Idiopathic constipation by colonic dysfunction. Relationship with personality and anxiety.

The personality of two groups of constipated women (by delayed colonic transit or by colonic inertia) was compared to that of two control groups of arthritic patients (rheumatoid or degenerative disease) with the Minnesota Multiphasic Personality Inventory (MMPI). All subjects suffered from chronic pain. Constipated women were found to have significantly higher scores on the hypochondria, hysteria, control, and low back pain scales and a lower score on the masculinity-femininity scale. Discriminant analysis permitted us to sort out constipated from arthritic patients in 83% of the cases, on the basis of only the personality data. In women with constipation by delayed colonic transit, multiple regression analysis demonstrated a close link (r = 0.90; P less than 0.001) between transit time in the ascending colon and levels of anxiety. It is concluded that women with constipation of colonic origin have a different pattern of personality than arthritic women and that severe constipation may play the role of a defense mechanism, where psychophysiologic responses to life stresses replace normal emotional reactions.

Late sequelae of juvenile rheumatoid arthritis of the hip: a follow-up study into adulthood.

A 4-13 year follow up study of 29 patients with juvenile rheumatoid arthritis into adulthood revealed several structural deformities of the hips: coxa magna, short femoral neck, subluxations associated with large cyst-like erosions of femur and acetabulum near the ligamentum teres and a flattened, wide femoral head (without steroid treatment). The deformities evoke the possibility of ischemia of the femoral head in the presence of active, compressive synovial proliferation during the growth period in a joint with largely intra-articular nutrient vessels.

Familial articular chondrocalcinosis in Quebec.

The existence of articular chondrocalcinosis was documented in 9 members of 3 generations of a Quebec family. No associated or secondary forms of the disease were found. The clinical manifestations appeared early in life, and extensive radiologic involvement was apparent. We determined that genetic transmission was dominant, either autosomal or sex-linked, and not related to the HLA system.

[Juvenile rheumatoid arthritis and HLA-B27].

Forty children with juvenile rheumatoid arthritis were studied to determine the frequency of the histocompatibility antigen HLA [human leukocyte antigen)-B27 in this disease and to characterize the arthropathy associated with this antigen. HLA-B27 was detected in four patients (10%). Its presence was associated in a statistically significant manner with sacroiliitis demonstrated radiologically and with a greater age at the time symptoms in the joints first appeared; this age was, on average, 10 years, compared with 6.29 years for the children without HLA-B27.

Chinese acupuncture.

Acupuncture has been described as it is practiced in the People's Republic of China. To understand fully the preoccupation of the Chinese with this mode of treatment one must be familiar with its economic, historical and cultural background. Since conditions in Western countries are totally different from those in China it is obvious that acupuncture cannot be transplanted in toto from country to country, but that it requires reassessment of its usefulness as a treatment modality. Although further basic and clinical studies must be carried out, it would appear at the moment that acupuncture may have a useful place as yet and additional means of treating pain, especially in areas in which present treatment is less than satisfactory. At the moment no clear indication is obvious for acupuncture anaesthesia in countries in which this specialty is well developed and where adequate numbers of trained anaesthetists are available.

Acupuncture Therapy

[An unusual dissecting cyst of the knee in a patient with rheumatoid arthritis].

An unusual synovial cyst of the knee in rheumatoid arthritis is reported. The communication with the knee joint anteriorly under the tibial insertion of the internal collateral ligament is in contrast with all the cases reported to date in the literature. The advantages of the air arthrogram (coupled with the arteriography) over the opaque contrast medium arthrography are shown to be superior for such special case.
