Immunocytochemical staining of serous effusions with the monoclonal antibody Ber-EP4.

Cytospin preparations were made from 102 serous effusions for immunocytochemical staining using a panel of monoclonal antibodies including a new monoclonal antibody Ber-EP4. On cytological examination, 32 fluids were reported to contain tumour cells consistent with metastatic adenocarcinoma; 66 contained benign cells only and three were reported to contain cells suspicious of malignancy. One effusion contained tumour cells consistent with malignant mesothelioma. Positive staining of the tumour cells with Ber-EP4 was observed in the 32 effusions (100%) which contained adenocarcinoma cells. No staining of the mesothelial cells in these 32 specimens was observed. Carcinoembryonic antigen, epithelial membrane antigen Ca2 and CD15 staining of tumour cells was noted in 53%, 50%, 50% and 9% of these cases, respectively. None of the mesothelial cells in the benign effusions stained with Ber-EP4. Nor did the malignant mesothelial cells in the only case of malignant mesothelioma. These findings suggest that Ber-EP4 is a valuable addition to antibodies available for the differential diagnosis of mesothelial cells and adenocarcinoma cells in serous effusions.

Stereochemical pharmacokinetics of the 2-arylpropionic acid non-steroidal antiinflammatory drug flunoxaprofen in rats and in man.

Stereospecific serum assays of the non-steroidal antiinflammatory drug flunoxaprofen (S(+)-2-(4-fluorophenyl)-a-methyl-5-benzoxazoleacetic acid, Priaxim) were performed in rats after the oral administration of 10 mg/kg b.w. of the different enantiomeric forms of the drug or of the racemate in order to establish the occurrence and the rate of biotransformation of R(-)-flunoxaprofen to the S(+)-enantiomer, which is the pharmacologically active form. Preliminary observations of the enantiomeric blood levels were also made in man after a single oral dose (100 mg) of R(-)-flunoxaprofen or of the racemate. Blood was withdrawn at different time intervals up to 120 h in rats and up to 48 h in man and serum levels of flunoxaprofen enantiomers were determined by a HPLC method. The results obtained in the rat show that S(+)-flunoxaprofen serum levels following the administration of a single oral dose of flunoxaprofen reach about the same values (between 24 and 30 micrograms/ml at 18 h) whichever form was dosed (i.e. 10 mg/kg b.w. of S(+)- or R(+)-, or 5 mg/kg b.w. of S(+)- as the racemate). On the contrary, R(-)-flunoxaprofen serum concentrations fall to values lower than 5 micrograms/ml either after the administration of 10 mg/kg R(-)- or of 5 mg/kg R(-)- as the racemate; these serum R(-)-flunoxaprofen values are close to those observed after the administration of S(+)-flunoxaprofen which contains 5% R(-)- as an impurity (i.e. 0.5 mg/kg b.w.).(ABSTRACT TRUNCATED AT 250 WORDS)

Pharmacokinetic study in man with the non-steroidal antiinflammatory drug flunoxaprofen. Serum concentration-time profile after oral or topical preparations.

A clinical study on normal volunteers was performed in order to establish the pharmacokinetic pattern of the non-steroidal antiinflammatory drug S-(+)-2-(4-fluorophenyl-a-methyl-5-benzoxazole-acetic acid (flunoxaprofen, Priaxim, FLU) after oral administration or after transcutaneous absorption. Six subjects (3 men and 3 women) received FLU orally (a single dose of 200 mg) and 6 subjects (4 men and 2 women) were treated by cutaneous application on the shoulder of 1.5 g of a gel preparation containing FLU 5%. Blood samples were withdrawn before and at several time intervals after the treatments up to 48 h after oral administration and up to 24 h after cutaneous application. Serum FLU concentrations were determined by a HPLC method. The results show that a single oral dose of FLU 200 mg follows a kinetic pattern very similar to that obtained with a 100 mg dose as referred to in the literature, without significant differences of t1/2 beta, Vd and clearance values in comparison with those after FLU 100 mg. The dose-related parameters (Cmax and AUC) were higher after FLU 200 mg than after FLU 100 mg. The cutaneous application of a flunoxaprofen gel preparation did not cause detectable systemic absorption of the drug. In conclusion FLU shows favourable pharmacokinetic parameters after 200 mg oral dose, since serum values of the drug exclude the risk of accumulation; moreover the topical application of a FLU gel preparation is a safe transcutaneous treatment for inflammation and pain due to lacking systemic absorption of the drug.

Double-blind crossover trial with oral sodium cromoglycate in children with atopic dermatitis due to food allergy.

Thirty-one children with atopic dermatitis, aged 6 months to 10 years, were selected for this trial. All had historical, clinical, and laboratory evidences that allergy to food was the cause of exacerbations of eczema. Either oral sodium cromoglycate (SCG) or a matching placebo was administered orally for 8 weeks, followed by the alternative treatment for a further 8-week period. During the first 4 weeks of each treatment period, patients remained on an exclusion diet. During the second 4 weeks, the offending food(s) was reintroduced into the diet. The severity of the eczema and the changes in severity as a result of diet or challenge were measured both by the clinician (using body diagrams) and by parents (using a daily diary card). Analysis of the clinician's scoring and the patient's diary card scores demonstrated a statistically significant difference in favour of SCG, especially in the group where the placebo preceded the active treatment. Sodium cromoglycate does seem to reduce the exacerbations of atopic dermatitis caused by food allergens.

[Effects of calcium antagonists on pancreatic endocrine secretion].

The influence of Verapamil, a calcium antagonist, on circulating levels of glucose, insulin and glucagon has been evaluated in 5 normal subjects and in 5 patients with non insulin-dependent diabetes (NIDDM). An oral glucose tolerance test was performed both in basal conditions and during intravenous infusion of the drug (5 mg/h). Administration of Verapamil didn't induce any significant change on the three parameters. The small decrease of glycemia in patients affected by NIDDM and treated with Verapamil was not related to reduction of glucagonemia.

The effects of hexavalent and trivalent chromium on fertilization and development in sea urchins.

The action of Cr6+ (as chromate) and Cr3+ (as sulfate and nitrate) on fertilization and development has been investigated in sea urchins. By rearing the embryos in the presence of chromate (5 X 10(-5) to 5 X 10(-4) M), the differentiation of the gut and skeleton was severely affected. The treatment of sperm before fertilization with CrO2- 4 (10 to 30 min, 10(-4) to 10(-2) M) resulted in a number of abnormal larvae, depending on the length of exposure and the CrO2- 4 concentration. Morphological and quantitative changes of mitotic activity during cleavage were observed as a result of CrO2- 4 exposure of zygotes, as well as of sperm and egg pretreatment. The exposure of embryos to Cr3+ after fertilization permitted larval differentiation, but affected the motility and hatchability of the embryos. The pretreatment of gametes with Cr3+ never induced larval malformations. Cr3+ exerted a strong depressive action on fertilization by decreasing the fertilizing capacity of sperm and leaving the fertilizability of the eggs almost unaffected.

Effectiveness of oral sodium cromoglycate (SCG) in preventing food allergy in children.

The efficacy of oral SCG in preventing food allergy symptoms in children was investigated. Ten children with cow's milk and/or egg IgE-mediated allergy were selected for this study. The subjects were challenged with the offending food before and after a seven-day pre-treatment period with oral SCG (30 mg/kg b.w. per day). Full protection was achieved in six out of eight children with cow's milk allergy and in four of the five children with egg allergy. The mode of action of SCG in the prevention of clinical manifestations of food allergy is discussed.

Immotile cilia syndrome: radial spokes deficiency in a patient with Kartagener's triad.

Mucociliary transport and ultrastructure of nasal cilia in a 13 year old boy with Kartagener's triad, were investigated. Mucociliary transport was significantly delayed (greater than 30 minutes). Electron microscopy showed cilia lacking radial spokes, eccentric central tubules, and a dislocation of one the outer doublets. Dynein arms were present. We consider the radial spoke defect as a distinct congenital anomaly which contributes to the pathogenesis of the "immotile cilia syndrome".

Artificial beta-cell application in two cases of insulinoma: a different pattern in beta-cell adenoma and carcinoma.

With the aid of an artificial beta-cell (Biostator, Miles Laboratories Inc.), a different metabolic and biological pattern of behaviour was observed in benign versus malignant insulinoma. In the patient with beta-cell adenoma but not in the one with carcinoma, plasma insulin concentrations decreased promptly and markedly, and blood glucose increased during diazoxide and somatostatin infusion. Moreover, only in the adenoma patient was glucose need characterized by a circadian rhythm with the maximum values during daytime. This behavior could reflect the degree of tumor beta-cell differentiation. The controlled glucose and insulin infusion was of great help during and after surgical treatment.

Red cell 2,3-DPG levels in diabetic vasculopathy.

RBC 2,3-DPG resulted statistically increased versus normal subjects in 22 insulin-dependent diabetics affected by arterial vasculopathy and in 19 vasculopathic non-diabetic patients. RBC 2,3-DPG, on the other hand, did not differ from the normal mean in 16 insulin-dependent diabetics without vascular involvement. The increase of RBC 2,3-DPG represents principally a consequence of the vasculopathy.

[A rare cause of urologic emergency in childhood: obstruction of the urinary tract caused by Candida].

AA. report a case history about a 3 month old female baby affected by bilateral pyelo-ureteric obstruction by fungus ball, presented as urine ascites because of fissure in renal pelvis and successfully treated by surgery. In spite of disobstruction and subsequent medical treatment, the obstruction relapsed and a second operation was necessary. This time a bilateral nephrostomy was done allowing irrigation with anti-candida drugs. This case has to be considered unusual because of the age of the patient, the clinical presentation as ascites, the bilateral affection, the tendency to relapse after disobstruction, and the successful treatment after the second operation.
