Bladder cancer and asbestos in Spain.

The relationship between bladder cancer and occupational exposure to asbestos was investigated between 1978 and 1982 at La Paz Hospital in Madrid in a case-control study conducted among 353 male patients and 53 women with a diagnosis of urothelium carcinoma, and among an equal number of controls matched in age (decade of life), sex and date of admission to the hospital. The results showed that bladder cancer is associated with occupational exposure to asbestos.

Bladder cancer and the consumption of alcoholic beverages in Spain.

The relationship between bladder cancer and alcohol consumption was investigated in a case-control study conducted in 506 patients (453 males and 53 females) with diagnoses of papillary urothelial carcinoma during 1978-1982 at "La Paz" hospital (Madrid), and an equal number of controls matched for age, sex and date of admission to the hospital. The results showed that the risk of bladder cancer does not increase with the intake of beer, wine and spirit beverages. However, a high risk of bladder cancer was associated with consumption of wine mixed with "gaseosa" (a refreshment beverage consisting of carbonated water plus artificial sweeteners).

[Tobacco and pregnancy. Weight of the newborn infant and weight gain of the mother].

865 patients delivered at women's clinic "La Paz" have been examined, to prove the hypothesis, that the weight of the newborn may be influenced by smoking indirectly via nutrition. 276 of the 865 women did smoke. In this group the weight of the newborns was lower significantly. If we select both groups of smoking and nonsmoking patients appropriate to the weight increase during pregnancy above or below the average value, we can state that those fetuses had a intrauterine growth retardation whose mothers did smoke and had a lower weight increase than the average value of all pregnant women.

Risk factors of bladder cancer in Spain.

A case-control study was performed on 406 (353 males and 53 females) bladder cancer patients and 406 controls matched in age, sex and date of admission to the hospital. The questionnaire included questions about socio-economic status, residence, personal and familial histories of diseases, professional occupations, and habits such as cigarette smoking and consumption of coffee, artificial sweeteners, alcoholic and nonalcoholic beverages, and water. Up to three different exposures to occupational risks were taken into account. Data were studied by a discriminant analysis technique. The results suggest a multifactorial etiology for this disease. The factors increasing the risk of bladder cancer, listed in order of importance, were: total number of cigarettes smoked, history of urologic disease, second exposure to an occupational risk, use of artificially sweetened beverages, low consumption of water, family history of cancer, third occupational exposure to a risk, use of artificial sweeteners, and years of coffee drinking.

Side-effects of sodium valproate, mainly related to its hepatic and pancreatic toxicity.

A study was made of 134 patients (67 males and 67 females) treated with sodium valproate, with ages from 1.5 to 70 years (50 on monotherapy and 84 on multitherapy), to detect side-effects of this treatment. To meet this goal, a clinical questionnaire was used with special emphasis on biological parameters to detect hepatic and pancreatic toxicity. 71.6% developed side-effects, without differences either between groups of sex or age, or patients on monotherapy and multitherapy, or the duration of the treatment, longer or shorter than six months. The side-effects were mild and transient, and without relationship with doses or plasma levels of the drug. The most noticeable side-effects in the study were the increase in amylase values, mainly in urine (23.9%), eosinophilia (30% in the monotherapy group), increase in gamma-glutamyltranspeptidase (20.2% in the polytherapy group) and weight gain in 25% of adult women on polytherapy. Only a 4.7% developed mild and transient elevation of transaminases, that did not differ from the control population. The relevance of using a clinical questionnaire and biological parameters to evaluate the side-effects of a drug is emphasized.

Histological, ultrastructural and mitochondrial oxidative phosphorylation studies in liver of rats chronically treated with oral valproic acid.

The effects of oral administration of valproic acid (VPA) have been studied on the liver of female adult Wistar rats, at doses of 60 and 120 mg/kg/day during 4 and 18 weeks (periods I and II). Other groups received the same doses of VPA with phenobarbital (PB). The animals developed periportal steatosis, more intense at the end of period II and in groups non-induced by PB, and also some degree of mitochondrial swelling. An uncoupling of the oxidative phosphorylation was also evident with both doses in the non-induced groups, and only with the high dose of VPA in the induced group. An in vitro study on isolated liver mitochondria showed uncoupling only with very high concentration of VPA (10 mM). We comment on these findings and propose a chronic model of experimental hepatotoxicity of oral VPA in the rat.

[Post-traumatic status epilepticus with only mental symptomatology].

The authors report the electroclinical data in a left-handed patient presenting with a partial paroxystic state immediately following cranial trauma complicated by a right temporal subdural hematoma. The clinical picture during the crisis was limited to symptoms evoking a subjective post-traumatic syndrome. Anti-convulsant treatment produced complete disappearance of clinical disorders and critical focal activity. The problems raised by this observation are discussed.

[Factors associated to non-attendance to psychiatric first visit].

INTRODUCTION: The failure to the first regular appointments in Mental Health Centres represent a problem in economic terms for the institution, a waste of time for the professional person and an increase in the waiting list in the centre with the subsequent prejudice for the patient. METHODOLOGY: In this investigation we study the influence of some sociodemographic (sex and age) and referral variables (geographic area, previous contact in the centre, referral reason and diagnostic by the family physician) in a group of 235 failing subjects compared with a group of 300 attending subjects. RESULTS: Results show that there's no relationship between the variables in terms of sex or age; that there are differences related to the geographic area from where they are referred and that the patients with a previous contact in the centre (more than one year) fail less than those who never attended.

[Takayasu arteritis: usefulness of radiologic and angiographic studies].

With the purpose of learning the usefulness of radiologic and angiographic procedures for the evaluation of Takayasu's arteritis (TA), we studied 64 patients (ratio women: 8.1, average age: 23.5 years, range: 13-52 years) in which we performed arteriographic studies in the clinically affected area. All cases had chest films: 53 had thoracic aortogram, 60 abdominal aortogram, 16 pulmonary arteriography. According to the topography of the lesions we found 8% of the cases with damage exclusive to the supra-aortic trunk, 6% with isolated alteration of the intermediate thoraco-abdominal aorta, 62% with mixed pathology of the two categories above, and 21% with lesions in the pulmonary artery besides systemic arteriopathy. The results were as follows: 1) radiology of the chest: cardiomegaly (48%), irregularities in the ascending aorta (31%), calcification in the aortic wall (29%), calcified granulomas (25%) and signs of pulmonary venous hypertension (21%); 2) thoracic aortogram: irregularities in the descending aorta (56.6%), thickening of the wall of descending aorta (39.6%), dilatation of the ascending aorta (26.4%), of the descending aorta (26.4%); occlusions: of the left subclavian (24 cases), left mammary (16 cases), left carotid (8 cases) and left vertebral (8 cases); 3) abdominal aortogram: irregularities of the outline, stenosis, prominent "supplementary" arteries and aneurysms in 53%, 43.3%, 38% and 13.3 of the studies performed. The arteries most commonly affected were: renal (74.7%), both (31.6%), right (28.2%) and left (14.9%), superior mesenteric (26.6%) and hepatic (21.6%); 4) pulmonary arteriography: arterial occlusions: right superior lobar branch (37.5%), right medial (6.2%), right inferior (12.5%), without predilection by any lobe; 5) coronary arteriography: one case with occlusion of anterior descending artery and circumflex coronary artery (the other 8 cases without significant lesions). We concluded that TA affects independently the arteries of different areas, hence it is necessary to perform multiple angiographic studies for adequate evaluation of the extension of vascular damage.
