Plasma pyridoxal phosphate as indicator of vitamin B6 status in morbidly obese women after gastric restriction surgery.

Plasma pyridoxal phosphate (PLP) concentrations were determined in 15 morbidly obese women before and after gastric restriction surgery for weight reduction. The subjects received a daily vitamin-mineral supplement containing 2 or 3 mg of vitamin B6 for 9 days before the operation and either a placebo or a multivitamin supplement containing 0.4, 0.8 or 2 mg of vitamin B6 for 3 months postoperatively. During the fourth month, all subjects received 2 mg of supplemental vitamin B6 per day. Dietary intakes of the vitamin were calculated from 3-day intake records kept by the subjects. Blood samples for PLP determination were obtained preoperatively and twice between weeks 4 and 8 and at 3 and 4 months postoperatively. The mean concentration of plasma PLP increased significantly from preoperation to 4 to 5 weeks postoperation and returned to the preoperative level by 6 to 8 weeks, with no further changes during the rest of the experimental period. There was no correlation between plasma PLP and either total or supplemental intakes of vitamin B6 at any of the time periods studied. Significant positive correlations were found between the preoperative and the first two postoperative plasma PLP levels (r = 0.93 and 0.67, p less than 0.001 and 0.005, respectively) and between the rate of weight loss and plasma PLP at 4-5 weeks and at 4 months postoperatively. Muscle PLP reserve may be mobilized during the early postoperative period and complicate the use of plasma PLP as a measure of vitamin B6 status.

The numerical composition of cellular samples from the female reproductive tract IV. Carcinoma in situ cases exhibiting other than normal vaginal flora.

The numerical composition of vaginal, ectocervical and endocervical (VCE) cell samples from patients with carcinoma in situ of the uterine cervix exhibiting other than normal vaginal flora is described. The comparative results of cell counts in patients with carcinoma in situ with normal and other than normal vaginal flora are presented.

The numerical composition of cellular samples from the female reproductive tract. V. Cell cluster patterns in cases of invasive squamous carcinoma of uterine cervix.

One hundred fifty thousand cells in a vaginal, ectocervical and endocervical smears from 25 patients with poorly to moderately differentiated squamous cancer of the uterine cervix were evaluated to determine the numerical composition of cellular clusters, the sizes of the clusters and the cellular types contained. The study is a baseline assessment for the design of automated cytology devices for which monocellular layers or individual cells are of importance. The evaluation indicates that the majority of cells appear in actual or facultative clusters, and that dispersement or breaking up of these clusters can yield improved machine-readable samples.

Misuse of scalp hypothermia.

Scalp hypothermia has been introduced to reduce the temporary epilation associated with certain cytotoxic drugs. This has improved compliance with drug delivery for some patients. It is currently not recommended for use in those tumors with a high prevalence of scalp metastasis, ie, leukemia and lymphoma. We have treated a patient for mycosis fungoides who demanded use of a "cooling cap" while undergoing consolidation chemotherapy. Cutaneous disease recurred on the scalp, with no other evidence of mycosis fungoides. Caution should be used in reducing drug delivery to the scalp while treating tumors manifesting cutaneous stem cell tumor nests.

Cushing's disease presenting as pubertal arrest.

The clinical features of Cushing's disease in childhood are usually abnormal fat distribution, moon face, growth failure and hirsutism. Laboratory studies include evidence of cortisol and ACTH hypersecretion. This report concerns a boy who presented only with arrested puberty and growth failure as physical evidence of Cushing's syndrome and showed rapid progression of puberty once pituitary adenomas were removed.

Delayed separation and the plasma amino acids arginine and ornithine.

When collecting blood for amino acid testing, leaving plasma in contact with cells at room temperature lowers the concentration of arginine and raises that of ornithine. This is presumably due to the arginase content of red blood cells. In contrast, the sum of arginine and ornithine is constant over the first hour, and defines a reference interval of 74-148 mumol/L (mean +/- 2 SD, n = 20) which is more insensitive to delayed separation. The ratio of arginine to the sum of arginine plus ornithine [arg/(arg + orn)] can be used to estimate the number of specimens not separated promptly. A ratio of 0.74-0.50 (mean +/- 2 SD, n = 20) is characteristic of specimens placed on ice and separated promptly, where delayed separation produces lower ratios. Of 91 adult specimens received for plasma amino acid analysis over five months, 35 (38 percent) showed a ratio < 0.50 suggestive of delayed processing.
