Sino-atrial dissociation: evidence by intracardiac recordings in man and by microelectrode studies on isolated rabbit atrium.

The electrophysiologic mechanisms of sinus dysfunction have recently been determined by direct recordings of the sinus node electrogram. The association of various degrees of abnormalities in the formation of the impulse within the sinus node and of sinoatrial conduction block, represents the pathophysiological substrate of the mechanism of sinus node dysfunction. The purpose of this work is to present clinical and experimental data supporting the concept of sinus node isolation. In our clinical case, the sinus node was probably intact despite aspects of sinus node dysfunction on the surface ECG. Sinus node electrograms were recorded with a sinoatrial conduction time of 100 ms (normal values in our laboratory: 83 ms +/- 38 ms). Atrial mapping demonstrated that the area depolarized by the sinus node involved a 2 cm2 zone surrounding it. This perisinusal activity could not be recorded on the surface ECG. Both exit and entry blocks in the sinus node were demonstrated. Our experimental data showed a total desynchronization between the electrical activity of the sinus node and that of the atrium under hypoxic conditions. Both types of cases demonstrated that an atrial dysrhythmia was coexisting with regular sinus activity. From these data we concluded that a sinus node free from any pathological involvement could be associated with severe symptoms of sinus node dysfunction on the surface ECG.

Pulmonary artery obstruction due to giant cell arteritis.

Giant cell arteritis is often referred to in the context of polymyalgia rheumatica with temporal artery involvement. There are, however, more malignant forms of presentation of this necrotizing arteritis involving either the great vessels of the aorta or, occasionally, the pulmonary arteries. Our case relates to giant cell arteritis presenting as pulmonary artery obstruction in a patient without polymyalgia rheumatica or extensive aortic or proximal great vessel involvement.

Stereomorphological arrangement of capillaries in skeletal muscle in normal and stenosed young and adult rats.

The stereomorphological arrangement of skeletal muscle capillaries depends on many factors. Genetic biochemical composition of muscle fibers, age, and training can interfere with the growth of the capillary network. This study is focused on the age effect of chronic reduction of blood flow, as in arterial stenosis, in skeletal muscle network of young rats when compared with adult rats. It has been observed that there is a statistically significant decrease of the length of short capillaries which cross the muscle fibers. In adult rats the opposite occurs.

[Psychopathological heterogeneity in opium drug addicts].

The aim of the study was to assess the presence and nature of psychiatric disorders in opium drug addicts. One hundred and six subjects receiving treatment at the CMAS in Cagliari were included in the study. Hathawai and McKinley's MMPI test was preventively carried out on all subjects; each drug addict was then interviewed three times in the space of three weeks in order to formulate a diagnosis in line with DSM III R criteria. The results obtained show a high incidence of psychopathological disorders which are not included among those caused by drug abuse, and a high degree of diagnostic heterogeneity on both axis I and axis II. The comparative assessment of three subsamples undergoing different phases of treatment reveals both qualitative and quantitative differences.

[Long-term sequelae of deep venous thrombosis of the legs. Experience with mesoglycan].

Ninety consecutive patients, affected by venographically proven deep-vein thrombosis (DVT) of the lower limbs, were given full-dose heparin followed by oral anticoagulants for 12 weeks, and then selected randomly to receive, for one year, either mesoglycan (72 mg/day orally) or placebo with a double-blind protocol. All patients wore elastic graduated compression stockings, and were prospectively followed for a period ranging from 5 to 48 months. In each scheduled examination programmed every three months for one year and then twice per year, an accurate clinical evaluation was performed and a predetermined objective score was applied. Furthermore, impedance plethysmography and Doppler ultrasound tests were executed serially to assess the persistence of venous obstruction and/or the development of valve incompetence. After a mean follow-up of 3 years, 80% of the patients were totally asymptomatic, and severe post-thrombotic sequelae (ulcer and/or edema associated with skin induration) were recorded in only 6 patients (6.6%). We failed to identify any correlation between post-thrombotic sequelae and persistence of venous obstruction (as shown by impedance plethysmography) or development of valve incompetence (as shown by Doppler ultrasound test). The behaviour of patients treated with mesoglycan did not differ from that of patients treated with placebo. However, objectively documented recurrences of DVT and/or pulmonary embolism were less frequent in patients treated with mesoglycan (6.6 vs 11.1%, non-significant difference), and the only two deaths attributable to pulmonary embolism occurred among the patients treated with placebo.(ABSTRACT TRUNCATED AT 250 WORDS)

[The side effects of phlebography of the lower extremities using iohexol. A prospective study].

In this prospective study 463 consecutive outpatients, who had undergone phlebography because of clinically suspected deep venous thrombosis (DVT) were examined with clinical follow-up and impedance plethysmography to evaluate the rate of contrast media complications. Seventy-nine patients had immediate and mild side effects, and one had moderate side effects (bronchospasm); no patient suffered from severe life-threatening conditions. There was only one case of DVT which occurred after an initially negative phlebography. In a subgroup of 40 patients, who underwent iodine-125-fibrinogen scanning after phlebography, the study was positive in 9 cases. None of them presented with any evidence of DVT at follow-up phlebography. Contrast phlebography with iohexol is a safe and comfortable procedure. Low-osmolality nonionic contrast media are well tolerated by the patient.

Ewing's sarcoma, extraskeletal and of bone. Case report with ultrastructural analysis.

Ewing's sarcoma is considered a primary malignant tumor of bone. Histologically, it is characterized by small round cells with poorly delineated borders. Ultrastructural studies reveal large amounts of glycogen aggregated in the undifferentiated cytoplasm of the tumor cells which show variable amount of organelles. The cell membrane shows desmosomes in some cases but not in others. Intercellular argentophylic fibers are by and large absent. In a survey of human sarcomas for the possible presence of viral agents, a sarcoma has been encountered in a 15 year old male originating in the area of the posterior bladder wall and prostate that satisfies morphologic criteria for the diagnosis of Ewing's sarcoma but is extraskeletal in origin. Histology and ultrastructure as well as tissue cultural characteristics of this tumor were studied in comparison with a second patient (age 14) with Ewing's sarcoma originating in the right clavicle. None of the two tumors yielded virus particles in culture.

[Evaluation of IGF1 and IGFBP3 during diet therapy in patients with food allergy with complex nutritional risk].

The seric levels of IGF1 and IGFBP3 and their molar ratio, together with clinical and auxological parameters, have been evaluated in 16 patients (8 males and 8 females), of average age of 11.6 +/- 8.66 months, who were suffering from complex food allergy and were subjected to a balanced diet will excluded the "offending" food. The results, compared with those of a control population of same age and body surface, have shown a significative reduction of IGF1 and an increase of the molar ratio IGFBP3/IGF1. These observations suggest that, in spite of no significative variation of growth in height and weight, an exclusion diet for a food allergy can reduce the biological activity of some hormonal factors of growth.
