[A case of hepatoblastoma using intraarterial hepatic chemotherapy with THP-ADR].

The patient was a 2-year-old who was admitted with the diagnosis of liver tumor. The diagnosis at admission was stage III A hepatoblastoma complicated with tumor thrombosis of portal vein. Soon after the admission, the patient's general condition became deteriorated with increased ascites and severer jaundice. As the patient was considered to be in oncogenic emergency, chemotherapy with systemic administration of ADR and CDDP was started. Despite decreases in tumor marker and tumor shrinkage on imaging examinations, tachycardia and arrythmia occurred from the end of the second course of chemotherapy, suggesting ADR-induced cardiomyopathy, when the systemic administration of ADR was switched to less cardiotoxic hepatic arterial infusion of THP-ADR. Following 13 courses of intra-arterial infusion with the total dose of THP-ADR of 200 mg/m2, the tumor was found to have reduced markably in size and thus surgical resection of the tumor was performed. Our experience of this case indicates that, considering the pharmacokinetics and side effects, THP-ADR should be a very effective anti-tumor agent for intra-arterial hepatic infusion. The patient died 10 months after surgery because of multiple metastases into the brain and lungs.

[Detection of a 14;18 breakpoint cluster region in B-cell lymphomas by PCR methods].

Cytogenetic studies of B-cell lymphomas have shown a t(14;18) chromosomal translocation in the majority of cases. However, its detection is not always simple to achieve and a new approach is urgently required. Recently, molecular biological analysis has revealed rearrangements of IgH and/or bcl-2 genes in the DNA of lymphomas. In these cases, a translocation-specific fragment, MBR (bcl-2/JH), could be amplified by PCR. Our present study clearly demonstrated the presence of MBR PCR products in 7 out of 8 cases with B-cell lymphoma. Thus, the results indicate that amplification of MBR by PCR is useful for detection of a translocation and therefore clinical diagnosis of lymphoma cells.

Photoaffinity labeling of lipoproteins in human cerebrospinal fluid with a heterobifunctional derivative of galactosylsphingosine.

The binding of a galactosylceramide (GalCer) analog to proteins in normal human cerebrospinal fluid (CSF) was examined by photo-affinity labeling using a radioiodolabeled 2-(p-azido-salicylamido) ethyl-1,3-dithiopropionate (ASD) derivative of galactosylsphingosine (GalSph) as a probe. The affinity-bound peptides appeared at 66, 36 and 28 kDa as radiolabeled bands. The latter two peptides, 36 and 28 kDa, were independently identified by immunostaining of the isolated peptides using biotinylated GalSph, and immobilized Avidin, and by immunoprecipitation of the photolabeled peptide, to apolipoproteins (ALPs) E and A-I, respectively. The direct binding of the GalCer analog to these apo-peptides suggested that the existence in the body fluids or the transfer between the fluids and cells of the glycolipid was related to high density lipoprotein (HDL) constructions.

Glycine conjugation activity of benzoic acid and its acinar localization in the perfused rat liver.

Glycine conjugation activity toward benzoic acid (BA) was studied in the single-pass perfused rat liver preparation. The steady-state hepatic extraction ratio for the portal vein (PV) perfused liver (at 10 ml/min) was maximal (0.6) at input concentrations < 40 microM among perfusions varying from tracer to 700 microM. Glycine conjugation was the predominant pathway; the kinetic parameters estimated after appropriately correcting for plasma protein binding (KA = 8.37 x 10(3) M-1 and 1.9 sites) revealed a low Km (12 microM) and a moderately high Vmax (101 nmol.min-1.g-1 liver) system. Biliary excretion of BA and its glycine-conjugated metabolite, hippuric acid, was minimal. Under first-order conditions (input concentration < 2 microM), the method of HAPV and HAHV perfusion (trace [14C]benzoate delivered via the hepatic artery (HA) at 2 ml/min and blank perfusate via the portal vein (PV) or hepatic vein (HV) at 10 ml/min) was used to examine the localization of glycine conjugation activity toward BA. During steady state, a multiple indicator dose of 51Cr-labeled red blood cells (vascular marker), [58Co]EDTA (interstitial space marker, which behaves similar to labeled tracer sucrose) and 3H2O (cellular marker) was injected as a bolus into the HA. Values of the extraction ratio of BA for HAPV perfusion (0.59 +/- 0.09) were dramatically reduced during HAHV perfusion (0.061 +/- 0.033, P < .01).(ABSTRACT TRUNCATED AT 250 WORDS)

Human blood group B-active ganglio-glycosphingolipid in rat glioma.

A human blood group B-active glycosphingolipid, belonging to the ganglio-series, was isolated from rat glioma cell line RG2 subcutaneous isografts. The oligosaccharide structure of the glycosphingolipid was completely characterized as Gal alpha 1-3(Fuc alpha 1-2)Gal beta 1-3GalNAc beta 1-4Gal beta 1-4Glc beta 1- 1'ceramide by NMR spectrometry, negative fast atom bombardment-mass spectrometry, sequential degradation by glycosidases and methylation analysis. Human blood group B antigenicity and the activity of this glycosphingolipid were confirmed by immunostaining on thin-layer chromatography and the inhibition of hemagglutination, respectively. Although the lipid has been detected in rat granuloma, bone marrow cells, spleen, thymus, ascites hepatoma cells and gastric mucosa, this is the first report of the occurrence of the B-active lipid in glioma.

A successful case of intraarterial prednisolone injection therapy in severe ulcerative colitis unresponsive to an intensive intravenous regimen.

A successful case of intraarterial prednisolone injection therapy in severe ulcerative colitis unresponsive to conventional therapy, including an intensive intravenous regimen, is presented. After the direct administration of prednisolone into the mesenteric arteries, the patient's conditions markedly improved, and he succeeded in overcoming the acute stage and entered remission. Accordingly, he safely underwent elective surgery which was finally required because of the inability to withdraw prednisolone therapy. It is well worthwhile to consider this therapy in the last resort of medical therapy for ulcerative colitis.

The effect of velocity of loading on the biomechanical responses of the periodontal ligament in transverse sections of the rat molar in vitro.

Stress-strain relations of this ligament in transverse sections of the mandibular first molar were examined over a wide range of velocities of loading from 1 to 10(4) mm/24 h. With increasing velocities, the maximum shear stress, tangent modulus and failure strain-energy density increased but the maximum shear strain decreased. The mechanical responses at the highest velocity for the molar ligament were compared with those previously found for the incisor ligament. Mechanical strength, stiffness and toughness were greater for the molar than for the incisor ligament; the molar ligament therefore has more extensible fibres or a different fibre arrangement. Comparison of the mechanical responses at the slowest velocity suggests that, though the stress level was greatly reduced (presumably because of stress relaxation), the fibre components of the molar ligament still reacted at this velocity. It is also suggested that the differences in the ratios of the mechanical measures in 10(4)-1 mm/24 h between the two types of teeth are due partly to their different periodontal fibre architectures.

Properties of gamma-glutamyl transpeptidase in squamous cell carcinoma.

gamma-Glutamyl transpeptidase (GGT) was extracted from squamous cell carcinoma tissues of human skin (SCC) by Triton X-100 and bromelain treatment, and some of its biochemical properties were compared with those of GGT extracted from eccrine gland-rich tissue and normal kidney. GGT activity significantly increased in SCC, but there was no definitive differences in enzymological properties between GGT of SCC and normal tissue enzyme. However, GGT of SCC was distinguishable from those of normal tissues by isoelectric point, electrophoretic mobility, and sensitivity to neuraminidase treatment. These results indicate that GGT of SCC has some variant properties which may be related to skin carcinogenesis.

Molecular characterization of human rotavirus VP4 genes by polymerase chain reaction and restriction fragment length polymorphism assay.

A restriction fragment length polymorphism (RFLP) assay was developed to examine the genetic variability and similarity of the VP4 genes of human rotaviruses. The VP4 genes of 14 human rotavirus strains, including VP4 serotype P1A strains (Wa, P, VA70), serotype P1B strain (DS-1), serotype P2 strains (M37, 1076, McN, ST3) and serotype P3 strains (AU-1, AU228, K8, PA151, PCP5, MZ58), and those of 2 feline strains (FRV-1 and Cat2) were reverse-transcribed and amplified by the polymerase chain reaction (PCR). The amplified VP4 cDNAs were then digested with a panel of restriction endonucleases (HindIII, NruI, HaeIII, and EcoRI), resulting in the identification of at least one enzyme with which digestion produced an RFLP profile specific for a particular P serotype. Of interest was the presence of two distinct RFLP patterns within the serotype P3 VP4 genes: one corresponding to the VP4 gene carried by the members of the AU-1 genogroup and the other corresponding to the VP4 genes carried by naturally-occurring reassortants between members of the AU-1 and other genogroups.

Increased plasma endothelin concentrations in patients with acute heart failure after myocardial infarction.

We have investigated the relationship between plasma endothelin (ET) concentrations and several clinical characteristics in 31 patients with acute myocardial infarction (MI). ET levels were also measured in 10 age-matched healthy subjects, 9 patients with unstable angina, and 20 patients with chronic heart disease. In patients with MI, although no significant relationship was observed between plasma ET concentrations and measured hemodynamic parameters, plasma levels were higher in patients with pulmonary congestion than in those without this complication (1.61 +/- 0.29 vs 1.21 +/- 0.33 fmol/ml; p < 0.01). No significant difference in plasma ET levels was found between cardiac and peripheral sampling sites (pulmonary artery; 1.07 +/- 0.28, right atrium; 1.02 +/- 0.28, peripheral artery; 1.12 +/- 0.23, peripheral vein; 1.14 +/- 0.38 fmol/ml: N.S.), or among patients with uncomplicated MI, unstable angina (1.00 +/- 0.32 fmol/ml), and healthy subjects (1.01 +/- 0.29 fmol/ml). Increased level were observed in patients with decompensated heart failure due to chronic heart disease, but were not found in patients without pulmonary congestion (1.62 +/- 0.60 vs 1.11 +/- fmol/ml; p < 0.01). These observations suggest that plasma ET concentrations are elevated in the presence of congestive heart failure or severe ventricular depression, but are not persistently increased by myocardial ischemia per se.

Prognostic implications of plasma levels of atrial natriuretic factor in patients with acute myocardial infarction.

Several neurohormonal factors have been proposed as markers of the severity of acute myocardial infarction (MI). To determine whether plasma concentrations of atrial natriuretic factor (ANF) might predict post-MI prognosis, we studied 130 patients with acute MI (97 males and 33 females, mean age 62 years). Within one-half to one day after admission, a blood sample was taken for estimation of circulating ANF. The mean follow-up period was 37 months, and the follow-up rate was 97%. Of the 130 patients, 28 died from cardiac causes during the follow-up period. Patients were classified into three groups according to plasma ANF levels (group 1, < 99 pg/ml; group 2, 100-199 pg/ml; group 3; > 200 pg/ml). The survival curves were constructed by the Kaplan-Meier method. There were significant differences in the cumulative survival rate among the three groups (group 1 > group 2 > group 3; p < 0.001). The baseline characteristics (age, atrial pressure, and cardiac index) were different among the groups, therefore these variables were analyzed by a Cox multiple regression model. Significant predictors of cardiac mortality were plasma ANF class (p < 0.002) and pulmonary capillary wedge pressure (p < 0.007). In conclusion, these observations demonstrated that stratification of acute MI patients by plasma ANF level is a useful non-invasive method for predicting prognosis and for identifying individuals at high risk of cardiac death.

Sclerosing peritonitis associated with keratoconjunctivitis sicca, pleurisy, and secretory otitis media.

A male case of sclerosing peritonitis of unknown cause, associated with keratoconjunctivitis sicca, pleurisy, and secretory otitis media, is presented. These unusual complications are very similar to the adverse reactions to the beta-adrenergic blocking agent, practolol, which is known to cause sclerosing peritonitis. This similarity suggests an etiological association between the sclerosing peritonitis of this case and that caused by practolol. The sclerosing peritonitis of this case may be considered to occur as part of some systemic disorder, and thus casts doubt on the hitherto accepted hypothesis that sclerosing peritonitis results from infectious peritonitis.

[Evaluation of hyperlipidemia in high school student--Part 1].

Recently the risk of ischemic heart disease (IHD) is increasing in young generations. Thus, we evaluated the degree of hyperlipidemia in the students of high school in Miyagi prefecture. At first, we examined the plasma level of total cholesterol in all students as screening. About 10% of all the students showed high level of total cholesterol over 200mg/dl. For second screening, we examined further indexes for hyperlipidemia including lipids, glucose and blood pressure at fasting state and got information about family history, diet and exercise. There was positive correlation between total cholesterol and LDL-cholesterol (LDL-C), but no relation was found among others. Lipoprotein (a) which was an independent factor of IHD showed a broad distribution as described before, and had no relation with other lipid levels. Early education and a familial consultation for protecting from IHD should be followed after the screening of lipids levels.

[Detection of myocardial ischemia by 99mTc-Teboroxime myocardial SPECT: a comparison with 201Tl myocardial SPECT].

Exercise myocardial SPECT using 99mTc-Teboroxime was performed and the regions of ischemic area were compared with 201Tl myocardial SPECT in 8 patients with coronary artery disease. All patients had no evidence of previous myocardial infarction, and underwent bicycle ergometer exercise according to a standardized multistage exercise protocol. At peak exercise, 555 MBq 99mTc-Teboroxime or 75 MBq 201Tl was injected intravenously. After termination of exercise, images were obtained with a gamma camera rotating through a 180 degrees arc from 45 degrees right anterior oblique to 45 degrees left posterior oblique. The concordance of 99mTc-Teboroxime SPECT and 201Tl SPECT was 63% in ischemic region and 83% in normal region. Furthermore, one patient underwent dipyridamole infusion and 99mTc-Teboroxime SPECT was carried out. The images obtained by dipyridamole 99mTc-Teboroxime SPECT were equivalent to the images of exercise 201Tl SPECT. 99mTc-Teboroxime SPECT seems useful for the detection of the coronary artery disease. It was suggested that evaluation of the myocardial viability in acute phase of myocardial infarction can be possible by using 99mTc-Teboroxime SPECT.
