Isolated thyrotrophin deficiency in diabetes mellitus.

Hypothyroidism in patients with diabetes mellitus is usually primary though rarely secondary hypothyroidism has occurred. An 11 6/12 year old white female developed diabetes mellitus at 8 6/12 years of age. She received treatment up to 40 units NPH daily with adequate control and normal growth. Hypothyroidism was diagnosed after a 3 month history of lethargy, constipation, dryness of skin and decreasing insulin requirement to 10 units NPH per day. Physical examination was entirely normal, except for dry skin. Serum levels of free thyroxine, thyroxine, T3 resin uptake, were low as was 131I uptake. Primary hypothyroidism was ruled out by the absence of goitre, absent antithyroid antibodies, low basal TSH levels and increased 131I uptake after TSH administration. Serum TSH levels rose 4-fold in respone to intravenous TRH administration. The patient was treated with 0.15 mg daily of L-thyroxine with very good response. This report describes a patient with juvenile diabetes mellitus and isolated TSH deficiency with hypothyroidism of probably hypothalamic origin, an association not previously described in children.

A double-blind trial of penbutolol: a new beta-receptor blocking agent in the treatment of angina pectoris.

A double-blind placebo controlled study of angina pectoris with penbutolol was undertaken in parallel groups in fifty-two patients. The duration of the study was six weeks. The dosage range for penbutolol was 8 mg to 50 mg per day. Six patients were dropped from the analysis. Seventeen patients (81%) in the penbutolol series exhibited a 50% reduction in anginal attacks, NTG consumption and subjective improvement. Significant reduction in nitrite intake was observed. Effort tolerance was improved significantly in those receiving penbutolol. Penbutolol was well-tolerated.

Spontaneous renal neoplasms in rats.

Spontaneous renal neoplasms in the rat are uncommon. This paper presents the incidence and histopathological features of various long-term developing renal tumors observed in control rats from 17 carcinogenicity studies (1340 males and 1329 females) in Sprague-Dawley rats and 10 carcinogenicity studies (530 males and 530 females) in Fischer-344 (F-344) rats. Renal cell adenoma (0.08% in Sprague-Dawley and 0.28% in F-344), lipoma/liposarcoma (0.37% in Sprague-Dawley and 0.75% in F-344) and transitional cell carcinoma (0.07% in Sprague-Dawley and 0.09% in F-344) were observed in both Sprague-Dawley (0.49%) and F-344 (1.13%) rats. Pulmonary metastasis was observed from one case of transitional cell carcinoma. Renal cell carcinoma with metastasis to the lung and liver was seen in one F-344 rat. In addition, transitional cell papilloma and nephroblastomas were also observed in Sprague-Dawley rats. Metastases from nephroblastoma were seen in the lungs and renal lymph node in two cases. In both rat strains, the tumor incidence was higher in males than in females.

Laparoscopic transcystic duct balloon dilatation of the sphincter of Oddi.

Balloon dilatation of the sphincter of Oddi has been performed via a laparoscopic transcystic duct technique. Small common duct stones and stone debris have been successfully lavaged into the duodenum in 17 of 20 cases (85%) by this method. Postoperative hyperamylasemia was noted in four patients. Mild clinical pancreatitis was observed in three patients (15%). Further evaluation of this technique as an adjunct to laparoscopic common bile duct stone extraction is warranted.

Biliary lithotripsy as an adjunct to laparoscopic common bile duct stone extraction.

We investigated various energy sources and delivery systems suitable for fragmentation of common duct calculi by a laparoscopic technique. We evaluated electrohydraulic lithotripsy (EHL) using 1.9-Fr probe delivering 80 W and laser lithotripsy using a 200-micron(s) fiber delivering 30-70 mJ/pulse at 5-20 Hz. In vitro biliary stone fragmentation analysis suggested that the laser lithotripsy produced a more controllable fragmentation than EHL. Initial attempts to employ EHL techniques in animal models resulted in common bile duct injury or inadequate fragmentation of stones. In contrast, biliary lithotripsy was accomplished in pigs using the pulsed-dye laser at 10 Hz and 60 mJ/pulse. Histologic evaluation revealed no evidence of ductal injury related to laser stone fragmentation. Subsequently, laser common duct lithotripsy was used in two human subjects. One patient had a 1.8-cm impacted ampullary stone and one patient had a 3-cm intrahepatic stone. In both cases, the stones were removed laparoscopically after laser fragmentation. Our experience suggests that the laser lithotripsy may facilitate laparoscopic common duct stone extraction procedures.

Oxidant stress mechanisms in heart failure.

Evidences clearly indicate that HF is accompanied by excessive generation of OFRs and depletion of endogenous antioxidant system. The resultant oxidant stress depresses myocardial contractility and function by decreasing Ca++ uptake in sarcoplasmic reticulum and by impaired Ca(++)-ATPase in cardiac tissue. The various sources of OFRs production in HF include increased production of nitric oxide, cytokines, prostaglandins, auto-oxidation of catecholamines, activation of polymorph leucocytes and ischemia induced xanthine-xanthine oxidase. The prevention of oxidative stress by antioxidant translates into better metabolism and function of myocytes. It appears that antioxidant drugs may represent a novel adjunct to the existing therapeutic armamentarium in patients of HF irrespective of its etiology and severity.

Laparoscopic cholecystectomy in the heart transplant candidate with acute cholecystitis.

We report the first successful laparoscopic cholecystectomy for treatment of acute cholecystitis in a heart transplant candidate with end-stage heart disease. Eight successful cases of conventional cholecystectomy in heart transplant candidates have been reported, but convalescence after the conventional procedure is prolonged, and morbidity often interferes with a timely heart transplantation. Laparoscopic cholecystectomy is a less-invasive method for treatment of symptomatic cholelithiasis and cholecystitis and may be better tolerated in this patient population. Although further study is needed, we believe laparoscopic cholecystectomy will have applications in patients with end-stage heart disease.

Critical appraisal of fine needle aspiration cytology in tuberculous lymphadenitis.

A cytomorphologic diagnosis of tuberculous lymphadenitis by examination of needle aspirates was made in 560 of 1,471 cases of lymphadenopathy studied over two years. Cytologic features were categorized into four groups: epithelioid clusters with or without Langhans's giant cells without necrosis (32.14%), epithelioid clusters with or without Langhans's giant cells with necrosis (50.35%), occasional epithelioid cells without characteristic necrosis/giant cells (2.85%) and necrosis without epithelioid clusters or Langhans's giant cells (14.64%). While a diagnosis of tuberculous lymphadenitis was offered with confidence in the first two groups, constituting about 82.49% cases, aspirates from the third- and fourth-group patients were subjected to Ziehl-Neelsen staining for acid-fast bacilli, which was positive in 12.5% and 75.6% of cases, respectively.
