[Variations in the ratio of total calcium to plasma calcium ions in chronic liver diseases].

Total Ca and Ca++ were evaluated with a Perkin Elmer 305 atomic absorption spectrophotometer and a specific Orion 93-20 electrode respectively in 29 cirrhotics and 76 non-cirrhotics. Stress is laid on the reliability of the Orion electrode. Mean C++ was 52.02 +/- 4.46 mg% in the non-cirrhotics, in cirrhotics was 49.90 mg% +/- 6.14 mg%. An inverted ratio vis-à-vis total Ca was also noted: 54.73 +/- 3.65% in the cirrhotics and 56.51 +/- 3.73% in the non-cirrhotics. Although lacking in significance, these data underscore the importance of both partial and total evaluation of plasma Ca in the prevention of mistaken therapeutic approaches when liver patients.

[A case of chondrodysplasia. Dolichomelic spondylo-epiphyseal chondrodysplasia].

A case of late spondyloephiphyseal chondrodysplasia is described: a 16-yrs patient showing a diffused platyspondyly with a short thorax plus a remarkable dysplasia of femoral heads. The patient exhibited no internal organs, psychical, plasma, or endocrine alterations. The Authors stress the unusual aspect of the case, which exhibited normal stature in comparison with the usual pattern of spondyloepiphyseal chondrodysplasia described; in fact in the typical cases the general stature remains short. According to the Authors, the normal stature of this case might be related with the dolichomyelic aspect presented by the patient. This dolychomyelia is difficult to explain since endocrine abnormalities were not observed and hormone therapy have never been performed. Finally the course and prognosis of this disease is discussed in detail.

[Evaluation of the percentage of increased spongy bone in experimental osteoporosis treated with synthetic anabolic agents].

In this paper, the authors discuss the variations in the quantity of spongy bone in rats of the same sex, same age and same weight subjected to a normocalcic diet, a hypocalcic diet or a hypocalcic diet plus synthetic anabolic agent. In the rats subjected to a hypocalcic diet, there is a statistically significant reduction in the quantity of spongy bone, in comparison to that of the other two groups. In other words, the hypocalcic diet led to a rarefying osteopathy, which was partially prevented by the use of synthetic anabolic agents. the authors also observed that the rarefying osteopathy was more intense in the distal epiphysis of the femur than in the neck of the femur. If this feature is confirmed in man, it would constitute a precious aid to the early detection of osteoporosis.

[Various forms of secondary lumbosciatic pain in children and adults].

Reference is made to two personal cases in underscoring the fact that meralgia paraesthetica, though regarded as a form that displays a preference for adult males, must be considered as extended to subjects of all ages when a benign or malignant neoformation irritating the L2 root or its trunk is involved. Before classifying a case as idiopathic, therefore, one must rule out the possibility of intra-abdominal disease-usually malformative in young subjects, and benign or malignant in persons of all ages. Lastly, stress is laid on the fact that atypical lumbar and sciatic pain in the adult may be secondary to aneurysm of the abdominal aorta, the common iliacs and the hypogastric. Hence the need to look for such conditions symptomatologically.

[The value of the calcitonin test in Paget's disease of bone].

Five untreated patients with Paget's disease and one who had been unsuccessfully treated with porcine calcitonin for 2 years were given 20 mg/kg/day disodic ethydronate. Alkaline phosphatase and urinary hydroxyproline values were examined and the progress of the calcitonin test was followed. In addition to being of great importance and in line with the other two parameters in following the course of the disease, this test was found to be an excellent pointer to the moment when no further response is being obtained from treatment with calcitonin.

[Duodenal absorption of calcium in rats treated with 25 OHD3, diphosphonates and synthetic anabolics].

In this study the authors developed new technics of study of duodenal absorption of calcium in rats: atomic absorption spectrophotometry, and dialysis to equilibrium. We considered that the main factors necessary for evaluation of duodenal absorption of calcium are the serum calcium, the calcium and alkaline phosphatase present in the duodenal cells and the calcium-binding proteins. The authors demonstrate that these factors are highest in rats treated with 25 OHD3 and norandrostenolone decanoate, than in control rats. In rats treated with diphosphonate, on the contrary, they obtained a reduction in the values of calcium and alkaline phosphatase contained in the duodenal cells.

[Sharp's syndrome. Clinical, immunological and nosographic aspects].

LE cells, ds-DNA antibodies (radioimmunoassay), antinuclear antibodies (ANA) by indirect immunofluorescence (IFI) and anti-ENA antibodies have been sought in 150 clinical cases observed over a 5-year period in the Rheumatology Division of Bari University. For the latter, three parallel techniques were adopted on each serum, each completed by RNA-sensitivity assay for the demonstration of anti-RNP, i.e. IFI, passive haemoagglutination (PHA) and controimmunoelectrophoresis (CIE). The series included systemic lupus erythematodes (SLE), 30 cases; rheumatoid arthritis (RA), 30 cases; progressive systemic sclerosis (PSS), 12 cases; unclassified connective tissue disease (UCTD), 8 cases; mixed connective tissue disease (MCTD), 7 cases; Sjögren's syndrome (SS), 4 cases; dermatomyositis (DM), 3 cases; overlap syndromes (PSS-SLE, SS-SLE), 2 cases; rheumatological and internal miscellanea, 54 cases, LE cells and ds-DNA antibodies were found exclusively in SLE; the anti-ENA were found in various groups of diseases, while the anti-RNP were only demonstrated in the 7 MCTD and in some SLE. Of the three techniques for demonstrating anti-ENA, the PHA proved most sensitive and CIE most specific, whereas IFI was considered most suitable for clinical screening. The clinical aspects of the 7 MCTD faithfully followed the disease picture described by Sharp, but some overlap-syndromes and the unclassified connective tissue diseases did not present anti-RNP. It is also pointed out that nephropathy is not rare in MCTD and that the clinical course of the disease is not always benign. To conclude, it is considered that MCTD merits nosographic autonomy, but further investigations are recommended for more exact nosographical typing of connective tissue diseases.

[Our experience with over 100 cases of carpal tunnel syndrome].

Reference is made to personal experience with regard to the carpal tunnel syndrome. Attention is drawn to the importance of electromyography in the detection of median nerve deficiency, even in recent forms. The most suitable ways of treating the syndrome are also discussed.

[Rarefying osteopathy in rheumatology. Methods of evaluation and comparison of urinary hydroxyproline and serum proline iminopeptidase].

Serum proline iminopeptidase and (where possible) urinary hydroxyproline were determined in 214 subjects: normal subjects, subjects with chronic kidney disease, subjects with chronic liver disease, subjects with osteitis deformans, subjects with rheumatoid arthritis, and subjects with osteoporosis. The values of the two parameters fully corresponded. It is suggested, therefore, that proline iminopeptidase can be determined instead of hydroxyproline in cases where a primarily destructive bone disease in present.
