The objective of this paper is to report the uncommon association of hyperparathyroidism (one case resulting from an adenoma and the other from chief cell hyperplasia) and sarcoma of bone. The radiographic appearances of both cases were thought to be a little unusual and are discussed in detail. The possible relationship of the sarcomas to brown tumors is discussed.
BACKGROUND: Although a strong association between allergy to cockroach (CR) and asthma has been observed in the United States and Asia, there are little data about the extent of exposure to CR allergen in Europe. OBJECTIVE: To determine the levels of CR allergens in dust samples from apartments in Strasbourg and to determine the concentration and size of CR allergens in the air. METHODS: Nine apartments in a public housing complex were chosen on the basis of visual evidence of CR infestation. Levels of CR allergens (Bla g 1 and Bla g 2) in kitchen and mattress dust samples were measured by immunoassay with the use of monoclonal antibodies. Air was sampled for 3 to 8 hours in the kitchen under undisturbed conditions, during artificial disturbance, and during normal domestic activity by using an impinger and a parallel glass fiber filter and at flow rates of 2 to 20 L/min. Airborne CR and mite allergens were measured concurrently in the bedroom of one apartment before, during, and after artificial disturbance. RESULTS: High levels of Bla g 1 and Bla g 2 were found in kitchen dust from the nine apartments (geometric means of 3919 U/gm [range 530 to 14306 U/gm] and 497 U/gm [range 73 to 1946 U/gm], respectively). Under undisturbed conditions, airborne CR allergens were not detectable in any of the apartments. During vigorous artificial disturbance, Bla g 1 and Bla g 2 were detectable in air samples from seven apartments (geometric means of 4.5 U/m3 [range 0.7 to 17.2 U/m3] and 1.0 U/m3 [range 0.4 to 3.4 U/m3], respectively). Both allergens were predominantly collected on the first stage of the impinger, and 76% to 80% of the airborne allergen was associated with particles greater than 10 microns in diameter. The levels were significantly higher than those collected on the second or third stages of the impinger (p < 0.001). A comparison of the levels of mite and CR allergens showed that the airborne properties of these allergens were similar, that is, measurable only during disturbance and not detectable 30 minutes after disturbance. CONCLUSION: Levels of CR allergen in low-cost public housing in Strasbourg can be as high as or higher than the levels measured in towns in the United States. CR allergens become airborne during disturbance and are primarily associated with particles greater than 10 microns in diameter. Patients with asthma living in urban areas of Europe in housing prone to CR infestation should be evaluated for sensitization and exposure to CR allergens.
We present a photomicrographic technique for analysis of meiosis in Oenothera species and hybrids. In many species, the 14 isomorphic, 1-2 microns chromosomes are organized into a permanent translocation ring. Interspecific hybrid meiotic analysis is complicated by variations in homology between chromosome sets (Renner complexes) of allospecific origin. Though most wild species from complete rings of 14 at meiosis, hybrids often form combinations of smaller rings and bivalents. There is evidence that asynapsis (chain and univalents) also frequently occurs in hybrids. The greater complexity of hybrid meiotic organization can hinder resolution of individual chromosomes in such chains and small rings and encumber photomicrography of all chromosomes in a cell. Confocal laser scanning microscopy (CLSM) offers a new cytogenetic approach in ring-forming Oenothera species and hybrids. Fortunately, crystal violet, the stain classically used with this difficult material, fluoresces at an excitation wavelength of 488 nm and may thus be employed with the CLSM. Serial projection of image maxima from Z-axis confocal optical sections enables the investigator to count chromosomes and establish pairing relationships, regardless of chromosomal distribution.
Although automatic processing is becoming more widely available in general dental practice, there is little objective information regarding the quality of this type of processing. Thus a postal study was undertaken to compare manual and automatic processing using a questionnaire together with a standardised, pre-exposed test radiograph for routine processing. Analysis of variance showed significant differences between the manually processed, automatically processed and control test films, in respect of D Min (P < 0.001), test strip (P < 0.001), and step density values (P < 0.01) but not for D Max (P > 0.1). The manually processed test films generally had significantly less density than the automatically processed test films or control test films.
This study describes an explant organ-culture system in which human colonic mucosa can be maintained for prolonged periods in serum-free medium. Following an initial phase of epithelial cell loss, there was intense regenerative activity, with the reformation of tubular crypts. Estimation of crypt lengths revealed a marked reduction after 5 and 9 days in culture with corresponding increases in labelling indices for the whole crypt. The shapes of bromodeoxyuridine (BrdU)-flash-labelling distribution curves were consistent with a proliferative compartment situated within the lower two-thirds of the crypt. We conclude that this is a useful in vitro model for the study of the effects of growth factors and growth-inhibitory agents in respect of cell proliferation in human colonic mucosa.
In this article we critique two prominent theories of reasoning-mental logic and mental models-and argue that reasoning does not consist of either applying logical rules or constructing mental models. Instead, we propose an operational semantic theory of reasoning, according to which reasoning is based on children's operational understanding of key terms in a given problem. We then go on to consider an important recent developmental theory of reasoning, fuzzy-trace theory. In order to illustrate the view of reasoning proposed here we report a study of class inclusion. Dramatic differences in class inclusion performance were found as the result of linguistic context; performance was significantly higher when an explicit request for a subclass comparison preceded the class inclusion question as compared to a standard condition when the class inclusion question alone was asked. This was the case, however, only when the prior subclass comparison question referred to the same dimensions as the class inclusion question and not when irrelevant subclasses were referred to. Children's performance was also better when they sorted the materials into the supraordinate class as compared to the subclasses, but not when the experimenter sorted them for the child. These effects due to the operational and linguistic manipulations are discussed in terms of a general operational semantic theory of reasoning.
Spirochetes from microbial mats and anaerobic mud samples collected in salt marshes were studied by light microscopy, whole mount and thin section transmission electron microscopy. Enriched in cellobiose-rifampin medium, selective for Spirochaeta bajacaliforniensis, seven distinguishable spirochete morphotypes were observed. Their diameters ranged from 0.17 micron to > 0.45 micron. Six of these morphotypes came from southwest Cape Cod, Massachusetts: five from Microcoleus-dominated mat samples collected at Sippewissett salt marsh and one from anoxic mud collected at School Street salt marsh (on the east side of Eel Pond). The seventh morphotype was enriched from anoxic mud sampled from the north central Cape Cod, at the Sandy Neck salt marsh. Five of these morphotypes are similar or identical to previously described spirochetes (Leptospira, Spirochaeta halophila, Spirochaeta bajacaliforniensis, Spirosymplokos deltaeiberi and Treponema), whereas the other two have unique features that suggest they have not been previously described. One of the morphotypes resembles Spirosymplokos deltaeiberi (the largest free-living spirochete described), in its large variable diameter (0.4-3.0 microns), cytoplasmic granules, and spherical (round) bodies with composite structure. This resemblance permits its tentative identification as a Sippewissett strain of Spirosymplokos deltaeiberi. Microbial mats samples collected in sterile Petri dishes and stored dry for more than four years yielded many organisms upon rewetting, including small unidentified spirochetes in at least 4 out of 100 enrichments.
Human breast cancer frequently metastasizes to the skeleton to cause osteolysis and subsequent pain, pathological fracture, and hypercalcemia. Because bone continuously releases growth factors stored in bone matrix by bone resorption during physiological remodeling and, thus, possibly provides a favorable microenvironment for metastatic breast cancer cells to proliferate, inhibitors of bone resorption used either prophylactically or in patients with established disease, therefore, would seem likely to be useful adjuvant therapy in patients with breast cancer. However, the parameters for monitoring progressive osteolytic bone disease in humans are imprecise. We examined the effects of the third generation bisphosphonate, risedronate, which is a specific inhibitor of osteoclastic bone resorption, in a bone metastasis model in nude mice in which intracardiac injection of the human breast cancer cell line MDA-231 leads to osteolytic bone metastases. Risedronate (4 micrograms/animal/day) was given s.c. to animals (a) after radiologically small but defined osteolytic metastases were observed; (b) simultaneously with MDA-231 cell inoculation through the entire experimental period; or (c) by short-term prophylactic administration before inoculation of MDA-231 cells. In all experiments, risedronate either slowed progression or inhibited the development of bone metastases assessed radiographically. Furthermore, mice treated continuously with risedronate showed significantly longer survival than did control mice. Histomorphometrical analysis revealed that osteoclast numbers were diminished at metastatic tumor sites. Unexpectedly, there was also a marked decrease in tumor burden in bone in risedronate-treated animals. In contrast, the growth of metastatic breast cancer in soft tissues surrounding bones was not affected by risedronate. Moreover, risedronate had no effects on the local growth of s.c. implanted MDA-231 breast cancers in nude mice or on MDA-231 cell proliferation in culture. These data demonstrate that risedronate decreases metastatic MDA-231 breast cancer burden selectively in bone, as well as suppresses progression of established osteolytic lesions and prevents the development of new osteolytic lesions; thus, the data suggest that inhibition of osteoclastic bone resorption may be a useful adjunctive therapy for the treatment of cancers that have colonized in bone.

OBJECTIVES: To investigate the potential advantages of epidural anesthesia in an in vitro fertilization and embryo transfer program. STUDY DESIGN: Between January 1991 and December 1992, 148 infertile patients underwent transvaginal ultrasound guided oocyte retrieval. A total of 44 patients (group A) had 50 retrievals under epidural anesthesia, and 104 patients (group B, control group) 112 retrievals under intravenous sedation (propofol) with mask-assisted ventilation (nitrous oxide). RESULTS: In group A satisfactory anesthesia was achieved in 49 of the 50 cases (98%); one patient required additional intravenous administration of propofol owing to extreme anxiety. No complications were observed. Adversely, in group B nausea was observed in 16 cases (14%) and nausea and vomiting in 7 cases (6%). In group A the fertilization, cleavage and pregnancy rates were 67.2%, 92% and 20% respectively, while in group B the rates were 69.3%, 93% and 19.6% respectively; the differences are not statistically significant. CONCLUSIONS: Epidural anesthesia is an effective method for transvaginal oocyte retrieval but does not improve the treatment outcome as compared to an intravenous sedation with mask-assisted ventilation using propofol and nitrous oxide.
BACKGROUND: Because allergic sensitization seems to occur especially during infancy, we decided to evaluate such an exposure in day nurseries. METHODS: Thirty day nurseries in Marseilles, which were selected at random, were visited during 2 weeks in April 1993. Routine cleaning includes daily cleaning of smooth floors, weekly laundering of sheets, and monthly cleaning of soft toys. Mattresses are encased in synthetic covers. Dust samples were collected from four settings: infants' mattresses and pillows, smooth floors, and soft toys. Levels of mite, cockroach, cat, and dog allergens were analyzed with a monoclonal antibody-based ELISA. RESULTS: Mite allergen levels were lower than the proposed threshold level for sensitization (2 micrograms/gm of dust) on 94% of mattresses and soft toys and on 100% of floors and pillows. Cat allergen levels in mattresses ranged from less than 0.1 to 4.5 micrograms/gm dust. On floors, cat allergen levels ranged from less than 0.1 to 2.4 micrograms/gm dust. Only 10% of pillows and soft toys had levels greater than 2 micrograms/gm of dust. Fel d I levels were significantly higher (p < 0.03) in mattresses from nurseries with curtains and were correlated with the percentage of children with a cat at home. In almost all day nurseries, cockroach allergen (Bla g I and Bla g II) levels were very low. Only three samples from mattresses had dog allergen levels greater than 2 micrograms of Can f I allergen per gram of dust. On floors the level was always lower than 2 micrograms/gm. CONCLUSIONS: These data clearly show that indoor allergen levels are much lower in day nurseries than in most houses. Most samples contain allergen levels below threshold levels for sensitization. Thus children of atopic parents are less likely to become sensitized to indoor allergens in day nurseries than in their own homes. In addition, this study emphasizes the efficacy of avoidance measures such as use of synthetic protective mattress covers, frequent washing of sheets and soft toys, and avoidance of carpets and curtains.
The authors' retrospective survey assessed the incidence and causes of sport- and exercise-related injuries in college students participating in intramural, recreational, and club sports. For their survey, they examined clinical screening forms and medical charts of 300 students (160 men and 140 women) to determine the type of injury, the body part affected, loading behavior of tissues involved, and mechanism of the injuries. Sprains resulting from tensile forces accounted for most of the injuries, with the knee the most frequently injured body part. The occurrence of injuries was equally divided between injuries from overuse, caused primarily by improper body mechanics, and acute injuries resulting from collisions with the ground or floor or from falls. The study suggests a need for programs in sport-injury prevention for college students.

BACKGROUND: Allergic reactions to house dust mites of the genus Dermatophagoides play an important role in the pathogenesis of asthma and other atopic diseases. Dermatophagoides siboney has been described as a species from Cuba. Together with D. pteronyssinus and Blomia tropicalis, it is frequently found in house dust from homes of asthmatics. OBJECTIVES: The aim of this study was to investigate the allergenic composition from the house dust mite D. siboney. METHODS: The characterization of D. siboney extract was performed by SDS-gPAGE and immunoblotting. Purification of individual components was performed by affinity chromatography. RESULTS: At least 16 components between 13 and 98 kDa stained by Coomassie Blue were found. Using a panel of 35 sera from atopic mite sensitive patients 13 components reacted to different extent with patient IgE. Two components, 25 and 14 kDa, bound to specific IgE strongly and frequently, i.e. 80 and 91% of the patients, respectively. Affinity chromatography using crossreacting monoclonal antibodies to group 1 and 2 allergens resulted in purified preparations of 25 and 14 kDa proteins, which showed IgE-binding with the majority of the human sera when tested by immuno-dot. CONCLUSION: Based on the IgE binding profile of D. siboney and on the capacity to react with crossreacting monoclonal antibodies for groups 1 and 2, it is proposed to name these two allergens, 25 and 14 kDa, Der s 1 and Der s 2, respectively.
The relationships between mite and cat allergen levels in the home, skin test reactivity, and severity of asthma were explored in 120 asthmatic subjects, 57 in Vancouver and 63 in Winnipeg. Patients in the two cities were similar in age, gender distribution, and severity of asthma. Skin tests were performed using 13 common allergens, including D. pteronyssinus, D. farinae, and cat dander, as well as control subjects. Patients recorded their asthma symptoms, medication requirement, and twice daily peak expiratory flow rate for 1 yr. Dust samples were collected every 3 mo during each season of 1992 through 1993 in both cities. Mite and cat allergen levels were determined with an ELISA using monoclonal antibodies against Der p I, Der f I, and Fel d I. There was no relationship between skin test reactivity and levels of mite and cat allergens. In children with positive skin tests to either mite allergen, total mite (sum of Der p I and Der f I) allergen level was positively related to the mean daily symptom score and negatively related to the daily mean PEF (% of predicted). There was no such relationship among adult asthmatic patients with positive skin tests to either mite allergen. No relationship was found between cat allergen level and the severity of asthma.
The relationship between exposure to aeroallergens and the acquisition of allergy and asthma has been shown over the past ten years. Thanks to new developments for detecting major allergens amongst the principle aeroallergens, in future it will be possible to measure their airborne concentration and to determine the particle size of particles carrying them. We report the results obtained from three studies in which we have shown that airborne mites Group I and II and cockroaches Bla g 1 and Bla g 2 allergens have a broadly similar airborne behaviour. That is to say that they are not measurable unless the atmosphere is artificially disturbed and they are carried principally on particles of > 10 microns. On the other hand, 30-40% of cat allergens are carried on particles < 5 microns and measurable in the air without any artificial disturbance. According to our data and those of the literature, we propose a different classification for aeroallergens according to characteristic aerodynamics. Although some progress in the standardisation of techniques for sampling airborne allergens may have been accomplished, other studies are required to improve their reliability in order that airborne measurements can become a marker of allergic exposure in both domestic and occupational environment.
The effect of a comprehensive lymphedema management program was assessed in 25 patients in whom moderate to severe lymphedema had developed after surgery and/or radiotherapy for carcinoma of the breast. Intensive treatment (4 weeks) involved massage, compression bandaging, and sequential pneumatic compression, with an adjunct program of education to provide skills in exercise, massage, bandage, and containment garment use. The intensive treatment phase was followed by a self-management phase based on the skills that had been acquired. A significant reduction in limb circumference and volume, with continuing improvement over 12 months of self-management, was observed. There was a decrease in need for physical assistance. Quality of life generally remained high and stable throughout the 12 months. Quality of life specific to lymphedema, however, declined during the intensive phase of treatment, but recovered and surpassed pretreatment levels during the self-management phase of treatment. Perceived comfort and strength in the lymphedematous limb improved, and perceived size decreased. The study confirmed that the combination of multimodal physical therapy and education for self-management reduces lymphedema and its adverse subjective consequences and maintains the improvement thus achieved.
