The ability of a Self-Administered Alcohol Screening Test (mSAAST) to detect future excessive alcohol consumption in persons on methadone maintenance.

A modified Self-Administered Alcoholism Screening Test (mSAAST) was prospectively administered to 625 narcotic addicts participating in a randomized clinical control trial to study its effectiveness in detecting future excessive alcohol consumption. At entry into the study, of 105 patients categorized as active alcoholics, 84 (80%) obtained a positive score on the mSAAST. Of 77 patients initially identified as being at risk for developing alcoholism (potential alcoholics) by mSAAST scores, 31 (40%) subsequently developed characteristics of excessive alcohol consumption. Of the 321 patients classified as nonalcoholic by history as well as mSAAST score, a significantly lesser proportion (15%) developed characteristics of excessive alcohol consumption (p less than 0.001). These findings suggest the mSAAST to be a useful adjunctive indicator of persons at risk of developing excessive consumption of alcohol. It is probable that the usefulness of the mSAAST extends beyond narcotic-dependent populations and may be of value in identifying the potential for excessive alcohol consumption in other populations.

DNA sequence relationship of the baboon endogenous virus genome to the genomes of other type C and type D retroviruses.

Baboon endogenous virus (BaEV) is a type C retrovirus present in multiple proviral copies in the DNA of baboons. Although interspecies antigenic determinants present on reverse transcriptase and gag proteins are shared among all mammalian type C viruses, no nucleic acid homology between BaEV and other type C viruses (except RD-114) has been found in conventional liquid hybridization experiments. In this study, we used restriction fragments of cloned BaEV DNA immobilized on nitrocellulose to test for relatedness with [(32)P]cDNA's of various type C and type D viruses. We detected the following distant relationships previously found only through immunological and protein sequencing techniques: (i) eight type C viral cDNA's (the endogenous virus of rhesus monkeys, feline leukemia virus, simian sarcoma virus, gibbon ape leukemia virus, Rauscher murine leukemia virus, BALB-2, NZB, and RD-114) and two type D viral cDNA's (Mason-Pfizer monkey virus and squirrel monkey retrovirus) were able to hybridize with cloned BaEV DNA; (ii) the eight type C probes hybridized to restriction fragments spanning most of the BaEV genome, but only RD-114 hybridized to fragments within the 1.9 kilobases at the 3' end of the genome; (iii) the two type D probes hybridized primarily to fragments within the 1.9 kilobases at the 3' terminus and weakly or not at all elsewhere; and (iv) [(32)P]cDNA's of several other oncornaviruses (mouse mammary tumor virus, equine infectious anemia virus, bovine leukemia virus, and reticuloendotheliosis virus) exhibited no homology with BaEV DNA. DNA sequence analysis has allowed us to orient the BaEV restriction map with the genetic map at both ends of the genome. Homologies between retroviral cDNA's and BaEV clone restriction fragments could thus be related to specific BaEV genes. Whereas type C cDNA's hybridized to fragments from gag, pol, and the pol-env junction, squirrel monkey retrovirus cDNA hybridized only to a fragment coding for the p15E portion of env. Mason-Pfizer monkey virus cDNA also hybridized within the p15E region, but exhibited homology to the 3' half of gp70 as well. These results are discussed relative to previously reported antigenic relatedness of retroviral proteins. The data suggest that BaEV represents an important link in oncornavirus evolution.

In situ hybridization: general infectivity assay for retroviruses.

We have devised a general infectivity assay for retroviruses. A virus-specific [32P]DNA probe is hybridized in situ to a monolayer culture, and foci of infected cells in the monolayer are detected by exposure of the hybridized culture to X-ray films. The method is quantitative, in that it gives the same titer for Moloney murine leukemia virus as does the standard UV-XC test. The specificity of the assay is indicated by the fact that murine leukemia virus and baboon endogenous virus do not cross hybridize under the conditions used. The assay is completed within 1 to 3 weeks and should be broadly applicable for retroviruses which replicate without altering cellular morphology: its use is demonstrated with mouse mammary tumor virus and the helper virus of the reticuloendotheliosis complex.

Therapeutic neuromuscular paralysis in neonates: characteristic radiographic features.

Neonates with respiratory distress requiring mechanical ventilation may be treated with muscular paralysis to improve oxygenation. This results in characteristic radiographic features that relate in part to the specific drug used. The radiographic signs are: bell-shaped chest, decreased bowel gas, and soft-tissue edema. When all three findings are present, the use of neuromuscular blockade can be suggested from the radiographs alone without the aid of clinical history. Radiographs of 57 infants treated with muscular paralysis and mechanical ventilation were compared to 20 infants treated with mechanical ventilation alone. In paralyzed patients, a characteristic bell-shaped chest was seen in 24 of 57 and decreased bowel gas in 46 of 52. Soft-tissue edema was seen in patients treated with metocurine, and the incidence increased with duration of therapy (18 of 25 treated for 5 or more days); it was not radiographically detected in patients treated with d-tubocurarine (0 of 13). Bell-shaped chest, decreased bowel gas, and soft-tissue edema occurred one, three, and one times, respectively, in 20 nonparalyzed control infants, and each time the findings carried significantly different clinical implications. All cases were reviewed to determine if pulmonary edema can result from mobilization of soft-tissue edema fluid after cessation of neuromuscular paralysis, and this was found not to occur.

Abdominal masses in children: multiorgan imaging with 99mTc methylene diphosphonate.

Technetium-99m methylene diphosphonate (99mTc MDP) imaging was performed in 29 consecutive children with abdominal masses. Dynamic images of the inferior vena cava were obtained by injecting the radiotracer in the feet. Serial renal images were obtained for the next 30 min. Routine bone imaging was performed at about 3 hr. The radionuclide studies of the inferior vena cava accurately diagnosed total obstruction and displacement to the left but not partial obstruction and displacement to the right. The abnormalities on early renal imaging included displacement (14), distortion (seven), obstruction (eight), and nonvisualization (one). All patients with Wilms tumor (eight) had either nonvisualized or distorted renal parenchyma. Patients with neuroblastoma (17) and other tumors (four) had displacement and obstruction. Soft-tissue accumulation of 99mTc MDP was noted in two Wilms tumors and 12 neuroblastomas.

Lung CT for detection of metastases: solid tissue neoplasms in children.

The results of 73 computed tomographic studies of lung and pleura in 52 children with solid-tissue tumors have been analyzed. In nine children computed tomography provided information not evident by chest radiograph or whole lung tomography. In four of these cases computed tomography demonstrated lung nodules not seen on chest radiography or whole lung tomography; in another four cases it revealed pleural involvement with tumor; and in one case it revealed calcium in a nodule. In two children computed tomography missed lung mestastases seen by lung tomography and chest radiography.

Alcohol consumption in persons on methadone maintenance therapy.

Alcohol use among methadone maintenance treatment program (MMTP) patients is substantial and a frequent impression is that addicts accelerate their consumption once in MMTP. This study reports an attempt to more clearly define alcohol consumption in opioid dependence as well as changes associated with methadone maintenance therapy. Two-day alcohol consumption (2-day EtOH) data were collected initially and quarterly on participants in a randomized controlled trial of intervention for alcoholism. Blood alcohol levels (BAL) were also determined. 17% of the participants were classified as active alcoholics (AA) and 8% as inactive alcoholics (IA). Two-day EtOH and BAL were significantly higher for AA than AI and nonalcoholics (NA, and AI higher than NA. Two-day EtOH decreased significantly over time for AA, AI, and NA. These findings indicate that AA among narcotic addicts and those on MMTP consume large quantities of EtOH but that consumption decreases with time on MMTP. The results, in confirmation of the work of others, suggest that entry and participation in MMTP has a broader effect than just on the use of narcotic drugs.

Reversible doxorubicin-induced congestive heart failure.

Doxorubicin hydrochloride is a chemotherapeutic agent highly effective against a wide range of neoplasms. A prime limiting factor to the administration of this drug is cardiotoxicity, which frequently develops when the cumulative dose exceeds 500 mg/sq m. Late cardiomyopathy, which may develop up to a year after therapy has been discontinued, was thought to be rapidly progressive and unresponsive to standard cardiac therapy. An adult who received 475 mg/sq m of doxorubicin hydrochloride experienced a cardiotoxic reaction one year after the completion of therapy. The patient responded to standard cardiac therapy. Resolution of left ventricular dysfunction was verified by echocardiography and radionuclide angiocardiography.

An evaluation of prostaglandin E2 administered intrarectally to induce labour.

Prostaglandin E2 (PGE) 4 mg administered as an intrarectal suppository failed to induce labour in 70% of patients with a favourable cervical score. This may indicate that intravaginally administered PGE, which is effective in inducing labour in 70% of patients, acts at least in part directly on the cervix.

Short-term efficacy trial and twenty-four-month follow-up of flunisolide nasal spray in the treatment of perennial rhinitis.

Seventy-eight patients with perennial rhinitis underwent a double-blind, placebo-controlled. 12-wk trial with flunisolide nasal spray, a new potent topical steroid. Eighteen of these patients were followed in an open study and evaluated at intervals for side effects and dosage of spray used. Baseline and plasma cortisol concentrations were performed before and at the end of the 12-wk, double-blind period. Adrenocorticotropic hormone (ACTH) stimulation testing was performed on six patients after 1 yr of flunisolide therapy at 300 micrograms/day or less. Flunisolide was found to be safe and effective over a short period. Over a 2-yr follow-up there were no serious side effects or evidence of adrenal suppression. Ten patients with perennial rhinitis continue to obtain subjective benefit after 2 yr of therapy with flunisolide nasal spray.

Adsorption of monovalent and divalent cations by phospholipid membranes. The monomer-dimer problem.

A generalization of the Stern theory is derived to treat the simultaneous adsorption of monovalent cations and divalent cations by single-component phospholipid membranes, where the ion:phospholipid binding stoichiometries are 1:1 for the monovalent cations and 1:1 and/or 1:2 for the divalent cations. This study treats both the situation in which the monovalent and divalent cations compete for membrane binding sites and that in which they do not compete. The general formalism of the screening/binding problem is reviewed, and it is shown how the adsorption problem can be isolated from the electrostatics. The statistical mechanics of mixed 1:1- and 1:2-stoichiometric adsorption (the monomer-dimer problem) is treated, and the problem of simultaneous 1:1 and 1:2 binding is solved. A simple expression for this solution, given in the Bethe approximation, is combined with the electrostatics to yield an adsorption isotherm encompassing both 1:1 monovalent-cation, and 1:1 and 1:2 divalent-cation, binding to charged membranes. A comparison with the simplified treatment of previous authors is made and the significance of their assumptions clarified in light of the present result. The present and previous treatments are plotted for a representative case of Na+ and Ca++ binding to a phosphatidylserine membrane. Criteria are established to permit unambiguous experimental testing of the present vs. previous treatments.

Imine analogues of tricyclic antidepressants.

Analogues of tricyclic antidepressants were synthesized in which the alpha-carbon of the side chain was replaced by nitrogen. The antidepressant activity of these imines, as measured by the reversal of the effects of tetrabenazine in mice, showed a structure-activity relationship similar to that of the carbon analogues. The most active imine (19) was six times as potent as amitriptyline. Some of the compounds differed from amitriptyline in that they produced stimulation in mice.
