Spinal myoclonus.

The focal involuntary muscular contractions of spinal myoclonus have been associated with neoplastic, infectious, traumatic, and degenerative lesions of the spinal cord. Four patients are described here. In two, the myoclonus is associated with severe cervical spondylitis. One patient had herpes zoster. In the fourth, a segment of thoracic spinal cord is narrow and probably atrophic. Both tetrabenazine and clonazepam were therapeutically effective.

Effect of guanidine, germine, and steroids in a case of botulism.

A patient with severe type A botulism demonstrated several interesting features. Characteristic electrophysiologic findings are not necessary for the diagnosis. Guanidine hydrochloride provided mild improvement. High doses of steroids and low dose of germine-monoacetate had no effect.

Guanidine and germine in Eaton-Lambert syndrome.

A 43-year-old man with Eaton-Lambert syndrome developed chronic interstitial nephritis after 4 years of guanidine hydrochloride therapy. The diagnosis of Eaton-Lambert syndrome was confirmed by clinical neurophysiologic studies and by intracellular electrode studies of end-plate potentials from an intercostal muscle biopsy. Because guanidine had toxic effects in this patient, an alternative form of therapy was tried. Germine-3-acetate (500 mg per day orally) resulted in clinical and electric improvement of the myasthenic syndrome. However, the sensory side effects of numbness of the limbs and unpleasant taste were sufficiently annoying that germine was discontinued.

Locked-in syndrome caused by a tumor.

A case of locked-in syndrome is described. Clinical and radiologic evidence suggested the presence of a brain-stem tumor. Autopsy showed that the patient had a reticulum cell sarcoma. This is the first reported case of locked-in syndrome caused by a tumor.

Wound botulism.

Botulism occurring in patients with wounds has been thought of as a rare disease. A patient with a lacerating wound of his hand and wrist and an avulsion of his fourth finger developed diplopia, dizziness, and slurred speech one week later, followed by generalized weakness and difficulty in swallowing. Repetitive nerve stimulation studies showed signs of neuromuscular block consistent with the diagnosis of botulism. Results of bacteriologic and immunologic tests were not revealing, but the subsequent course of progressing, and retrogressing, bulbar signs and symptoms with eventual nearcomplete recovery confirmed the diagnosis. Increasing awareness and employment of electrophysiologic studies are uncovering increasing numbers of cases.

Amorphosynthesis on the chess board.

Visual spatial perception difficulities on the chest board have been studied in a patient with a dominant hemisphere infarction. The game of chess can be useful to demonstrate and follow the evolution of amorphosynthesis.
