Six-drug chemotherapy (hexamethylmelamine, doxorubicin, cisplatin, cyclophosphamide, methotrexate, and 5-FU; CHAMP-5) for ovarian carcinoma: alternating sequences of combination regimens.

Twenty-five patients with previously untreated adenocarcinoma of the ovary (International Federation of Gynecology and Obstetrics, stages II, III, and IV) were placed on a six-drug trial (CHAMP-5) for 18 months. This trial consisted of 28-day cycles of hexamethylmelamine, doxorubicin, and cisplatin alternating with hexamethylmelamine, cyclophosphamide, methotrexate, and 5-FU. After a minimum follow-up of 31 months, 14 (56%) of the 25 evaluable patients achieved complete or partial response and nine (36%) achieved complete pathologic response. Although CHAMP-5 is effective in the treatment of ovarian cancer, the results in these patients are not substantially different from those achieved with hexamethylmelamine, doxorubicin, and cisplatin alone.

Treatment of advanced endometrial carcinoma with doxorubicin and cisplatin: effects on both untreated and previously treated patients.

Sixteen patients with advanced (International Federation of Gynecology and Obstetrics stage III and IV) adenocarcinoma of the endometrium were treated with twelve 28-day cycles of doxorubicin and cisplatin. Response was achieved in 92% of patients (11 responses among 12 patients) who had received no prior chemotherapy and in 50% (two responses among four patients) of previously treated patients. Median survival was 10 months. Doxorubicin and cisplatin were readily administered on an outpatient basis with comparatively low major toxic effects, primarily hematologic, renal, and gastrointestinal. These results indicate that doxorubicin and cisplatin combination therapy is effective with acceptable toxicity in patients with advanced endometrial carcinoma.

Family structure and dating behavior of adolescents.

This study investigated the effect of family structure on previous dating behavior of older adolescents, and examined the relationship of family structure, family relationship variables, and personal attributes to previous dating behaviors. Data were obtained from 96 males and 81 females enrolled in human development courses at two midwestern universities; 122 were from intact, nuclear families and 55 were from nonintact families. The Dating and Courtship Behavior Questionnaire, adapted from questionnaires by Winch (1949a) and Landis (1963), was administered to subjects during regular class periods. Family structure did not have an effect on number of partners or steadies, but adolescents from nonintact households began dating at younger ages than those from intact households. Personal attributes were more often related to dating behaviors than family structure or family relationship variables. Little support was found for the proposition that dating behaviors were affected by family structure.

Fibrochondrogenesis: lethal, autosomal recessive chondrodysplasia with distinctive cartilage histopathology.

Fibrochondrogenesis is a rare, neonatally lethal rhizomelic chondrodysplasia distinguished from other forms of lethal dwarfism by broad long-bone metaphyses, pear-shaped vertebral bodies, and by microscopic changes of cartilage with unique interwoven fibrous septa and fibroblastic dysplasia of chondrocytes. We report the second and third well-documented cases of this apparently autosomal recessive disorder and discuss the differential diagnosis.

Treatment of advanced ovarian carcinoma with hexamethylmelamine, doxorubicin, and cis-platinum (HAC): results in both untreated and previously treated patients.

Twenty-two patients with advanced (FIGO stages III and IV) adenocarcinoma of the ovary were treated with 28-day cycles of hexamethylmelamine, doxorubicin, and cis-dichlorodiamine-platinum (II) (HAC). After 45 months, there were 21 evaluable patients. The median survival was 16 months. Response was achieved in 82% (9/11) who had received no prior chemotherapy, and in 50% (5/10) previously treated. HAC therapy was readily administered on an outpatient basis, with comparatively low major toxicities, primarily hematologic, neurologic, and gastrointestinal. These results indicate that HAC therapy is an effective regimen for patients with advanced ovarian carcinoma, regardless of their prior treatment status.

The treatment of melanoma with m-AMSA. A Cancer and Leukemia Group B phase II study.

The activity of m-AMSA was evaluated IN 39 patients with advanced malignant melanoma. Seventy-nine percent of the patients had some prior chemotherapy. The others had some combination of surgery, radiotherapy, and immunotherapy prior to this study. Patients were treated every 3 weeks starting with 60 or 120 mg/m2 of m-AMSA depending on the extent of prior treatment. Doses were escalated if nadir WBC counts were greater than 2500/microliter. Leukopenia was the dose-limiting toxicity with 7.5% of patients having nadir WBC counts less than 1000/microliter. Of the 39 patients evaluable for response, all had progressive disease. In this study, m-AMSA in myelosuppressive doses was not active in malignant melanoma.

Effect of fluoridation on dental health in 5- and 11-year-old Irish schoolchildren.

Sixty percent of the population of the Republic of Ireland live in communities served with fluoridated water supplies. A study was carried out to compare dental caries levels, treatment needs, and the cost of meeting these needs, in one town with fluoridated water supplies and two smaller towns without. In the fluoridated town 5-yr-old children had 36% less caries and would cost 29% less to treat than the corresponding age group in the non-fluoridated towns. Restorative treatment levels in both areas were minimal for this age group. In the 11-yr-old children there was a 42% difference in DMFT and a cost saving in treatment required because of caries of 50% in favour of the fluoridated community. Anterior permanent teeth benefited most, with a reduction of 81% in mean DMFT. In both areas most of the decay was seen in the permanent first molar teeth. Restorative treatment levels, while higher for the 11-yr-old age group, were still unsatisfactory.

Localized renal aspergillosis with hairy cell leukemia: a review of urinary tract aspergillosis in malignant and nonmalignant conditions.

A case of extensive unilateral renal aspergillosis in a man with hairy cell leukemia is described. This case report is unique because of the long duration of the infection that is more characteristic of patients without malignancy. The degree of unilateral renal involvement found, which included invasion of kidney parenchyma, is uncommon in patients with malignancy. Such patients typically have diffuse disseminated disease with microabscesses. The literature on urinary tract aspergillosis in patients with and without underlying malignancy is reviewed.

Copper clearance in a subgroup of patients with chorea.

Rat studies recently have suggested that the corpus striatum appears to be selectively and markedly affected by copper deficiencies beginning in the post-weanling period. A study of copper clearances has revealed that four out of five patients with chorea have depressed clearances between 0.00674 and .00313 (reference range 0.0137-0.0404 cc/min). The patients with depressed copper clearances had psychiatric illness themselves or in first-degree relatives. In the case of one patient, copper supplementation resulted in an abatement of symptoms that reversed on a single-blind crossover.

Plasma catecholamine levels in normal subjects and in patients with secondary hypertension.

We compared normal values for human venous norepinephrine (NE) and epinephrine (E) as reported in the literature with values determined in this laboratory and we measured and contrasted NE levels in patients with primary and secondary hypertension. Analysis of published data from many laboratories involving more than 800 supine, resting, healthy subjects indicated an average circulating level of venous NE of 260 pg/ml and of E, about 35 pg/ml. Supine levels of NE normally double when normal subjects stand for 5 min. This simple test provides one assessment of overall sympathetic nervous system integrity. Levels of catecholamines have been extensively studied in essential hypertension but much less so in secondary hypertension. Of the groups we studied with secondary hypertension (diabetes mellitus, primary hyperaldosteronism, polycystic kidney disease, chronic bilateral renal parenchymal disease, and unilateral renal arterial stenosis), only the group with renal parenchymal disease had supine NE levels significantly higher than the control group. Patients with essential hypertension and diabetes had a blunted increase in NE on standing. Plasma levels of NE do not reliably differentiate these groups of secondary hypertension from one another or from patients with primary hypertension.

Administration of a complex chemotherapy regimen: inpatient versus outpatient treatment.

The ability to administer a complex chemotherapy regimen, adriamycin and cis-platinum, was evaluated in two different settings, inpatient and outpatient. On comparison, outpatient administration proved superior. This resulted from the interest and experience of the staff administering the medications. It is recommended that outpatient administration of chemotherapy, even with complex protocols, be employed whenever feasible.

Glomerulonephritis associated with chronic bacterial infection of a dacron arterial prosthesis.

Glomerulonephritis associated with severe chronic visceral or systemic infection is being increasingly recognized. The development of glomerulonephritis in association with chronic infection of prosthetic material permanently implanted in the circulatory system has been regularly reported since the description of "shunt nephritis" by Black, Challacombe and Ockenden in 1965. Nearly all of these cases have involved ventriculo-vascular shunts. However, this report documents the apparently rare association between glomerulonephritis and chronic bacterial infection of a bifurcate dacron aorto-bifemoral graft. The need for awareness of glomerulonephritis as a cause of acute renal failure in chronic infective states is highlighted.
