[The surgical form of the hemolytic-uremic syndrome].

The hemolytic-uremic syndrome consist of micro-angiopathic hemolytic anemia, acute renal failure, and thrombocytopenia following a prodromal illness of gastroenteritis. The syndrome can present in dramatic fashion with severe abdominal pain and signs of peritonitis suggesting an acute surgical crisis. In some cases, laparotomy is necessary, and we report a case with colectomy.

[Retrospective study of 56 congenital diaphragmatic hernias. Prognostic factors, therapeutic deductions].

Fifty-six neonates were treated for congenital diaphragmatic hernia (48 left and 8 right) in our intensive care unit from 1972 to 1980. Because of the high mortality, we studied the factors which could predict the outcome. Early onset of symptoms (before the 2nd hour of life) and low post-ductal shunt (PAO2/FiO2 less then 15) after the 3rd postoperative hour appear to be signs of poor prognosis. Our present attitude is early surgical procedure, controlled ventilation with a FiO2 1 with Pancuronium, prevention of pneumothorax and different use of pulmonary arterial vasodilatators.

[Pseudo-surgical aspects of hemolytic-uremic syndrome in childhood (author's transl)].

The hemolytic-uremic syndrome consists of hemolytic anemia, acute renal failure and thrombocytopenia. In childhood, some hemolytic-uremic syndrome can take the mask of a pseudo-surgical crisis mimicking intussusception or ulcerative colitis. Two cases are reported in children with abdominal pain, rectal bleeding, anomalies on the barium enema and in one case histologic features of ulcerative colitis. In the two cases, the biological disturbance of the hemolytic-uremic syndrome appeared with delay after the onset of the abdominal syndrome making the diagnosis difficult. Typical findings on the barium enema must evoke the hemolytic-uremic syndrome before renal failure: narrowing of the lumen of the colon, "thumbprinting", spasms and ulceration. But sometimes the diagnosis is more complicated because of the possibility of true surgical complications of hemolytic-uremic syndrome.

[Acute leukaemia during pregnancy: favourable course of pregnancy in two patients treated with cytosine arabinoside and anthracyclines (author's transl)].

A report of two cases of acute leukaemia discovered during pregnancy. The first was treated from the 24th week with cytosine arabinoside, daunorubicine and vincristine and then rubidazone. Only an incomplete remission was obtained. The pregnancy ended with the birth at the 36th week of a normal child, which remains well at the age of 13 months. The second patient received during the 29th week of pregnancy chemotherapy combining cytosine arabinoside and daunorubicine. Signs of foetal distress appeared at 31 weeks. Delivery took place during the 33rd week in a phase of aplasia. The child was born apparently dead and was resuscitated. A complete remission of the leukaemia was obtained. The child was well at the age of 13 months. A transient neutropaenia was noted at the age of 2 months in both children.

Diagnosis of pneumocystis carinii pneumonia using an endobronchial brushing technique. A report on twenty-one cases in immunocompromised children.

By examining the endobronchial brushings from 65 infants and children with interstitial pneumonia--characterized by a severe hypoxia and diffuse lung infiltrates--we have detected the cysts of pneumocystis carinii in 17 of 21 subsequently confirmed cases. The rapid diagnosis and subsequent specific treatment have allowed the recovery of 18 of these children. The particular significance of this article is the youth of the patients, whose ages ranged from 4 months to 15 years.

[Neonatal hernias of the dome of the diaphragm. Why a gloomy prognosis?].

The prognosis is neonatal hernias of the dome of the diaphragm remains extremely gloomy. Associated malformations, prematurity or dysmaturity, cannot explain it. The surgeon is now faced with conditions diagnosed in the labour room, which he previously never saw (and which therefore did not figure in his statistics). Those forms in which the lung gas exchange area is reduced are above all those in which there is a syndrome of persistence of foetal circulation. The association of drugs from the alphalytic group (tolazoline) and curarising agents (pancuronium) is often effective without having completely resolved the problem.

[Creatine kinase BB sera in neonatal injury children (author's transl)].

The purpose of this study is to find, besides the determination of lactate in blood, a suitable and specific indicator of brain damage in newborn. Accordingly, we studied the total creatine kinase (CK) activity, by the method of Rosalki, and its subunit isoenzymes BB (CK BB) by fluorescence following electrophoresis on cellulose acetate. A first group (thirty newborns) without any disease represents our taest group. We havae found that the normal ranges for CK total were elevated (about 7 200 nkat/l). Electrophoresis of sera from these patients showed in addition to the normally migration isoenzyme (CK BB) was present in very small amounts, no sufficient for quantification. A second group was constituted to thirty ill newborns, with perinatal brain damage. In twenty-eight to thirty children, we find a significant correlation between the level of CK BB and brain insult. But, we have showed that it was necessary to take in consideration the time passed between the hypoxic insult and the blood puncture. Otherwise, we showed by immuno inhibition with specific antibody and by chromatography with gradient elution that identification of CK BB by electrophoresis cannot be misinterpreted. Furthermore, we demonstrated by this method, that position of CK 1 BB in blood is exactly the same that an purified human extract. This study concluded that a high level CK 1 BB in blood of newborn infants with perinatal brain damage has an accurately diagnosis value, if the blood puncture is done immediately during the severe CNS damage.

[Severe blood coagulation disorder as the first sign of a peritoneal hemangio-endothelioma: a new therapeutic approach (author's transl)].

The authors describe a 3 1/2 month old infant with hemangio-endothelioma and a severe blood coagulation disorder. The tumor was inoperable and the severe blood coagulation disorder posed considerable therapeutic problem. After treatment with massive amounts of blood clotting factors, an antifibrinolytic drug and radiotherapy, the child's condition improved.

[Neonatal cardiac tumour: diagnostic and therapeutic problems. One case (author's transl)].

We report a case of cardiac tumour, presenting as cardiac failure during the first hours of life and refractory hypoxia. The diagnosis was made with difficulty by angiography which revealed a filling defect in the right atrium, whilst echocardiography had shown only a localised pericarditis. The tumour (which extended into the pericardium) was found to fill the right atrium and appeared to extend towards the venae cavae and the left atrium, contraindicating any attempt at excision. The newborn infant died on D7 of a strictly intra-atrial well-limited fibromyoma.
