Evaluation of bedside myocardial scintigraphy with 201Tl in acute myocardial infarction.

Bedside myocardial scintigraphy was performed on 149 patients admitted to the Coronary Care Unit (CCU), after IV injection of 74 MBq 201Tl, using a mobile gamma camera (Dynamo). The study was displayed on Polaroïd pictures, without any image treatment, and read by two independent readers. Clinical history, findings, and final diagnosis were assessed by an independent clinician. The following conclusions were reached: 1) The sensitivity of the study for the detection of a recent myocardial infarction (MI) was 0.84 with a specificity of 0.87. 2) There was a good correlation between scintigraphic and ECG localization. 3) No firm correlation was found between scintigraphic and enzymatic estimates of infarct size. 4) Abnormal visualization of the right ventricle was probably associated with more extensive infarction.

Tc-99m-pertechnetate scanning in metastatic thyroid carcinoma: a case report.

Radionuclide imaging with Tc-99m-pertechnetate in a patient with a mixed papillary-follicular carcinoma of the thyroid gland showed evidence of increased radioactivity at sites of functioning metastases. The use of Tc-99m-pertechnetate as well as I-131 for imaging in search of functioning thyroid metastases is discussed.

Gastrointestinal blood loss after diflunisal and after aspirin: effect of ethanol.

Fecal blood loss was evaluated in normal subjects with 51Cr-labeled red cells. In a double-blind parallel study in 10 subjects, 250 mg diflunisal twice daily did not significantly increase blood loss in two consecutive treatment periods, while 750 mg acetylsalicylic acid (ASA) 4 times daily did so. In a double-blind crossover study in 2 subjects, diflunisal, 250 mg twice daily again did not significantly affect fecal blood loss during a 4-day treatment period, and there also was no significant effth diflunisal during two additional treatment days. ASA, 600 mg 4 times daily, induced an increase in blood loss and this effect was significantly enhanced by the addition of alcohol. The difference between treatments in the way they interact with alcohol was also statistically significant.

Evaluation of sacro-iliitis: comparison of radiological and radionuclide techniques.

A 99mTc count profile of the sacro-iliac region was performed in 14 control subjects and in 50 patients with inflammatory pain of the sacro-iliac joints. In addition, 85mSr count profiles were recorded in 14 control subjects. The sacro-iliac/sacral uptake ratio was calculated. There was no significant difference between Sr and Tc count profiles. The uptake in patients with sacro-iliitis was not above the range for controls. The mean uptake ratio was significantly below that for controls, and was lower in cases with radiologically advanced changes than in those with few or no signs. This method was not useful for early diagnosis of sacro-iliitis.

Comparison of 133Xe washout curves after bolus inhalation, perfusion, and equilibration.

In 6 healthy subjects and 13 patients with chronic obstructive lung disease, 133Xe washout curves after bolus inhalation, perfusion and equilibration were compared, using a gamma camera and computerized data handling. In healthy subjects the washout after equilibration was significantly slower than after the two other procedures. As this phenomenon occurred at the basal zones, it was attributed to the presence of airway closure in some units. In patients, the three washouts were significantly different from each other. This indicated the existence of intraregional inhomogeneity in ventilation, perfusion and ventilation-perfusion ratio. Our conclusion is that comparison of these different washout methods yields valuable information not only on absolute values of pulmonary function but also on the intraregional distribution, which cannot be obtained by other examinations.

Emphysema with minor airway obstruction and abnormal tests of small airway disease.

In 9 patients complaining primarily of exertional dyspnea, the diagnosis of early emphysema was made on the basis of a decrease of the elastic recoil of the lung and of the single breath diffusing capacity, in the presence of only minor airway obstruction as estimated from the routine pulmonary function tests. Closing volume, the alveolar plateau, dynamic compliance and maximal flow at 50% of the vital capacity were markedly abnormal; the maximal flow-static recoil pressure relationships suggest, however, that none of the patients suffered from small airway disease. We conclude that one should systematically consider early emphysema in the differential diagnosis of small airway disease.

Comparison of technetium uptake in small joints with other indices of inflammation in rheumatoid arthritis.

The uptake of 99mTc in the small joints of the hand has been compared to various other mesures of inflammation in 22 patients with rheumatoid arthritis. The 99mTc uptake in the finger joints can reflect both disease activity at a given time and change in activity over a period of time. The place of Tc uptake amongst other measures of inflammation, from the point of view of the 'relative efficiency' of the assessment method, is rather inferior. In evaluating the effect of antiphlogistic drugs on local inflammation, techneticum uptake may, however, prove more discriminating than grip-strength or the number of painful joints, since the latter measures depend largely on the degree of destruction.

Horizontal gradient in ventilation distribution due to a localized chest wall abnormality.

Horizontal gradients in the distribution of ventilation and of regional vital capacities, as well as a reversed vertical, esophageal pressure gradient, were observed in a patient with a unilateral painful chest wall lesion. The distribution abnormalities disappeared after surgical treatment. These findings suggest that the interdependency between chest wall and lungs, and within the latter, between lobes, is an important factor determining the regional distribution of ventilation and the pleural pressure gradient in man.
