Diet and nasopharyngeal carcinoma.

Nasopharyngeal carcinoma (NPC) is a disease with a remarkable racial and geographical distribution. It is very rare (incidence of less than 1 per 100,000 person-years) in most parts of the world and only a handful of populations are known to deviate from this low-risk profile, which include people of southern China. Eskimos and other natives of the Arctic region, natives of southeast Asia, and mainly Arab populations of north Africa and Kuwait. There is now convincing evidence implicating dietary factors as the primary cause of NPC among Chinese. A series of case-control studies conducted in various Chinese populations with distinct risks of NPC, ranging from the very high-risk Cantonese to the relatively low-risk Northern Chinese, have suggested that ingestion of salted fish and other kinds of preserved foods by the Chinese constitutes the most important cause of NPC development among these people. Preliminary data on Malays in southeast Asia, Eskimos in Alaska, and Arabs of north Africa also suggest that ingestion of preserved foods by these population groups may be responsible for their raised incidence of NPC.

Regardless of race and geography, the commonest form of nasopharyngeal cancers are those that arise from the epithelial cells lining the nasopharynx. These carcinomas (commonly referred to as NPCs) constitute 75-95% of nasopharyngeal cancers in low-risk populations and virtually all nasopharyngeal cancers in high-risk populations (Ho, 1971; Sugano et al, 1978; Levine and Connelly, 1985).

A case-control study of occupational and dietary factors in colorectal cancer in young men by subsite.

With the hope that exposures responsible for colorectal cancer might be especially obvious among those in whom it develops early, 147 men with colorectal carcinomas first diagnosed between the ages of 25 and 44 years were compared to neighborhood controls. Physical activity on the job was protective for tumors located in the transverse and descending portions of the colon. Rectal cancer and to a lesser extent sigmoid cancers were associated with jobs in which dusts or fumes were inhaled, especially if those jobs were held for long periods in young adulthood. While risk for rectal cancer did not seem to be limited to any particular type of dust or fume, the excess risk was strongest for wood and metal dusts. Consumption of fruits and vegetables and a preference for whole grain breads were protective for colon but not rectal cancers, while consumption of deep fried foods and barbecued/smoked meats increased risk at specific subsites. Beef intake, alcohol consumption, and cigarette smoking appeared to play little or no role at any subsite.

Diet and nasopharyngeal carcinoma: a case-control study in Guangzhou, China.

We conducted interviews on 306 histologically confirmed incident cases of nasopharyngeal carcinoma (NPC) occurring in residents of Guangzhou City, China, who were under the age of 50 years, and an equal number of age-, sex-, and neighborhood-matched controls. We also interviewed 110 mothers of patients under age 45 years and 139 mothers of controls who were matched to patients under age 45 years, to obtain information on childhood exposures of study subjects. Exposure to salted fish, whether in adulthood or in childhood, was significantly associated with an increased risk of NPC. The association was strongest for exposure during weaning, and exposures at all other periods were no longer related to NPC risk after adjustment for exposure during weaning. Besides salted fish, childhood exposure to 5 other preserved foods (fermented fish sauce, salted shrimp paste, moldy bean curd, and 2 kinds of preserved plum) was significantly related to NPC, and the effects were independent of salted fish exposure. Finally, cases ingested significantly less fresh vegetables and fruits than controls, especially during early childhood, and the differences were not explained by their differing consumption pattern of salted fish and other preserved foods.

Hepatitis B virus, aflatoxins, and hepatocellular carcinoma in southern Guangxi, China.

We examined the roles of the hepatitis B virus and aflatoxin B1 in the development of primary hepatocellular carcinoma (PHC) in a cohort of 7917 men aged 25 to 64 yr old in southern Guangxi, China, where the incidence of PHC is among the highest in the world. After accumulating 30,188 man-yr of observation, 149 deaths were observed, 76 (51%) of which were due to PHC. Ninety-one% (69 of 76) of PHC deaths were hepatitis B surface antigen (HBsAg) positive at enrollment into the study in contrast to 23% of all members of the cohort (RR = 38.6). Three of the four patients who died of liver cirrhosis also were HBsAg positive at enrollment. There was no association between HBsAg positivity state and other causes of death. Within the cohort, there was a 3.5-fold difference in PHC mortality by place of residence. When estimated aflatoxin B1 levels in the subpopulations were plotted against the corresponding mortality rates of PHC, a positive and almost perfectly linear relationship was observed. On the other hand, no significant association was observed when the prevalence of HBsAg positivity in the subpopulations was compared with their corresponding rates of PHC mortality.

Diagnostic radiography as a risk factor for chronic myeloid and monocytic leukaemia (CML).

This interview study included 136 Los Angeles County residents aged 20-69 with CML diagnosed from 1979 to 1985 (cases) and 136 neighbourhood controls. During the 3-20 years before diagnosis of the case, more cases than controls had radiographic examinations of the back, gastrointestinal (GI) tract and kidneys, and cases more often had GI and back radiography on multiple occasions (odds ratio (OR) for back X-rays on five or more occasions = 12.0; P less than 0.01). Published estimates were used to assign a minimum dose to the active bone marrow for various radiographic procedures. ORs were estimated for cumulative marrow doses for each of four time periods (3-5 years, 6-10 years, 11-20 years and 3-20 years before the diagnosis of the case). The ORs for exposure to 0.99, 100-999, 1000-1999 and greater than or equal to 2000 mrad in the 3-20 years before diagnosis were 1.0, 1.4, 1.6 and 2.4 (P for highest exposure category and P for trend both less than 0.05). The association was strongest for the period 6-10 years before diagnosis, and the effects of radiation exposure during this period remained significant after consideration of other risk factors in a logistic regression analysis.

Induction of malignant nasal cavity tumours in Wistar rats fed Chinese salted fish.

Epidemiological evidence has implicated Chinese salted fish as a human nasopharyngeal carcinogen. In the present study, 221 Wistar-Kyoto rats aged 21 days were randomly assigned to one of three experimental groups. Rats in group 1 (high dose group) were fed a powder diet of one part Chinese salted fish to three parts certified rat chow during the first 18 months. Similarly, rats in group 2 (low dose group) were fed a powder diet of one part salted fish to five parts rat chow for 18 months. Rats in group 3 were given rat chow only throughout the 3-year experiment. Four malignant tumours of the nasal cavity were observed among rats fed the experimental diets (three and one respectively in the high and low dose groups). No comparable tumours were observed in controls, compatible with the historical control rate of zero. Our results, therefore, further strengthen the hypothesis that Chinese salted fish is a human nasopharyngeal carcinogen; they also establish Wistar rats as a viable animal model for carcinogenicity studies of this food in the laboratory.

Personal and family history of lung disease as risk factors for adenocarcinoma of the lung.

To identify risk factors for adenocarcinoma of the lung, a population-based case-control study of 336 female cancers of this cell type and an equal number of neighborhood controls was conducted between 1983 and 1986. After adjusting for personal smoking, personal and family histories of lung disease emerged as additional risk factors. A personal history of any lung disease was associated with a 40% increase in risk [smoking adjusted relative risk (SARR) = 1.4, 95% confidence interval (CI) = 1.0, 2.0] with a more marked increase in risk for lung diseases occurring during childhood (SARR = 1.9, 95% CI = 1.2, 3.2) and for tuberculosis (SARR = 10.0, 95% CI = 1.1, 90.1). Family histories of tuberculosis (SARR = 2.0, 95% CI = 1.1, 3.6) and of lung cancer (SARR = 3.9, 95% CI = 2.0, 7.6) were also risk factors for adenocarcinoma of the lung. Increasing risk was observed with decreasing intake of dietary beta-carotene. After adjusting for personal smoking, women in the lowest quartile of intake showed a two-fold increased risk relative to those in the highest quartile of intake (P = 0.003). There were also some suggestive differences between cases and controls in their reproductive history and hormone use.

Risk factors for breast cancer in Chinese women of Beijing.

A self-administered questionnaire was completed by 497 Chinese women in Beijing with histologically confirmed breast cancer and by an equal number of age- and neighborhood-matched control women. High body weight (71 + kg) was a risk factor for breast cancer in women aged 50 + (RR = 1.90), but this effect did not quite reach statistical significance. Nulliparity and late age at first birth were associated with an elevated risk of breast cancer. In comparison to parous women who had their first child before age 20, those who delayed this event until after age 29 had an RR of 1.65. The comparable RR for nulliparous women was 3.72. Late age at menopause was a risk factor for breast cancer. The RR for menopause after age 50 compared to menopause before age 45 was 2.16. High parity and long duration of lactation were protective factors against breast cancer. These 2 factors were highly correlated with each other and also with age at first birth. The effect of each on breast cancer risk was somewhat reduced after adjustment for the other two. A personal history of benign breast disease (RR = 3.21) and a family history of breast cancer (RR = 2.17) were also associated with an increased risk of breast cancer.

Tobacco, alcohol, diet, occupation, and carcinoma of the esophagus.

Information on occupation, smoking, food and beverage consumption, and medical history were compared between 275 incident cases of carcinoma of the esophagus and 275 neighborhood controls who were matched to the cases on age (within 5 years), race, and sex. Tobacco use, mainly cigarette smoking, was a significant risk factor for carcinoma of the esophagus. Ex-smokers of cigarettes showed a reduced risk relative to those who continued to smoke, and current smokers of two or more packs per day displayed a higher risk than those who smoked less. Alcohol consumption was another significant risk factor for carcinoma of the esophagus; there was a highly significant trend with average daily dose of ethanol. Relative to controls, cases also consumed significantly more fried bacon or ham, less fresh fruits and raw vegetables, and were more likely to prefer white than whole grain bread. Finally, there was a significant association between carcinoma of the esophagus and long-term occupational exposure to metal dust; this association was largely confined to the lower one-third section of the esophagus.

Risk factors for breast cancer in Chinese women in Shanghai.

Five hundred thirty-four histologically confirmed incident cases of breast cancer in Chinese women of Shanghai and an equal number of age and sex-matched population controls were interviewed as part of an epidemiological study of breast cancer risk factors. Early age at menarche was positively associated with breast cancer risk whereas early age at first full term pregnancy, high parity, and long duration of nursing were each negatively associated. We found high average body weight to be a risk factor, especially among women over age 60. Use of oral contraceptives after age 45 also was a risk factor, but use in general was not. Personal history of benign breast disease and history of breast cancer in first degree female relatives both increased risk. Multivariate analysis showed that each of these risk (or protective) factors was independently related to breast cancer. In addition to confirming most of the breast cancer risk factors of Western populations in a low risk developing Asian country, this study demonstrates a clear beneficial effect on breast cancer risk of lactation in a population characterized by a long cumulative duration of nursing in the majority of women. Finally, this study supports several other recent reports of a residual and beneficial effect of parity on breast cancer risk after controlling for age at first full term pregnancy.

Preserved foods and nasopharyngeal carcinoma: a case-control study in Guangxi, China.

One hundred twenty-eight mothers of nasopharyngeal carcinoma (NPC) cases under age 45 in Yulin Prefecture, China and 174 mothers of population controls were interviewed as part of an epidemiological study to examine childhood exposures in relation to the development of NPC. Exposure before age 2 years to a number of fermented foods was a risk factor for NPC. During weaning, intake of salted fish [relative risk (RR) = 2.6, one-sided P (P) = 0.01], salted duck eggs (RR = 5.0, P = 0.03), salted mustard green (RR = 5.4, P = 0.03), and chung choi (RR = 2.0), P = 0.003), a kind of salted root, was significantly related to an increased risk of NPC. Between ages 1 and 2 years, intake frequency of dried fish [P for linear trend test (linear trend P) = 0.002], fermented black bean paste (linear trend P = 0.0009), and fermented soy bean paste (linear trend P = 0.007) was also positively associated with NPC. A multivariate analysis of these different foods showed all except fermented black bean paste to be independently related to NPC.

Myelogenous leukemia and electric blanket use.

In a case-control study of adult acute and chronic myelogenous leukemia in Los Angeles County, we tested the hypothesis that excess exposure to electromagnetic fields from electric blankets was associated with risk of leukemia. We did this by studying 116 cases of acute myelogenous leukemia (AML) and 108 cases of chronic myelogenous leukemia (CML) along with matched neighborhood controls. The cases and controls were queried as to electric blanket use and the risks computed. For AML the risk was 0.9 (95% CI 0.5-1.6) and for CML the risk was 0.8 (95% CI 0.4-1.6). Cases did not differ from controls by duration of use, year of first regular use, year since last use, or socioeconomic status. Our best estimates of exposure indicate that electric blanket use increases overall exposure to electric fields by less than 50% and magnetic fields by less than 100%. We conclude that there is no major leukemogenic risk associated with electric blanket use in Los Angeles County.

Intake of Cantonese-style salted fish as a cause of nasopharyngeal carcinoma.

Ho (1971) of Hong Kong first proposed consumption of Cantonese-style salted fish, a traditional food among southern Chinese, as a possible risk factor for nasopharyngeal carcinoma (NPC) in this high-risk population. Four case-control studies have examined Ho's hypothesis; all results are supportive. The most convincing evidence of a causal association between intake of salted fish and NPC derives from a recent case-control study of young Hong Kong Chinese. It is estimated that over 90% of NPC cases under age 35 in Hong Kong are due to intake of this food during childhood. Preliminary experimental data on Cantonese-style salted fish indicate that N-nitroso compounds may be involved in the carcinogenicity of this human food.

Pancreas cancer and smoking, beverage consumption, and past medical history.

Four hundred and ninety pancreas cancer patients representative of confirmed cases in Los Angeles County residents of working age were compared to healthy controls individually matched by age, sex, race, and neighborhood. Home interviews were conducted on occupation, smoking, food and beverage consumption, and medical history. Cigarette smoking was a strong and consistent predictor of pancreas cancer occurrence; the effect disappeared after a decade of nonsmoking, and there was no increase in risk among current smokers as daily dose increased. There was no link between pancreas cancer and past consumption of tea, carbonated beverages, beer, or spirits; and an association with coffee consumption was inconsistent. A strong association between pancreas cancer and history of subtotal gastrectomy at any past time could not be explained by chance or any other factor. Pancreas cancer patients had experienced fewer allergies of any kind.
