[Anti-arrhythmic effect of magnesium cations. Protection against ventricular excitability during cardiotonic therapy].

This study showed the effectiveness of magnesium sulphate during digitalis therapy complicated by ventricular hyperexcitability. 28 patients with cardiac disease and ventricular arrhythmias in heart failure were studied. Magnesium sulphate was given by slow intravenous infusion (30--50 mg/min) twice daily. The anti-arrhytmic action allowed treatment with digitalis until improvement in cardiac function was observed. The mechanism of the action of magnesium sulphate in digitalis--induced arrhythmias is discussed.

Studies on the mechanism of reflex vasodilatation. The cholinergic component in the baroreceptorial reflex in the dog.

This study was designed to investigate whether the cholinergic system is involved in the genesis of the reflex vasodilatation which follows the systemic hypertension induced by fast intravenous injection of norepinephrine in the dog. Accordingly, in 7 dogs the gracilis muscle was isolated and perfused and the reflex evoked. The analysis of the integrated areas of vasodilatation after atropine pretreatment showed a significant decrease of the reflex response in the perfused circulation. In fact, the mean value of the integrated areas of vasodilatation which was 66 +/- 8 mm Hg/min in the control condition, was reduced to 45 +/- 4 mm Hg/min after administration of atropine in the gracilis artery; meanwhile the integrated areas of systemic hypertension did not show any change. Phentolamine intra-arterial administration completely abolished the reflex. These results suggest the existence of a cholinergic component in the reflex vasodilatation induced by transitory baroreceptorial stimulation.

[Permanent atrial paralysis].

A case is described of permanent atrial paralysis in a 68 year old female with diabetes and cirrhosis of the liver. Absence of electrical atrial activity was expected on the standard electrocardiogram, and confirmed by intracavitary electrophysiological investigation which showed the ineffectiveness of stimulation of both the right atrium and the coronary sinus. Recording of the His potentials showed that the resultant rhythm was of supra-ventricular origin. The absence of mechanical atrial activity was confirmed radiographically, on the jugular venogram, on the apexogram, and on the tracings of right atrial and pulmonary capillary pressure. A search was made for muscular or neuromuscular dystrophy which has often been found in association with this arrhythmia, but none was found. There may be an etio-pathological relationship between carbohydrate metabolism and atrial paralysis, as certain authors have suggested.

[Stimulation with automatic radiofrequency scanning in the long term treatment of tachyarrythmias].

A pacemaker is described which uses radiofrequency signals for synchronisation and pacing, and seeks automatically the phase of the cycle at which the tachycardia may be interrupted. The patient himself can activate the pacemaker. The programme of the external transmitter, which scans nearly all the R-R cycles of the tachycardia, may be pre-set to transmit one or two synchronised stimuli, with a progressive delay of 5 or 10 ms. If the tachycardia is interrupted the pacing stops automatically. The implantable unit of the device may be connected to a normal endocardial electrode, has no batteries and is of small size and weight. The method has been used with success in 4 patients with reciprocal junctional tachycardia (3 of these patients had the Wolff-Parkinson-White syndrome), and in one case of chronic recurrent ventricular tachycardia. All of them were refractory to conventional therapy.

[Efficacy and tolerability of a cardioselective beta-blocking drug (atenolol) in the treatment of essential hypertension. A double blind study (author's transl)].

The antihypertensive effect of atenolol, a new beta 1 receptor blocking agent, was studied in a double blind non cross-over trial in 40 patients (pts) affected by mild to moderately severe essential hypertension with normal plasma renin activity. After a run-in period (15 days) of placebo treatment pts were assigned to two groups. The first (group A) continued placebo treatment for 30 days, the second (Group B) were given atenolol (ICI 66082) 100 mg daily for 30 days also. Atenolol significantly reduced systolic and diastolic blood pressure in recumbent and standing position and heart rate at rest. No significantly changes of the same parameters were observed in group A. Body weight and plasma renin activity was unchanged in both groups. Atenolol treatment never was discharged in order to side effects. These results seem to suggest that atenolol can be an useful drug in the treatment of systemic blood hypertension.

[Effects of nifedipine in the treatment of effort stable angina: a double blind study (author's transl)].

The effectiveness of chronic treatment with nifedipine in patients (pts) with a history of stable angina on effort has been evaluated in a double-blind study in 15 subjects of both sexes, aged 45 to 65 years. All pts underwent diagnostic cardiac catheterization and selective coronary angiography, which showed stenosis of one or two main coronary vessels between 50 and 70%. The pts, after 2 weeks of observation, underwent erect bicycle exercise test with increasing workloads. After 4 weeks of treatment with nifedipine or placebo the stress test was repeated. Nifedipine treatment reduced the weekly number of anginal attacks as well as the weekly number of nitroglycerin tablets consumed by the pts, increased maximum workload tolerated and reduced the S-T segment depression for the same workload, meanwhile heart rate and blood pressure did not change significantly either at rest or during exercise. The result of this study indicate that nifedipine is useful in the treatment of pts with stable angina on effort.

[Specificity of systolic anterior motion of the mitral valve in the echocardiographic diagnosis of hypertrophic cardiomyopathy (author's transl)].

In order to determine the prevalence of sistolic anterior motion (SAM) of the mitral valve, we evaluated the echocardiographic records of 229 consecutive patients with a variety of cardiovascular conditions. SAM was present in all the 9 patients of this series with hypetrophic cardiomyopathy and in additional 4 patients with other conditions. Their diagnosis were: arterio-venous fistula of the forearm, tetralogy of Fallot, Prinzmetal angina and idiopatic hypercynetic cardio-circulatory state. Thus SAM without hypertrophic cardiomyopathy is not a rare event in patients undergoing echocardiography. The wide variability of conditions associated with SAM suggests that it represents a functional and aspecific abnormality and does not involve peculiar anatomic factors.

[Stimulation by automatic scanning in the long-term treatment of hyperkinetic arrhythmias (author's transl)].

The use of programmed stimulation in the long-term treatment of re-entry tachycardia offers encouraging perspectives. Among the others proposed the "scanning" system seems to be the most effective. However, an implantable stimulator with such pictures is not yet available and a temporary electrode is thus required. These difficulties have been overcome utilizing the radiofrequency as a sincronizing and stimuli-producing means. An implantable appliance was therefore designed which is triggered by the patient himself and automatically researches the zone of interruption of tachycardia by exploring the R-R cycle. The program of external transmitter which can produce one or two sincronized impulses, seams the cycle with progressive steps of 10 msec each; when tachycardia is interrupted, further stimulation is inhibited. The implanted part of the appliance connected with a normal endocavitary electrode, does not have any power supply and thus is very small. The efficacy of this method has been shown in 4 junctional reciprocating and 1 ventricular recurrent chronic tachycardias, resistent to conventional pharmacologic therapy.

[Usefulness of x-ray parameters in the functional evaluation of patients with nonvalvular cardiopathies].

In 49 patients suffering of heart diseases we have studied the changes of radiologic cardiac measurements and systolic time intervals (STI) in the four functional classes of the New York Heart Association (NYHA) classification, investigating also the existence of any relationship between these different parameters. Only the patients in functional classes 3rd and 4th showed significant changes in STI and radiologic measurements as compared to the control group. Moreover, a significant negative correlation has been observed between relative heart volume and left ventricular ejection time (LVET) (r = 0.69, P less than 0.001) and LVETc (r = 0.82, P less than 0.001) and a positive correlation between relative heart volume and pre-ejection period (PEP) (r = 0.59, P less than 0.01) and PEP/LVET ratio (r = 0.75, P less than 0.001). These results seem to demonstrate that relative heart volumetry is a fairly accurate index of the cardiac conditions in non valvular heart diseases.
