Chylous ascites after retroperitoneal lymphadenectomy: successful management with peritoneovenous shunt.

A case of chylous ascites occurring after retroperitoneal lymphadenectomy for testicular carcinoma is described. After failure of conservative therapy, the patient was successfully treated with peritoneovenous shunt. The pathophysiological and therapeutic aspects of this rare complication are described; in the world literature only 7 cases of chylous ascites following retroperitoneal lymphadenectomy are reported.

Granulomatous orchitis: clinical and pathologic aspects.

3 cases of granulomatous orchitis are described. Treatment consisted of radical inguinal orchiectomy in 2 cases, and biopsy followed by steroid therapy in the 3. The clinical and pathologic aspects of this rare inflammatory disease of the testis are reviewed.

Inverted papilloma of the ureter.

The clinical and pathological findings of a case of inverted papilloma of the ureter are reported. Treatment consisted of partial ureteral resection, followed by end-to-end anastomosis. This uncommon lesion can be considered as a benign neoplasm, since recurrence and malignant degeneration are quite exceptional. Therefore a plea is made for conservative surgical treatment, although careful long term follow-up is advisable.

TxA2 production by human arteries and veins.

Human arterial and venous segments from patients under-going operations when incubated in Tris buffer both alone and with arachidonic acid were able to produce thromboxane B2 (assessed by radioimmunoassay). Thromboxane B2 (TxB2) production was progressive in time (till 40 min.) and was enhanced by the addition of 1mM norepinephrine. Contamination of tissues by platelet was checked and platelets did not contribute to thromboxane formation. The investigation of the conversion of 1-14C arachidonic acid by vascular tissue indicated that human vascular tissues produce the metabolites of the cyclooxygenase dependent pathway and that prostacyclin is the main metabolite with a PGI2/TxA2 ratio of 4:1. The arterial wall was found to possess an active lipoxygenase dependent pathway. Thromboxane production by intimal cells was negligible and the main source of thromboxane was the media. The production of thromboxane did not change in relation to age, but arterial segments from men produced significantly larger amounts of thromboxane than those from women.

The effect of cigarette smoke on aryl-hydrocarbon hydroxylase (AHH) activity of the human kidney.

AHH activity was measured in kidney cortex biopsies obtained during surgery from patients with hydronephrosis and from unaffected portions of kidneys with renal cell carcinomas. AHH activity was slightly, but not significantly, higher in the group of non-smoking cancer patients when compared to non-smoking hydronephrotic subjects. Smoking induced a significant increase of AHH activity in the hydronephrotic kidneys but not in cancer kidneys, probably due to the higher baseline values and scatter in the cancer group. Pooling cancer and non-cancer kidneys, smokers had a higher AHH activity than non-smokers, this increase being dose-related.

Myxoid liposarcoma of the bladder.

We report a case of myxoid liposarcoma of the bladder dome. Treatment consisted of partial cystectomy and pelvic lymphadenectomy. The patient had no evidence of recurrence 30 months postoperatively. This is the first reported case of primary bladder localization of liposarcoma.

Retrovesical echinococcal cyst causing acute urinary retention.

A case report of a fifty-six-year-old man with acute urinary retention due to a retrovesical echinococcal cyst is presented. He had a history of surgery for hepatic hydatidosis; diagnosis was made preoperatively with the help of conventional x-ray studies, pelvic sonography, and CT scanning. Treatment consisted of removal of the cyst.

Surgical repair of ureteral defects.

A nineteen year surgical experience with 35 patients requiring repair of ureteral defects is reported. Causes of ureteral damage are described, and the result of 18 patients treated with ileal ureter are compared with those of 17 cases where surgical procedures preserving urothelial continuity were used, such as bladder psoas hitch and renal autotransplantation. Significantly better results were observed with the latter techniques.

Adrenal myelolipoma.

The case of a 46-year-old man with a 6 cm right adrenal myelolipoma is described. This benign tumor presents a "fatty" density at c.t. scan, and can be easily recognized at intraoperative biopsies.

Further studies on adriamycin induced in vitro lipid peroxidation.

Adriamycin (AM)-induced lipid peroxidation was studied in rat liver microsomal system, measuring colour produced by thiobarbituric acid (TBA) reactive material and the development of volatile hydrocarbons. The onset of this degradative process is closely related to the AM content. This phenomenon appears to be highly influenced by the buffer used for the microsomal preparation. The effect of AM on FE--ADP-induced lipid peroxidation in liver microsomes has also been studied.

Experience with ureterocalyceal anastomosis.

Experience with 10 cases of ureterocalyceal anastomosis is reported. Most cases involved scleroatrophic scarring of the pelvis after repeated stone surgery, and 1 case each for failed pyeloplasty, tuberculous stricture of the pelvis, transitional cell carcinoma of the pelvis and calyces, and ureteropelvic junction obstruction associated with renal malformation. Three patients had a solitary kidney. End-to-end ureterocalyceal anastomosis was performed in 5 patients; laterolateral in 1 case, and ureteropyelocalyceal anastomosis in the remaining 4. In 3 cases omentoplasty was also performed.

Ureteral motility after renal autotransplantation.

Twelve kidneys were autotransplanted to the internal iliac vessels in 11 patients for the treatment of renovascular hypertension. The ureter had been left unresected in all but one case, where a transuretero-ureterostomy was performed. A radiographic study of the ureteral motility was obtained during the postoperative period recording images every 1.5 s with a spot-camera system. The ureters appeared to follow different courses, due to the different surgical procedures. In all but two cases the transport of urine was obtained through separate boluses, frequency and direction of peristalsis were within normal ranges, and significant dilation was never observed.

Loss of substrate binding capacity of the hepatic microsomal cytochrome P-450 in Fasciola hepatica infected rats: toxicological implications.

Experimental fascioliasis in the rat is responsible for a dramatic decrease in the drug-metabolizing ability of the hepatic monooxygenase system. The present investigation, through a spectroscopic study of hexobarbital interaction with microsomal cytochrome P-450 and in vitro and in vivo studies of hexobarbital metabolism in the rat, demonstrates that this decrease is due to an alteration in the structure of the hemoprotein (loss of substrate binding capacity of cytochrome P-450 followed by denaturation). These results might be responsible for a decreased safety margins for those flukicidal agents that are detoxified by the monooxygenase pathway, and might explain the accumulation problems frequently associated with chemotherapy of Fasciola hepatica.

Surgical treatment of retroperitoneal fibrosis with omentoplasty.

The main goal of surgery for retroperitoneal fibrosis is to free the ureters from sclerotic tissue and pressure them from further extension of the disease; the omentum may be used as an efficient barrier against recurrence of fibrosis. We present 11 patients with idiopathic retroperitoneal fibrosis treated with ureterolysis and omentoplasty - in four patients the procedure was performed bilaterally, in seven the omentum was used to protect only one ureter, but in six there was a surgical or functional solitary kidney. Out of 15 ureters, residual dilatation was observed only in two, with marked clinical improvement in all patients. Mean follow-up was 51 months.
