for the treatment of drug dependency in pregnancy.

Morphometric evaluation of histological sections of the thyroid gland in benign and malignant follicular lesions.

A morphometric analysis was made on histological sections of thyroid tissue in various aspects of follicular cell pathologies (goiter, adenoma, and carcinoma) and in normal follicular cells. Three cases of each aspect and 150 cells of each case were measured. The results obtained demonstrate the importance of the nuclear area in the differential diagnosis of normal thyroid, goiter, adenoma, and carcinoma. The other parameters considered, i.e., perimeter, maximum diameter, and nuclear form factor, proved to be able to exclusively distinguish between benign lesions (normal thyroid, goiter and, adenoma) and malignant ones (carcinoma).

Late contrast enhancement in the CT diagnosis of herniated lumbar disk.

Twenty-five patients clinically suspected of herniated lumbar disk have undergone a CT examination before and 40 min after contrast enhancement. In every case, 13 of which were surgically verified, significant contrast enhancement of herniated disks was seen; there was only one false positive case. The authors conclude that late contrast enhancement really improves the CT diagnosis of herniated lumbar disk, particularly in cases with a doubtful or negative precontrast scan. The mechanism of the late contrast enhancement is discussed.

Metrizamide-enhanced computed tomography of intracranial arachnoid cysts.

Between July 1981 and August 1983 the authors studied 21 patients with intracranial arachnoid cysts using metrizamide-enhanced CT. With this method we are able to determine the degree of communication between the cysts and the surrounding subarachnoid space as well as to assess the evolving character of some of these lesions. This information will allow a more rational choice of surgical treatment.

A technique for evaluation of CSF shunt patency using telethermography.

Sixty-two operated hydrocephalic children have been tested in order to evaluate the functioning of CSF shunt using a telethermographic technique. The obtained results indicate that the thermal transfer due to CSF flow within the shunt may be utilized for detecting the patency of the inserted CSF shunt.

Immunologic monitoring during combined radiochemoimmunotherapy in inoperable lung cancer.

49 patients with advanced lung cancer underwent a combined treatment including radiochemoimmunotherapy. Before and at the end of the combined treatment, the immune status of the patients has been evaluated by testing the immunoreactivity to PPD in vivo and the phytohemagglutinin lymphocyte response and the rosette-forming cells in vitro. While the in vitro tests were not significantly modified by immunostimulation, as regards the immunoreactivity to PPD, we observed a positive conversion at the end of the treatment in 11 out of 25 patients who showed a negative reaction before therapy. All the patients who converted from negative to positive showed also a good response to therapy with regression or no progression of the tumor. Survival curves confirmed the prognostic value of PPD reactivity.

Studies on the etiology of the experimental neuropathy from industrial adhesive (glues).

Chickens treated by paintbrushing with glue distillate (used in shoe industries), hydrocarbon mixture and TOCP in hexane, developed paralysis (4/5); in these, we have demonstrated a diffuse degeneration of the myelinic sheath of the peripheral and central neurites. Besides TOCP, cyclohexane (because of its higher concentration in the glue distillate and in the hydrocarbon mixture) may be indicated as responsible for occupational neuropathy.
