[Lateral sinus thrombosis].

Lateral sinus thrombosis has become a rare complication of the ear in the developed world with the widespread use of antibiotics. The classic clinical picture is often modified by previous antibiotic treatment, which difficult diagnosis and treatment. The authors present the case of a seven year old female who, after an acute left otitis media, treated and later complicated with mastoiditis, presented headaches, neck pain, papilledema and binocular diplopy. Etiologic investigation lead to the diagnosis of left lateral sinus thrombosis. Anticoagulation was started with heparin followed by dicumarinics. This treatment was maintained during eight months. At the moment, the child has no symptoms, with no papilledema or repercussions on visual acuity. The diagnosis of lateral sinus thrombosis and the need for a selection of complementary studies is discussed. A protocol treatment, based on data from a review of the literature, is suggested.

[Secondary stroke prevention].

The guidelines for secondary stroke prevention, graded following available scientific evidence, are presented. Stroke and TIA are defined and the indications for referral established. Basic assessment of stroke patients should include laboratory evaluation, ECG, brain CT, ultrasound examination of the extracranial vessels for events in the carotid distribution, and transthoracic or transesophageal echocardiogram if cardioembolism is suspected. The pharmacological and non-pharmacological reduction of blood pressure and serum cholesterol, stopping smoking and reducing alcohol intake are general measures recommended for secondary stroke prevention, together with healthier life-style changes (eating a Mediterranean type diet and performing regular moderate physical exercise). Concerning antithrombotic therapy, oral anticoagulants are recommended for patients with atrial fibrillation and other high to medium emboligenic cardiac risk conditions. Antiplatelet drugs are recommended for all other survivors of an ischemic cerebral event. Aspirin (75-325 mg/day) is the drug of choice. Alternative antiplatelet agents are clopidrogrel, ticlopidine, dipiridamol or triflusal. They can be used in patients with intolerance or contraindication to aspirin or in high-risk subjects. Endarterectomy of the symptomatic carotid is an additional procedure recommended for patients with ischemic stroke or TIA and carotid stenosis > 80% on the side of the symptomatic cerebral hemisphere.

[Latent AIDS in young children of mothers who were HIV-negative in the first trimester of pregnancy].

The authors describe two cases of infants admitted to our Unit with respiratory failure caused by a Pneumocystis Carinii pneumonia (PCP). Because their mothers were HIV negative in the first three months of pregnancy. HIV infection was not suspected and the diagnosis was only made after the PCP. Based on these examples we discuss the importance of detecting HIV infection during pregnancy.

[Microscopic analysis of "prato", "mussarela" and "mineiro" cheese sold in street markets of the City of São Paulo, southeastern Brazil].

INTRODUCTION: Cheese should be produced from ingredients of good quality and processed under hygienic conditions. Further, cheese should be transported, stored and sold in an appropriate manner in order to avoid, among other things, the incorporation of extraneous materials (filth) of biological origin or otherwise, in contravention of the relevant food legislation. The aim of the study was to evaluate the hygienic conditions of "prato", "mussarela", and "mineiro" cheeses sold at the street food markets in the city of S. Paulo, Brazil. MATERIALS AND METHOD: Forty-seven samples of each of the three types of cheese were collected during the period from March, 1993 to February, 1994. The Latin square was used as a statistical model for sampling and random selection of the street markets from which to collect the cheese samples. The samples were analysed for the presence of extraneous matters outside for which purpose the samples were washed and filtered and inside, for which the methodology of enzymathic digestion of the sample with pancreatine, followed by filtering, was used. RESULTS AND CONCLUSIONS: Of the 141 samples analysed, 75.9% exhibited at least one sort of extraneous matters. For the "prato" and "mussarela" cheeses, the high number of contaminated samples was due mainly to extraneous matters present inside the cheese, whereas in the "mineiro" cheese, besides the internal filth, 100% of the samples had external filth.

[Squamous cell carcinoma in individuals under 40 years of age].

We reviewed all the cases of Squamous Cell Carcinoma in people under 40 years of age, diagnosed histologically at the Dermatopathology Laboratory of the Santa Maria Hospital, from January 1985 to December 1993. The 19 cases detected were analysed for age, sex, occupation, anatomical site of the tumor, evolution, smoke and alcoholic habits, skin phototypes, immunosuppression, previous viral infections, previous radiotherapy or PUVA therapy and associated diseases. A prevalence of an occupation involving more exposure to the sun was found. Males were more affected than females (2.2/1). In 37% of the patients we detected heavy alcoholic habits. The most frequent anatomical site was the lower lip (42%). Patients with skin phototype I were more affected (53%).

[Adult type respiratory distress syndrome (ARDS) induced by hydrocarbons].

The authors present a case of a child aged 32 months, victim of accidental ingestion of an unidentified hydrocarbon product. Sixteen hours following hospitalization, the patient began polypnea with progressive hypoxemia, with the radiograph showing ARDS pattern. Mechanical ventilation was initiated on the 2nd day of admission. During hospitalization the patient developed pneumomediastinum and bilateral pneumothorax which justified active drainage. Inspite of the initial poor prognosis, the patient improved, with no symptoms after discharge. The authors discuss certain theoretical considerations regarding hydrocarbon intoxication.

[Cardiac amyloidosis: review of the literature].

The authors review the clinical impact of cardiac amyloidosis and new aspects concerning the management of this disease in the literature. Cardiac amyloidosis accounts for 5-10% of non-coronary myocardial disorders. It is a distinct form of cardiomyopathy with a severe prognosis. The authors review the role of endomyocardial biopsy and the relative sensitivity of non-invasive diagnostic methods. Chemical and genetic typing of amyloid, which is of considerable clinical value, is also reviewed due to recent evidence of treatment for different types of systemic and cardiac amyloidosis, such as the most common hereditary form caused by transthyretin gene mutation.

Conditioned taste aversions and latent inhibition in Egyptian spiny mice and Long-Evans rats.

The acquisition and extinction of a conditioned taste aversion in Egyptian spiny mice and Long-Evans rats was compared during 20 posttest sessions using a cross-over design and double-blind control procedures. Spiny mice preexposed to a sucrose CS demonstrated more latent inhibition and a faster rate of extinction than did Long-Evans rats preexposed to the same CS. Preference indices did not differ between control animals or as a function of gender. The present results are the first report of the effects of latent inhibition on learning taste aversion in Egyptian spiny mice.

[A case of Legionella pneumophila pneumonia].

Legionella pneumophila is an uncommon cause of pneumonia, mainly in children. We present a case with aggressive disease and unfavourable outcome. The diagnosis was established by serum antibody detection and immunofluorescence and culture of a fragment of lung tissue obtained at necropsy. We focus the importance of this aetiology in nosocomial pneumonia and community-acquired pneumonia with an atypical evolution. The need for specific therapy and some preventive measures are also stressed.

[Visualization of thrombi in the left auricular appendix using transesophageal echocardiography. Apropos of a clinical case].

A case of Transesophageal Echocardiography (T.E.E.) Assessment of "Rheumatic Mitral Stenosis" is presented. T.E.E. has been recently referred as having superior resolution than transthoracic Echocardiography on the detection of thrombi. In the presented case a thrombi was detected in the left atrial appendage. This fact has allowed a better planning of surgery.

Controversies in posttraumatic epilepsy.

In civilian accidents the factors involved in the origin of posttraumatic epilepsy are controversial. In this study of 506 consecutive head trauma patients and 101 epileptic patients developing seizures after head trauma, we have examined the importance of the duration of coma, type of seizure and drug therapy, time to first seizure, age and focal lesions and compared our results with the literature. The importance is stressed of focal lesions and of neurological deficits for the origin of posttraumatic seizures.

[Efficacy of the electrocardiographic diagnosis of left ventricular hypertrophy on the basis of gender].

STUDY OBJECTIVE: To determine the efficacy of electrocardiographic criteria of Left Ventricular Hypertrophy (RVL greater than 11 mm, SVE3 + RVL greater than 20 mm in females and greater than 28 mm in males, SV1 + RV6 or SV2 + RV5) in hypertensive patients according to sex. DESIGN: Retrospective study. Gold standard M mode echocardiography. SETTING: Pulido Valente Hospital Department of Cardiology (Lisbon). PATIENTS: 56 hypertensive patients (age 55.1 s.d. 11.6), 27 females and 29 males, with LVH by echocardiographic Left Ventricular Mass Index (LVMI) greater than 110 g/m2 in woman and greater than 134 g/m2 in men, respectively 10 and 19 patients. The LVMI was calculated by the formula of the American Society of Echocardiography modified by Devereux. RESULTS: The LVMI was smaller in females than in males (P = 0.007). The Cornell Criteria had the greatest efficacy in woman and the Sokolow criteria satisfactory efficacy but in man. The electrocardiogram (1 of the 4 criteria positive) had low diagnostic efficacy in woman. CONCLUSIONS: We concluded that echocardiogram is the best non-invasive diagnostic tool of Left Ventricular Hypertrophy in hypertensive females and is necessary to adjust and choose the electrocardiographic Left Ventricular Hypertrophy criteria according to sex.

[Electrocardiographic symptom-signal correspondence in Holter records].

STUDY OBJECTIVE: To determine the correspondence symptom-sign in 24-hours ambulatory electrocardiography records. DESIGN: Retrospective study. SETTING: Pulido Valente Hospital (Lisbon). Serviço de Cardiologia. Department of Ambulatory Electrocardiography. PATIENTS: 80 patients, who made 24-hours ambulatory electrocardiography and wrote correctly the diary. Two observators analysed the correspondence symptom-sign of 269 symptomatic moments, 10 minutes before and after the symptom by reviewing the compressed print. RESULTS: We found correspondence symptom-sign in 50% of the symptomatic moments. The correspondence change with the symptom type (increased for palpitations and dyspnea), the presence of Cardiopathy (increased for chest pain and dyspnea in patients with cardiopathy) type of cardiopathy (increased in patients with Mitral Valve Prolapse and Valvular disease) and expression used by the patient.
