Attempt at systematization in the treatment of isolated fractures of the zygomatic bone: techniques and results.

Two series of cases with isolated zygomatic bone fractures have been compared with regard to the treatment applied and its results, i.e. 258 cases treated by conventional methods, and 437 cases treated by osteosynthesis with miniaturized screwed plates (MSP). In the second series, reduction failure is eight times less frequent than in the first series, and fixation by plates is solid in the three spatial planes. The incidence of postoperative infra-orbital nerve sequelae was diminished by 50% in the second series. This easy, comfortable and efficient method is now used by the authors for the treatment of all unstable zygomatic fractures and in all cases presenting the following functional signs: limited buccal opening, diplopia by trapping, or involvement of the infra-orbital nerve.

[Study of the corrosion of metallic biomaterials by surface physical methods].

The authors investigated the corrosion mechanism of metallic implants inserted into the human body in contact with bone. It was shown that a significant degree of corrosion occurred after several months. The dissolution seems to be non-selective but can be different for each individual. The corrosion mechanism is complex, starting with an inter-granular process, but pitting also occurred . The oxidation of Fe2+ to Fe3+ favours the elimination of iron oxides and is followed by the destruction of the local alloy structure. An estimation of the quantity of each type of element released in the organism was also realized.

[Repair of soft palate deficiencies in adults by Orticochea's method (author's transl)].

The authors describe their experience of an elegant technique described by Orticochea in 1968, and which consists of conducting a transposition of the posterior palatine pillars onto a posterior pharyngeal flap, closing the cleft laterally, and thus producing a dynamic pharyngoplasty. This method was applied in 6 exceptional cases with very severe sequelae from clefts or injuries, and results were assessed as being of good quality.

[Treatment of a compound fracture of the mandible with a comminuted compound fracture of the maxilla (author's transl)].

A case is reported of a comminuted compound facial fracture associating a multisplintered comminuted compound fracture of the right palate, with a quadrifocal fracture of the mandible including severe displacement of fragments of a bi-subcondylar fracture. Treatment was difficult and consisted of producing a stable solid osteosynthesis of the body of the mandible allowing rapid extra-oral symphysial traction to reduce the condylar fractures for 3 weeks. Hospitalization lasted 33 days and the final buccal opening was 34 mm, with satisfactory dental articulation. The authors emphasize the need for a solid, stable osteosynthesis of the body of the mandible by using miniaturized plates serewed into the splintered facial complexes.

Mandibular osteosynthesis by miniature screwed plates via a buccal approach.

The modified Michelet's (1973) technique of mandibular osteosynthesis, which consists of monocortical juxta-alveolar and sub-apical osteosynthesis, without compression and without inter-maxillary fixation, is described. This technique can be used in many types of mandibular fracture, single or multiple, associated or isolated, except in the case of a fracture of the condylar neck and in the presence of pre-existing infection. Infected fractures are treated by orthopaedic methods. Materials used (plates and screws) and particulars of the method have been tested by multi-disciplinary experimentation, particularly by anatomical verification and biomechanical studies. The ideal line of osteosynthesis is described. For the author, this technique is a routine treatment of any type of mandibular fracture.

[Facial osteotomies without fixation].

The authors first review the problems associated with inter-maxillary blockage and of classical methods of immobilisation used in all types of facial osteotomy. Their experience of osteosynthesis in traumatology has led them to use whenever possible osteosynthesis alone with screw fixed microplates. The other methods are used only as secondary assistance in the case of necessity. Osteosynthesis without blockage prevents recurrence and does not compromise the result as far as satisfactory dental articulation is concerned.

[Study of stresses in the fractured mandible in man. Theoretical measurement and verification by extensometric gauges in situ].

The authors study the various possibilities for measuring the stresses which develop within the mandible under the effect of masticatory forces. Several methods are proposed:--calculation with the aid of mathematical models,---the use of extensometric gauges in vitro with the aid of traction machines,--the use of extensometric gauges in vivo. The results obtained with all three methods are in agreement and, in the opinion of the authors, provide experimental evidence of the well-founded nature of Michelet's technique of osteosynthesis.
