[Spontaneous biliodigestive fistulas. Clinico-radiological considerations on 17 cases].

An account of the aetiology, pathogenesis, anatomical and pathological picture, progression and complications of spontaneous biliodigestive fistula is flowed by a description of its clinical picture and diagnostic problems. Reference is also made to 7 patients subjected to emergency surgery and 10 electively operated. Diagnosis is ensured by careful review of the history, radiological examination of the abdomen in blank, and assessment of the digestive apparatus with an opaque meal and clyster. Little assistance is provided by the objective data and radiological examination of the bile ducts.

[Production of oxygenated free radicals and the role of exogenous antioxidants in liver transplantation in rats].

It has been suggested that, in transplantation organs, the lesions observed after conservation then reperfusion could be related to the formation of oxygenated free radicals. The aim of our work was first to verify the hypothesis that oxygenated free radical are formed after ischaemia-reperfusion of the liver, then to evaluate the effects of the allopurinol and glutathion, known antagonists of oxygenated free radicals, contained in the University of Wisconsin conservation fluid. The University of Wisconsin solution was compared with a Collins solution which does not contain oxygenated free radical antagonists. An orthotopic liver transplantation model was used in Wistar rats. Three groups were studied. In the control group, 5 rats underwent laparotomy then were closed with no surgery being performed. In the Wisconsin group (n = 6) and the Collins group (n = 6) the livers were washed and conserved in the corresponding solution at 4 degrees C before transplanting. Plasma levels of malonyldialdehyde, measured by high performance liquid chromatography, were used as a marker for the formation of oxygenated free radicals. Impaired liver function was assessed on the basis of mortality and serum transaminases, alkaline phosphatase and total bilirubin levels. Liver biopsy was performed at sacrifice. The level of malonyldialdehyde was significantly higher in the transplanted groups compared with the control group (p < 0.01). There was no difference between the Wisconsin and the Collins groups. Hepatic function was significantly reduced in the transplanted groups compared with the control group (p < 0.05). [Role of arteriography and percutaneous transluminal angioplasty in the diagnosis and treatment of arterial vasculogenic impotence].

High quality arteriographic studies of the iliopudendal vascular tree are mandatory for the correct examination of arteriogenic impotence patients before revascularization procedures. Twenty-three patients with chronic erectile dysfunctions due to stenosis or occlusive arteries diseases of the iliac arteries were treated with percutaneous transluminal angioplasty (PTA) in our Department. A positive clinical result was obtained in 15 of 23 cases (65.2%). The maneuver was successful in 8 of 14 patients with vascular lesions of the common and/or external iliac artery (57%). The erectile dysfunction was resolved in 4 of 6 patients with stenosis of the external iliac artery associated with a stenosis of the hypogastric artery (66.6%). The erectile dysfunction was also resolved in 3 patients with a single vascular lesion in the hypogastric artery. No major post-angioplasty complications were observed. PTA is a repeatable and not very invasive method with a low complication rate and could represent a valuable alternative to surgical revascularization in the patients with associated impotence and claudication of leg and hip. Moreover, it makes the treatment of choice in the patients with erectile dysfunctions due to isolated lesions of the hypogastric arteries.

[Topical laryngo-tracheal anesthesia and intraocular pressure in anesthesia for ophthalmic surgery].

OBJECTIVE: The reflex response to orotracheal intubation provokes an increase of arterial pressure accompanied by an increase of chorioides volume and a consequent ocular hypertone. There are several methods to reduce the reflex response due to intubation. One of the most effective is topical anaesthesia of larynx and trachea. Experiments were directed to evaluate the efficacy of topical anaesthesia to reduce the intraocular hypertone due to orotracheal intubation. DESIGN: A prospective randomized mask study was conduct on patients undergoing ophthalmologic (anterior segment) surgery at the Eye Clinic of Florence University. METHODS: Intraocular pressure was measured by a Goldman tonometer at four times: T0 = basal, T1 = 2' minutes after induction of general anaesthesia, T2 = 2' minutes after laryngoscopy, T3 = 2' minutes after orotracheal intubation. At the same moments, systolic blood pressure, heart rate, rate pressure pro duct were measured. Patients were randomly divided in two groups: Group L (n = 10) in which was evaluated the efficacy of laryngotracheal topical spray of lidocaine 4% (2 ml) and Group F (n = 10) in which saline was used instead of anesthetic. The filling of the LTA kit (Abbott) was made by a person not involved in the experiments. DATA ANALYSIS: Student's t test for unpaired data. RESULTS: Topical anaesthesia reduces the increase of intraocular pressure, hypertension and rate pressure product due to intubation. The intraocular pressure reduces to 13% less than basal value in Group L and increase to 50% more than basal value in Group F. CONCLUSION: The topical anaesthesia of larynx and trachea is effective to reduce the intraocular hypertension due to the reflex response evoked by orotracheal intubation.

Morphometric analysis of the lateral ventricles in living human.

The evaluation of the volume of the encephalic ventricles in the living human has a great importance in diagnosis and monitoring of several pathologies. As well as a good measuring accuracy, a great usefulness has the facility of execution. The present study would be a contribution to the search of systems of analysis that assure both fast execution and greatest precision of the quantitative values. Brain MRI scans from 10 female individuals, aged from 25 to 60, with no apparent neurologic or psychiatric pathology have been studied. The brains were evaluated with 5 mm slices thickness (gap 1 mm) scans performed in axial and coronal planes and obtained by means of long and short spin echo sequences. Images have been acquired on a Personal Computer and submitted to a semiautomatic morphometric analysis. The values obtained are in accordance with the range of normal values reported from the literature, showing also a lower standard deviation; besides this clear accuracy of measurement, it's important to emphasize the facility and swiftness in the execution of the imagine evaluation.
