[Oral lupus erythematosus. Description and analysis of 11 cases].

BACKGROUND: The aim of this study is to evaluate all the cases of lupus erythematosus with oral involvement coming to the Department of Oral Medicine in the last 15 years (1984-1998). METHODS: The files of the Oral Medicine Department of the University of Turin have been reviewed to search oral manifestations of lupus erythematosus according to international accepted diagnostic criteria. RESULTS: A total of eleven patients 7 women (64%) and 4 men (36%) with a mean age of 46 years were included. Three patients (27%) had discoid lupus erythematosus (LED) whereas 8 (73%) had systemic lupus erythematosus (LES). In four patients the diagnosis of lupus was made after oral diagnosis, whereas 7 patients (64%) had a previous diagnosis of lupus. Clinical aspect was typical in 8 cases (73%) while one patient had leukoplakia-like white patches and two patients (18%) had lichen-like oral lesion. CONCLUSIONS: This study suggest that primitive oral lesion of lupus are quite rare but frequently may have atypical features often difficult to differentiate from lichen planus.

[Determination of bone mineral content and correlations with calciotropic hormones in periodic hemodialysis patients].

The investigation on several forms of uremic osteodystrophy by means of bone mineral content (BMC) measurement led to contradictory conclusions. BMC in 27 patients on periodical hemodialysis treatment was measured correlating it to the seric levels of Ca, P, Mg, alkaline phosphatase (AP), calcitonin (Ct), osteocalcin (BGP), intact parathormone (PTHi), c-terminal and mean molecule PTH. Patients on dialysis treatment from a long period of time showed high AP and low BMC levels. This correlation proved significant just for the values recorded at a third distal site of radius. Patients with BMC under the normal range showed higher BGP levels and a longer period of dialytic treatment than those presenting normal BMC. The former showed a Ct inverse correlation as to age and mineralization indexes. Higher values of Ct and BMC have been reported in males rather than in females. Hence BMC is not suited to investigate different kinds of uremic osteodystrophy. Seric PTH dosage is certainly best fitted to discriminate patients affected with hyperparathyroidism from those with low turnover osteodystrophy. BMC determination is a valid support to evaluate the bone mineral loss in patients on haemodialysis treatment. It significatively correlates to the duration of the dialytic treatment; it is higher in female than in male population; it mainly affects cortical components rather than trabecular ones and is related to a seric Ct decrease.

[Epidemiology of osteoporosis in Apulia. A retrospective study on the prevalence of fractures of the wrist and femur in the population of the region].

A study of femur and wrist fracture prevalence in a large population of Apulia was carried out. These kinds of fractures are closely related to osteoporosis in post-menopausal women and in the elderly, suggesting that a survey of their prevalence in the population could demonstrate the prevalence of osteoporosis itself. In this study a high prevalence of total fractures in young men was demonstrated. The same prevalence was observed for wrist fractures, while femur fractures showed a high prevalence in the young and in men aged 60 to 90 years. In women a high prevalence in the first decades of life was observed for both wrist and femur fractures and a progressive and higher increase after 50 years was observed with significantly higher values than those observed in men of the same age. Finally, first admissions to hospital for femur and wrist fractures (representing the complication of osteoporosis) in our region were 5.9% of the total admissions in the Orthopedic Departments in subjects aged over 50 years; these fractures accounted for 0.65% of the total Apulian population of the same age. This datum, although underestimated, could represent the prevalence of osteoporosis in Apulia.
