[Oesophagectomy without thoracotomy. 5 cases (author's transl)].

The authors describe an original technique of excision in severe caustic burns of the upper digestive tract: oesophagectomy without thoracotomy. The new procedure avoids the wide pleural opening of the thoracic approach, which leads to frequent and often fatal respiratory and infectious complications, whereas extrapleural drainage of the mediastinum has always proved effective and safe. Five technically successful operations were performed in one year. One patient died on the 10th post-operative day of tracheal necrosis related of the burn and 4 patients survived. These preliminary results are encouraging.

[Tuberculosis and caustic eso-gastric burns. Critical reflections on one case].

The diagnostic problems which may be raised during caustic oesophago-gastric burns and concommittent tuberculosis, of which the symptoms may be masked by the richness of the initial clinical picture are discussed here. The authors emphasise the necessity of prophylactic anti-tuberculous treatment, before any surgery when there is any suggestion of tuberculosis even siabilised.

[Intensive care and diet in cases of caustic esophageal stenosis after the first eight days].

On the basis of 12 cases of caustic burns of the oesophagus, the authors describe the technique of alimentation, once the acute phase of the first eight days has passed. From the 8th to the 15th days, calories and provided parenterally. An upper GI series is carried out on the 15th day: if the stomach is healthy, the authors fashion an alimentation gastrostomy, if the stomach is damaged, they carry out gastrectomy with alimentation gastrostomy or jejunostomy. In all cases, continuity is re-established by coloplasty two months later.

[Large esophagogastric caustic burns in the adult].

The authors treated, as an emergency, severe caustic burns in adults. They present 13 cases and discuss four deaths. Critical analysis of these observations,compared with the findings of pathology and in the literature, permit one to consider the therapeutic indications. The place of often major surgical operations is discussed in relation to further specialist investigations or the appearance of complications. The results are assessed in 8 patients who were followed up for a long period.
