[Possibilities of emergency coronary surgery in acute myocardial ischemia (clinico-experimental study)].

The work is based on the analysis of data on 20 patients with acute myocardial infarction who underwent emergency operations for aorticocoronary shunting and on the findings of 50 dog experiments. Comparison of experimental and clinical data showed that the extrapolation of experimental parameters to a sick person should be undertaken with caution and with a greater probability for groups of patients suffering from myocarial infarction for the first time and with no chronic ischemic heart disease. A time period of six hours from the moment symptoms of myocardial infarction develop is the optimum term for emergency aorticocoronary shunting. A successful operation, however, is also possible at later periods, 15 to 18 hours form the time myocardial infarction develops.

[Aortocoronary shunting or mammary-coronary anastomosis?].

On the basis of the personal experience and literature the authors came to a conclusion that there must be no alternative: the aorto-coronary shunting or mammary-coronary anastomosis since each operation has its positive and negative features as well as indications and contraindications. However, in their opinion, the aorto-coronary shunting should be chosen for patients with the ischemic heart disease since this operation is simpler and more rational, has low postoperative mortality and good remote results.

[Hormone and mediator content in the blood of the adrenals and in the peripheral blood in hypertension].

The concentration of the fractions of corticosteroids, aldosterone, catecholamines, and cyclic nucleotides (cAMP and cGMP) in blood of the adrenal veins and the activity of renin in blood of the renal veins were studied in 22 patients with stages IIA and IIB hypertensive disease. At the same time the content of these substances in the peripheral blood was determined and compared with the level of steroid and catecholamine excretion in the daily urine. An increase in the content of free 11 OCS and F fractions in the peripheral blood and blood of the adrenal veins was revealed in all patients examined.

[Significance of the atrial stimulation test in the diagnosis of ischemic heart disease. A comparison with measured physical loading on the bicycle ergometer and with coronary angiographic data].

The results of the atrial stimulation test were compared with the data of the test on a bicycle ergometer and coronary arteriography. The atrial stimulation test made it possible to diagnose coronary insufficiency in 91.6% of patients with ischemic heart disease. It was established that atrial stimulation helps in determining exactly the causes of a vague cardialgic syndrome in some cases in which the test on a bicycle ergometer cannot be carried out or when it is difficult to appraise its results. As compared to the test with dosed physical load on a bicycle ergometer, the atrial stimulation test proved more sensitive and less specific.

[Reconstructive surgery in the subclavian steal syndrome].

On the basis of reports in the literature and the authors' personal experience, reimplantation of the subclavian artery into the common carotid artery is recommended as a method of extrathoracic correction of the subclavian steal syndrome. The results of intraoperative flow measurement indicate the hemodynamic efficacy of the suggested reconstructive operation. The advantages of reimplanting the subclavian artery into the common carotid artery are pointed out, one of which is the marked decrease in the number of postoperative complications.

[Pharmacological and clinical study of the liquid drug form of nonachlazine].

A 1.5% nonachlazin solution containing substances which promote its absorption from the gastrointestinal tract increases coronary blood flow in cats 2--3 minutes after its administration into the stomach. After single administration this solution arrests a developing attack of angina pectoris in patients, improves the findings of spiroergometry, and in regular (3 times daily) medication prevents attacks of angina pectoris or makes them occurs less frequently in the very first 24 hours. The solution is most effective in patients with a high or moderate coronary reserve and affection of one or, in a lesser degree, of two coronary arteries. The solution does not cause a drop in arterial pressure, headache or dizziness.

[Spiroergometry in assessing the late results of the aortocoronary shunting operation].

The remote results of aorticocoronary shunting were studied in 105 patients (follow-up periods of 1 to 5 years and more). The values of bicycle ergometry and spiroergometry (rehabilitation coefficient, oxygen expenditure per 1 kgm of work, maximum oxygen consumption per 1 kg body mass during muscular exertion, effectiveness of work) were studied before and after operation. The data obtained were compared with the results of spiroergometry in 175 patients with various lesions of the coronary arteries identified by coronarography and in 50 healthy individuals. Spiroergometry is an effective method for diagnosing coronary atherosclerosis before performing coronarography. Patients with good postoperative results showed values which were close to normal; in poor results of surgery and in thrombosis of the coronary shunts initial values characteristic of atherosclertoic involvement of the coronary arteries are encountered. Increase of the effectiveness of work to normal, decrease in oxygen expediture per 1 kgm of work, diminution of maximum oxygen consumption per 1 kg body mass during muscular exertion in the remote postoperative period make it possible to presume, before performing coronarography, that the shunts are functioning. The study conducted allow bicycle ergometry and spiroergometry to be considered demonstrative functional methods for appraising the remote results of operation.

[Bicycle spiroergometric studies in the diagnosis of ischemic heart disease].

The indices of pulmonary ventilation and gas exchange, oxygen consumption per 1 kgm of work, the restoration coefficient, the effectiveness of work, etc. were studied at rest, during dosaged physical exertion on a bicycle ergoraph, and in the restoration period in 50 healthy individuals and in 175 patients with ischemic heart disease. Selective coronary angiography was conducted in all of the patients. Spiro-bicycle-ergometry is an informative method in the diagnosis of ischemic heart disease, enabling the physician to presume the presence of coronary sclerosis and judge the degree and extent of involvement of the coronary arteries before performing coronarography. With the gradual increase in the number of involved arteries, there were noted a gradual decrease of the restoration coefficient, diminution of the effectiveness of work, and an increase in oxygen consumption per 1 kgm of load. Changes in spiro-bicycle-ergometric indices in patients with ischemic heart disease and normal coronary arteries had the same trend as those in patients with coronary atherosclerosis.
